Applanation tonometry is a non-invasive method of assessing the arterial blood pressure profiles in both the peripheral and systemic circulation. Thirty patients of pemphigus have been treated with Dexamethasone Cyclophosphamide Pulse (DCP) in our hospital in the past three years. Of these twelve patients are off all treatment and are in complete remission while the rest are in different phases of the treatment and responding quite well to the pulse therapy. Only one case of relapse has been experienced till date.

Dexamethasone-cyclophosphamide pulse (DCP) is the prefered mode of therapy in pemphigus in India because it is relatively free from the side effects seen with heavy doses of daily oral steroids. One hundred forty-six pemphigus patients treated with DCP were observed for side effects of this regimen. The efficacy of PTH suppression of oral 1,25 (OH) (2)D(3) pulse therapy was greater in the group with well-controlled phosphate levels (mean serum P<6.0 mg/dl) in hemodialysis patients with secondary hyperparathyroidism. The restriction of dietary phosphate can decrease the level of serum phosphate without increasing the dose of phosphate binders and continue the treatment without increasing Ca x P product. The dietary phosphate reduction is important in oral 1,25 (OH) (2)D(3) pulse therapy.

Eight patients with neuroimmunological disorders refractory to conventional immunotherapies were given totally 17 courses of pulse-dose cyclophosphamide. In this report, we describe two pacemaker dependent patients with outer-coil fractures in their bipolar ventricular leads who averted potentially catastrophic consequences because of a new safety feature incorporated into their pulse generators (Intermedics Cosmos II, model 284-05).
