The results of pyloric manometric studies conflict, some showing evidence of a sphincter at the pylorus, others not: hence the term 'the fickle pylorus'. In this study we used perfused catheters to investigate the manometric properties of the human pylorus, in an attempt to explain the different results of similar experiments. Bariatric surgery is more effective in the management of morbid obesity and related comorbidities than conservative therapy. There are two main groups, restrictive and malabsorptive procedures. Laparoscopic gastric plication with pylorus-preserving loop duodenoileal bypass is classified into the latter group. Coronary artery bypass grafting using right gastroepiploic artery and pylorus-preserving pancreaticoduodenectomy are both well known and commonly performed procedures independently. However, pylorus-preserving pancreaticoduodenectomy after coronary artery bypass grafting using right gastroepiploic artery has not been reported in the literature. We report the first case with operative demonstration of pylorus-preserving pancreaticoduodenectomy in a patient who had undergone coronary artery bypass grafting using an in situ right gastroepiploic artery graft.
