CLINICAL QUESTION: What is the evidence for antibiotic resistance in acne, and how does resistance affect treatment?
BOTTOM LINE: Use of topical and systemic antibiotics for acne is associated with formation of resistance in Propionibacterium acnes and other bacteria, with clinical consequences. Guidelines recommend resistance reduction strategies including avoidance of antibiotic monotherapy, combination treatment with topical modalities, and limiting the duration of oral antibiotic use.

A 69-year-old man developed Propionibacterium acnes left knee hardware infection after suffering from an infected ingrown toenail. The hardware was removed and he was treated with intravenous vancomycin. Ten days after initiation of vancomycin, he developed severe thrombocytopaenia, epistaxis and petechiae. Vancomycin was discontinued, and platelets rapidly recovered. Serum vancomycin IgG were positive. Patient completed a 6-week course of ceftriaxone with no further complications.

Bioassay-guided extraction and fractionation of the aqueous methanolic extract of the cones of Pinus densiflora (Pinaceae) afforded one new labdane-type diterpene aldehyde, 15-nor-14-oxolabda-8(17),12 E-diene-18-oic acid, along with eight known diterpenes. Their structures were elucidated using spectroscopic methods as well as by comparison with previously reported data. The isolates showed antibacterial (Propionibacterium acnes) and antifungal activities.

This article describes the consensus on the treatment of acne, reached by a Belgian working group. An effective treatment has to rely as much as possible on the pathophysiologic factors: the increased production of sebum, the abnormal desquamation (retention hyperkeratosis) of the epithelium of he sebaceous gland, the proliferation of Propionibacterium acnes and inflammation. The therapeutic arsenal contains topical as well as systemic drugs. This consensus gives an overview of both modalities and an algorithm is presented describing the practical approach to acne treatment.

Recent advances in our understanding of inflammatory skin diseases now afford an opportunity to delve deeper into microbial/host interactions in acne. Agak et al. report that Propionibacterium acnes induces IL-17 expression in peripheral blood mononuclear cells and present new evidence that IL-17+ cells are found in the perifollicular infiltrate of comedones. Additional studies are needed to assess the clinical relevance of IL-17 in acne.

Benzoyl peroxide (BPO) cleansers are commonly prescribed for treatment of acne vulgaris. In fact, they represent approximately half of all BPO prescriptions from dermatology practices. Data are limited on the ability of BPO cleansers to reduce counts of Propionibacterium acnes, impact on reduction and emergence of antibiotic-resistant P acnes strains, and efficacy for facial and truncal acne vulgaris. Morbus Whipple is known as a systemic disease caused by bacteria and inducing the formation of mucopolysaccharides which is absorbed by macrophages. In a mucosal bioptate of a 60 years old patient suffering from this disease we could prove numerous rod-shaped bacteria (Propionibacterium) by electronmicroscopy. In the duodenal secretion aerobic and strictly anaerobic bacteria were to be found. Therapy with tetracycline was followed by a fast and distinct improvement of the clinical symptoms of the absorption disturbances but not so clear of the histological findings.

Low grade endophthalmitis is usually a chronic indolent inflammation that may be misdiagnosed as noninfectious uveitis. Therefore, this form of endophthalmitis is often unsuccessfully treated with corticosteroids. The authors report the results of bacteriologic cultures of the anterior chamber aspirate after cataract surgery. The results were positive for 15 cases (24%). Nine cultures were positive for 27 eyes operated by phacoemulsification and 6 cultures were positive for 35 eyes operated by extracapsular cataract extraction. The origin of these organisms is the normal bacterial flora of the conjunctiva and the inoculum size is very small. The most commonly identified organisms are coagulase-negative staphylococcus and propionibacterium acnes.
