Periapical diseases ranges from mild granulomatous lesions to large cystic ones, with the treatments corresponding to their respective pre-operative diagnoses. However, the determination of cause of periapical radiolucency is impossible on pre-operative clinical and radiographic examinations. We present a case highlighting the difficulties encountered in treating a periapical cyst using the current evidence in literature. Alpha-1-antitrypsin (AAT) is a serum protease inhibitor, with a well established role in bodily defense against destructive inflammatory disease. In order to investigate the role of this protease inhibitor in the periapical lesions, 13 patients were investigated. Serum alpha-1-antitrypsin levels of 13 patients were 871.8 +/- 73.6 mg/dl whereas 296.6 +/- 32.6 mg/dl in the control group. Difference was important between both groups (P less than 0.01). Intradural spinal arachnoid cysts are a relatively uncommon lesion that may be either intra, or extradural, and intradural spinal arachnoid cysts are even less common. Arachnoid cysts are cerebrospinal fluid collections in the spine that can present with neurological symptoms. We report a case of large radicular cyst progression related to endodontic origin to emphasize proper intervention and follow-up for endodontic pathosis. A 25 yr old man presented with an endodontically treated molar with radiolucency. He denied any intervention because of a lack of discomfort. Five years later, the patient returned. The previous periapical lesion had drastically enlarged and involved two adjacent teeth. Cystic lesion removal and apicoectomy were performed on the tooth. Immunostaining with monoclonal antibodies was used to study and compare the cytokeratin content of odontogenic cysts and normal gingival epithelium. BACKGROUND: Interdural spinal cysts are rare lesions located between the layers of the dura mater, not included in traditional spinal cyst classifications. Only 12 cases have been reported in the literature, with two located in the lumbar region.
