A performance evaluation of eight geometrically different 10 Fr pediatric arterial cannulae under pulsatile and nonpulsatile perfusion conditions in an infant cardiopulmonary bypass model.

This investigation compared pressure drops and surplus hemodynamic energy (SHE) levels in eight commercially available pediatric aortic cannulae (10 Fr) with different geometries during pulsatile and nonpulsatile perfusion conditions in an in vitro infant model of cardiopulmonary bypass. For each trial, the cannula was placed at the distal end of the arterial line, and the insertion tip was fixed to the inlet of the simulated patient. The pseudo patient was subjected to seven pump flow rates ranging from 400 to 1000 ml/min (at 100 ml/min increments), and the mean arterial pressure was set at a

Arterial and ductus venosus Doppler in fetuses with absent or reverse end-diastolic flow in the umbilical artery: correlation with short-term perinatal outcome.

BACKGROUND: High perinatal morbidity and mortality rates have been reported in association with absent or reverse end-diastolic flow velocities in the umbilical artery. Doppler of fetal venous blood flow might be a helpful instrument for predicting fetal outcome. However, clinical data addressing this issue are rare. Thus, the aim of this study was to evaluate arterial and ductus venosus blood flow velocities as a predictor of short-term outcome in fetuses with umbilical absent or reverse end-diastolic flow velocities. METHODS: Thirty-five high-risk single pregnancies complicated by umbilical

Perfusion is Not Measurably Decreased in Idiopathic Clubfoot.

BACKGROUND: Vascular aberration has been accepted as a potential etiology of clubfoot, and abnormal vasculature has been observed in as high as 85% of children with severe clubfoot. The perfusion index (PI) corresponds to the ratio of pulsatile to nonpulsatile blood flow at a monitoring site and can be used to quantify perfusion of the extremities. The purpose of this study was to use PI to compare the perfusion of clubfeet to controls in order to further assess the role of abnormal vasculature in clubfoot. METHODS: A Masimo Radical 7 Pulse Oximeter (Masimo Corporation, Irvine, California) wa

Color-flow Doppler sonography in Graves disease: "thyroid inferno".

Graves disease is a common diffuse abnormality of the thyroid gland usually characterized by thyrotoxicosis. We performed color-flow Doppler sonography in 16 patients with Graves disease and compared the results with those in 15 normal volunteers and 14 patients with other thyroid diseases (eight with multinodular goiter, four with focal masses, and two with papillary thyroid carcinoma). All 16 Graves disease patients exhibited a pulsatile pattern we call "thyroid inferno." This pattern consists of multiple small areas of intrathyroidal flow seen diffusely throughout the gland in both systole

Color Doppler imaging of the cystic artery.

Color Doppler ultrasonography was used to examine the gallbladder vascular supply in 30 volunteers and in 30 biliary lithotripsy patients pre- and 2 hours posttreatment. A preliminary study of 10 cases of acute cholecystitis was also performed. A cystic artery waveform was obtained in 24 of the volunteers, 18 pre- and postlithotripsy patients, and in 1 of 10 patients with acute cholecystitis. The mean pulsatility indices obtained were 1.04 in the volunteers, 0.98 pre- and 1.02 postlithotripsy (p greater than 0.1). This study shows that the normal gallbladder blood supply can be consistently id

Cerebral Blood Flow under Pressure: Investigating Cerebrovascular Compliance with Phase-contrast Magnetic Resonance Imaging during Induced Hypertension.

BACKGROUND: Induced hypertension is used clinically to increase cerebral blood flow (CBF) in conditions such as vasospasm after subarachnoid hemorrhage. However, increased blood pressure also raises pulsatile force. Cerebrovascular compliance plays a key role in buffering flow dynamics and protecting the microcirculation, but whether it adapts to elevated pressure remains unclear. This study assessed the response of compliant cerebral arteries to induced hypertension in healthy adults using phase-contrast magnetic resonance imaging (PCMRI) and two compliance models: a two-element Windkessel (c

Placental size at gestational week 27 and 37: The associations with pulsatility index in the uterine and the fetal-placental arteries.

INTRODUCTION: Fetal growth restriction is known to be related to decreased fetal and placental blood flow. It is not known, however, whether placental size is related to fetal and placental blood flow. We studied the correlations of intrauterine placental volume and placental-fetal-ratio with pulsatility index (PI) in the uterine arteries, fetal middle cerebral artery, and umbilical artery. METHODS: We followed a convenience sample of 104 singleton pregnancies, and we measured placental and fetal volumes using magnetic resonance imaging (MRI) at gestational week 27 and 37 (n = 89). Pulsatilit

Spontaneous Regression of Parapharyngeal Arteriovenous Malformation.

Arteriovenous malformations (AVMs) are fast-flow vascular malformations that mostly occur in the head and neck region. They are typically progressive and their spontaneous regression is almost never seen. We present a case with pulsatile tinnitus and a parapharyngeal AVM. It resolved completely after diagnostic catheter-based angiography alone.

The Influence of Different Operating Conditions on the Blood Damage of a Pulsatile Ventricular Assist Device.

Because of pulsatile blood flow's benefit for myocardial recovery, perfusion of coronary arteries and end organs, pulsatile ventricular assist devices (VADs) are still widely used as paracorporeal mechanical circulatory support devices in clinical applications, especially in pediatric heart failure patients. However, severe blood damage limits the VAD's service period. Besides optimizing the VAD geometry to reduce blood damage, the blood damage may also be decreased by changing the operating conditions. In this article, a pulsatile VAD was used to investigate the influence of operating conditi

Pulmonary Circulation in Advanced Heart Failure and Cardiogenic Shock: State-of-the-Art Review.

The pulsatile nature of blood flow and the hydrostatic effect of pulmonary capillary wedge pressure are 2 fundamental, yet often overlooked features of right ventricular-pulmonary arterial interactions in advanced heart failure and cardiogenic shock. These 2 features (above all others) define both the mechanical forces experienced by the pulmonary arteries, and in turn, the vascular afterload imposed by the pulmonary circulation on the right ventricular. For over half a century, it has been assumed that the pulsatile components of the pulmonary circulation exist in predictable and constant pro

Cross-sectional study of mechanism of arterial pulsation amplitude modulation under ultrasound probe compression: Similarities and differences compared with the oscillometric blood pressure measurement principle.

Arterial pulsation is used to differentiate arteries from veins on ultrasound imaging. However, the mechanism by which the arterial pulsatio … onized. The timing of the maximum amplitude occurrences in both signals coincided. Conversely, when pulsatile flow was stopped by cuff inflation above the systolic pressure, neither arterial pulsation nor probe pressure oscillations were observed. Arteria lusoria aneurysm with truncus bicaroticus: surgical resection without restoring blood supply to the right arm.

Arteria lusoria, an aberrant or anomalous right subclavian artery, is the most common anomaly of the aortic arch. It may be associated with other congenital anomalies of the heart and great vessels-including, rarely, truncus bicaroticus (a common trunk of both common carotid arteries), and, even more rarely, aneurysmal formation.Herein, we report the case of a 72-year-old man who had both an atherosclerotic aneurysm of an aberrant right subclavian artery and truncus bicaroticus. We resected the aneurysm through a posterolateral thoracotomy and did not restore the distal pulsatile blood supply

Ultrasound Tissue Pulsatility Imaging Suggests Impairment in Global Brain Pulsatility and Small Vessels in Elderly Patients with Orthostatic Hypotension.

BACKGROUND: Orthostatic hypotension (OH) is highly prevalent in the elderly, and this population can be exposed to serious complications, including falls and cognitive disorders, as well as overall mortality. However, the pathophysiology of OH is still poorly understood, and innovative methods of cerebral blood flow (CBF) assessment have been required to accurately investigate cerebrovascular reactivity in OH. OBJECTIVES: We want to compare brain tissue pulsatility (BTP) changes during an orthostatic challenge in elderly patients over 80 with and without OH. MATERIALS AND METHODS: Forty-two

Oligohydramnios and fetal cerebral blood flow.

Blood flow velocity waveforms in the fetal internal carotid artery were recorded in five pregnancies complicated by prolonged severe oligohydramnios due to bilateral renal agenesis. End-diastolic flow velocity was reduced, absent or even reversed, resulting in a raised pulsatility index in all five cases. These data suggest that prolonged severe oligohydramnios may hamper cerebral blood flow through fetal head compression.

Characterization of hepatic, portal, and renal venous flow patterns by Doppler ultrasound across tricuspid regurgitation grades.

INTRODUCTION AND OBJECTIVES: Hepatic, portal, and intrarenal venous flows are impaired in significant tricuspid regurgitation (TR), but the impact of massive and torrential TR remains unclear. This study assessed these venous flow patterns across the 5-grade TR classification and their potential as grading markers. METHODS: Patients with TR were prospectively included from 3 centers. Exclusion criteria were admission for heart failure, cirrhosis, and stage V renal disease. TR severity was classified using biplane vena contracta width and 2-dimensional effective regurgitant orifice area. Venou

CSF and venous blood flow from childhood to adulthood studied by real-time phase-contrast MRI.

PURPOSE: In vivo measurements of CSF and venous flow using real-time phase-contrast (RT-PC) MRI facilitate new insights into the dynamics and physiology of both fluid systems. In clinical practice, however, use of RT-PC MRI is still limited. Because many forms of hydrocephalus manifest in infancy and childhood, it is a prerequisite to investigate normal flow parameters during this period to assess pathologies of CSF circulation. This study aims to establish reference values of CSF and venous flow in healthy subjects using RT-PC MRI and to determine their age dependency. METHODS: RT-PC MRI was

Relationship of pulsatility and resistance indices to cerebral arteriovenous malformation angioarchitectural features and hemorrhage.

The role that hemodynamics plays in the pathophysiology of cerebral arteriovenous malformation (AVM) hemorrhage remains unclear. Here, we examine the relationship of pulsatility and resistance indices to AVM angioarchitectural features and hemorrhage. Records of patients with cerebral AVMs evaluated at our institution between 2007-2014 and with flows obtained before treatment using quantitative magnetic resonance angiography (QMRA) were retrospectively reviewed. Flow volume rate and flow velocity were measured in primary arterial feeders and compared to their contralateral counterparts. Pulsat
