Choriocarcinoma with brain metastasis after term pregnancy: A case report.

RATIONALE: Although the incidence of postpartum choriocarcinoma is extremely low, careful postpartum placental examination, histopathological examination in patients with abnormalities, and blood β-human chorionic gonadotropin (HCG) monitoring in high-risk pregnant women are necessary for early diagnosis of postpartum choriocarcinoma and improvement in prognosis. PATIENT CONCERNS: A 32-year-old woman presented with the chief complaint of postpartum irregular vaginal bleeding for 45 days and coughing and hemoptysis for 7 days. DIAGNOSIS: Clinical findings when combined with her medical histor

[Peripartum cardiomyopathy].

Peripartum cardiomyopathy. Peripartum cardiomyopathy (PPCM) is a dilated cardiomyopathy occurring during or following pregnancy in the absence of pre-existing heart disease. The aetiology is still uncertain. The clinical presentation is similar to that of other etiologies of heart failure. Echocardiography shows the presence of systolic heart failure and helps to monitor the evolution of the CPMP. The basic treatment is symptomatic. New therapeutic perspectives (bromocriptine, immunosuppressant) are being evaluated. Cardiogenic shock and thromboembolic events are the main complications. The ri

Management of Reproductive Disease in Dairy Cows.

Postpartum diseases are common in dairy cows, and their incidence contributes to reduced fertility and increased risk of culling, making their prevention and management extremely important. Reproductive efficiency has a major impact on economic success of any dairy production unit. Optimizing reproductive efficiency contributes to overall efficiency of production units, minimizing environmental impacts and contributing to sustainability of food production. Additionally, control of reproductive diseases is important for maintenance of health and welfare of dairy cows; for minimizing use of anti

Is the cold loop hysteroscopic technique a myometrial sparing treatment for placenta accreta residuals in a puerperal uterus?

Placenta accreta is a life-threatening obstetric pathology characterized by an abnormal invasion of chorionic villi into the uterine wall. The management represents a challenge for the gynecologist, especially in patients desiring to preserve their fertility. Several methods have been proposed to avoid hysterectomy. A case of a hysteroscopic conservative management with the cold loop technique in a puerpera with a large mass of placenta accreta residuals is described. The chorionic tissue was safely detached and it was subsequently removed by an electric cutting loop. Even in the absence of a

Ovarian vein thrombosis--a case report.

OBJECTIVE AND IMPORTANCE: Ovarian vein thrombosis is a rare condition usually seen in the puerperium. The incidence is 0.05-0.18% of pregnancies. Possible complications of ovarian vein thrombosis are sepsis, thrombus extension into the inferior vena cava or renal veins and pulmonary embolism. CLINICAL PRESENTATION: A 31-year-old puerpera complained about abdominal pain in the right lower quadrant and right inguinal region. INTERVENTION: Ultrasonographic examination and multislice computed tomography (MSCT) revealed the right ovarian vein thrombosis. Patient was treated with low-molecular wei

Posterior Reversible Encephalopathy Syndrome in late postpartum eclampsia.

Posterior Reversible Encephalopathy Syndrome (PRES) is a neurological complication associated with several medical conditions and it has been described in clinical findings of seizures, headache, vomiting, altered mental status, and visual changes and focal neurologic deficit, in conjunction with radiological findings of primarily posterior cerebral white matter edema of both cerebral hemispheres. PRES can develop in a wide array situations including pregnancy and postpartum in patients with or without symptoms and signs of eclampsia. A prompt diagnosis of PRES by magnetic resonance imaging an

When should we use diagnostic imaging to investigate for pulmonary embolism in pregnant and postpartum women?

Pulmonary embolism (PE) is a leading cause of death in pregnancy and postpartum. Clinicians face a difficult choice when deciding whether to use diagnostic imaging to investigate for suspected PE in these patients, between risking potentially catastrophic consequences of missed diagnosis if imaging is withheld and risking unnecessary iatrogenic harm to both mother and fetus if imaging is overused. This paper explores the options for imaging and evidence for the use of clinical features, clinical predictions scores or biomarkers to select pregnant and postpartum women for imaging. It also consi

Peripartum cardiomyopathy. An update.

Peripartum cardiomyopathy (PPCM) is a rare disease of the heart muscle that affects women with previously unknown heart diseases during pregnancy or in the first months after delivery. Cardinal symptoms are dyspnoea and fluid retention. However, tachycardia, cardiogenic thromboembolism and other clinical signs of cardiac dysfunction may also herald this uncommon cause of heart failure. The estimated incidence of PPCM shows large regional variations: in Europe and the United States it is between 1:2000 and 1:4000. The markedly higher incidence rates observed in Haitian or South African women (u

[Bell's palsy and cardiomyopathy in the postpartum: case report and review of the literature].

It is a 29-year-old patient who presented in the postpartum a facial palsy. A few days later she developed progressively a worsening dyspnea accompanied by tachycardia. Examinations have showed severe heart failure. Etiological research demonstrated reactivation of Epstein Barr virus infection with facial neuritis and concomitant myocarditis. The patient has fully recovered and was in complete remission after 6 months. To our knowledge this is the first case where a facial palsy and postpartum cardiomyopathy are associated and may be explained by a common viral etiology.

Segmental Darier's disease postpartum and following tubal ligation.

Segmental Darier's disease is a rare clinical variant of autosomal dominant Darier's disease (keratosis follicularis) exhibiting eruptions in a unilateral arrangement following the lines of Blaschko. It occurs in approximately 10% of patients with Darier's disease. We report two cases of type 1 segmental Darier's disease that appeared a few months after childbirth; in one case, recurrence of the disease occurred after tubal ligation.

Listeriosis in pregnancy complicated by postpartum heart block.

BACKGROUND: Listeria monocytogenes is a food-borne pathogen that primarily affects pregnant women. Cardiac involvement is an uncommon complication of infection. We present a case of a gravida with Listeria bacteremia at 36 weeks of gestation. CASE: Two of a patient's blood cultures grew L monocytogenes after she experienced chills, headache, myalgia, and diarrhea. The patient was treated with antibiotics for 48 hours, and then labor was induced, resulting in a normal delivery with a healthy neonate. On day 5 postpartum, the patient developed progressive heart block, resulting in a third-degre

[Puerperal sepsis in the 19th century and Trendlenburg's ligature of the internal iliacal vein].

With the establishment of institutions for child delivery in city hospitals and with incorporation of routine autopsy into the field of activity of assistants at birth, childbed fever spread epidemically and was fatal in most cases. Nearly every third woman in childbed died of puerperal sepsis in the Hôtel Dieu in Paris and in the Erste Wiener Gebärklinik in Vienna. The tragic fate of obstetrician Ignaz Semmelweis began here, he recognized disease transmission from pus and body parts by physicians during the process of delivery. At the end of the 19(th) century puerperal sepsis occurred less f

Call-Fleming postpartum angiopathy in the puerperium: a reversible cerebral vasoconstriction syndrome.

BACKGROUND: The postpartum angiopathy (Call-Fleming syndrome) is a rare, reversible cerebral vasoconstriction syndrome. Unlike in eclampsia, neither proteinuria nor hypertension have been observed in the Call-Fleming syndrome. CASE: A 17-year-old woman developed headache, seizures, confusion, cortical blindness, and denial of visual loss (optic anosognosia, Anton syndrome) on the first postpartum day. Transcranial Doppler demonstrated severely elevated blood velocities. Magnetic resonance angiography showed multifocal segmental narrowing of cerebral arteries. Magnetic resonance imaging reveal

Infectious mononucleosis presenting as postpartum fever.

Infectious Mononucleosis and pregnancy are common conditions seen by obstetricians and family physicians. However, infectious mononucleosis in the postpartum period has not been reported in the literature. A 20 year-old woman presented with a four-day history of fever of 40 degrees C, and chills at her six-week postpartum visit, which prompted an evaluation of the cause of the fever. The atypical presentation of mononucleosis in this patient delayed the eventual diagnosis. Although Infectious Mononucleosis has not been mentioned in the literature as a cause of postpartum fever, it is likely mo

[Epidemiology of anal lesions (fissure and thrombosed external hemorroid) during pregnancy and post-partum].

Thrombosed external hemorrhoids (TEH) and anal fissure (AF) are 2 frequent sources of anal pains during childbirth. We are going to define their incidences as available in publications and in our experience in Bichat hospital. Then we will define their risk factors. According to Martin's and Corby's studies, AF was observed in 10% of the delivered women. In Bichat hospital we performed a proctological assessment to 165 pregnant women during the last third of pregnancy and within the 2 months following delivery. We observed 2 AF (1,2%) during the first period and 25 (15,2%) during the second. R

[Sheehan's syndrome: a recurrent obstetrical complication].

Case report of a 25-year-old primigravida who sustained a necrosis of the anterior pituitary gland after a haemorrhagic Caesarean section. The diagnosis was delayed, as the early symptoms (apathy, anorexia, nausea, orthostatic hypotension and hypoglycaemia) were imputed to more usual causes.
