Use of an Amplatz dilator to facilitate transrectal abscess drainage in children.

Image-guided transrectal drainage is an effective treatment option for many deep pelvic abscesses in children. Currently, a variety of imaging and catheter techniques are used to access and drain these abscesses. The present report describes an approach for introducing the transrectal catheter by using transabdominal ultrasound guidance to direct transrectal advancement of an Amplatz dilator with coaxial placement of the metal cannula and inner trocar from a pigtail catheter set, followed by placement of the pigtail catheter via Seldinger technique. This approach eliminates some of the limitat

Interstitial laser thermotherapy for liver tumours.

BACKGROUND: Primary hepatocellular carcinoma (HCC) and metastases from colorectal cancer are the most common malignant liver tumours. Surgical resection is the optimum treatment in suitable patients. Interstitial laser thermotherapy (ILT) is gaining acceptance for the treatment of irresectable liver tumours and as a potential alternative to surgery. An understanding of the principles of therapy and review of clinical outcomes may allow better use of this technology. METHOD: An electronic search using the Medline database was performed for studies on the treatment of hepatic malignancy publish

Impact of Intravascular Ultrasound-Guided Percutaneous Coronary Intervention on Long-Term Clinical Outcomes in Patients Undergoing Complex Procedures.

OBJECTIVES: This study sought to determine whether intravascular ultrasound (IVUS) guidance compared with angiographic guidance reduces long-term risk of cardiac death in patients undergoing complex percutaneous coronary intervention (PCI). BACKGROUND: Although IVUS is a useful tool for accurate assessment of lesion profiles and optimal stent implantation, there are limited data on long-term clinical outcomes between IVUS-guided and angiography-guided PCI for patients with complex lesions. METHODS: From March 2003 through December 2015, a total of 6,005 patients undergoing PCI for complex le

Transnasal X-Ray-Guided Biopsy for a Clival Lesion.

Skull base biopsies are technically challenging due to complex anatomy and proximity to vital structures. Traditional transcervical and transoral approaches offer limited access to the clivus and risk poor depth perception and complications. Although transnasal techniques have improved access, x-ray-guided methods are rarely reported. We describe the first transnasal x-ray-guided clival biopsy in a middle-aged patient with suspected skull base osteomyelitis. Preoperative CT, MRI, and PET/CT enabled precise targeting and safe trajectory planning. In a biplane angiographic suite, a guidewire was

Evolving operator practices reduced patient radiation dose in interventional cardiology: Trends in a single center.

BACKGROUND: This retrospective study addresses the role of operator and fluoroscopy equipment in reducing patient radiation exposure in the Cath lab. METHODS: Data from 99,400 procedures performed in our institution between 2007 and 2019 were reviewed. Dosimetric parameters included reference point air kerma (Ka,r), Kerma Area Product (PKA), fluoroscopic time, and contrast volume. Results are characterized by their 50th and 99th percentiles. Data from a subset of fluoroscopes that were in continuous use and a subset of operators who used the same "continuous" fluoroscope in every year of the

CT-guided paramaxillary glossopharyngeal nerve block: A novel and potentially safer approach for diagnostic confirmation and temporary treatment of glossopharyngeal neuralgia.

The glossopharyngeal nerve block is an interventional technique utilized for head and neck pain management in the setting of glossopharyngeal neuralgia, dental procedures, awake endotracheal intubations, and post-pharyngeal surgery. This procedure is traditionally performed with landmark technique or ultrasound-guided techniques, all of which are associated with potential risks and complications for the patient. We report the novel technique of a CT-guided paramaxillary approach glossopharyngeal nerve block, which has not been previously reported, eliminates the trans-parotid trajectory, and r

Percutaneous Sclerotherapy of Venous Malformations.

Venous malformation (VM) stands as the most prevalent form of vascular malformation, characterized by its diverse morphology. These lesions can manifest in any part of the body, affecting different tissue planes and giving rise to symptoms such as pain, swelling, or physical dysfunction. In the realm of treatment, direct puncture VM sclerotherapy holds its place as the primary approach. This technique involves the administration of a sclerosing agent into the VM channels during contrast phlebography while simultaneously managing the outflow veins through different methods. The process of VM sc

Pediatric interventional neuroradiology: "How I do it" vol. II-Embolization.

Pediatric arteriovenous shunts can be challenging to embolize and the consequences of excess venous penetration disastrous. Moreover, the congenital nature of most shunts necessitates neonatal intervention whenever (medically refractory) heart failure occurs. Here, we describe current endovascular treatment strategies based on personal experience and literature review. While disparate, arteriovenous malformation, dural arteriovenous fistula (AVF), pial AVF, and vein of Galen aneurysmal malformation share unifying features such as high output heart failure and venous hypertension. Fluoroscopic-guided Lumbar Subarachnoid Drain Placement: A Technical Report.

Spinal cord ischemia is a serious complication associated with major endovascular and open aortic repairs. Placement of prophylactic lumbar subarachnoid drains to prevent spinal cord ischemia is a common practice. Traditionally, anesthesiologists placed these drains using landmark techniques or consulted radiologists for fluoroscopic guidance for challenging drain placements. To overcome logistical challenges and improve success rates, vascular anesthesiologists at the authors' institution began routinely using fluoroscopy for lumbar drain placements in 2018. Over 6 years, this dedicated group

A Novel Catheterization Laboratory Radiation Shielding System: Results of Pre-Clinical Testing.

BACKGROUND: This pre-clinical study evaluated the efficacy of a novel shielding system to reduce scatter radiation in the cardiac catheterization laboratory. METHODS: Using a scatter radiation phantom in a standard cardiac catheterization laboratory, a radiation physicist recorded radiation measurements at 20 reference points on the operator side of the table. Measurements were made with fluoroscopy and cine with the C-arm in the posterior-anterior (PA) and 40 degrees left anterior oblique (LAO) orientations. Scatter radiation doses were compared with and without use of the shielding system.

Shading operators from the Gray: Are novel radiation barriers or changing physician behaviors the best next step?

Lead barriers to reduce operator radiation exposure in the catheterization laboratory are effective. This study of a novel vertical radiation shield suggests significant reduction in operator radiation exposure when used in addition to standard protection methods. Although additional barriers may help reduce radiation exposure, further education and training of operators in radiation safety may be as effective and perhaps more effective than additional barriers.

[Place of interventional neuroradiology in the prevention of hemorrhagic stroke].

The risk of bleeding of unruptured aneurysms is increasing with aneurysm size. Small unruptured aneurysms can be treated conservatively with a regular MRI/MRA follow-up to detect a potential increase in size. If coiling is still the main endovascular technique for the treatment of unruptured aneurysms, several other techniques are now available including stent-assisted coiling, flow diversion, and flow disruption. After ARUBA study, there is no recommendation to treat unruptured brain AVMs. According to their hemorrhagic risk, dural arteriovenous fistulas with cortical venous drainage have to

An interventional radiology technique to treat pharyngeal or esophageal perforation associated with mediastinal abscess in children.

PURPOSE: Pharyngeal or esophageal perforation with mediastinal abscess is notably dangerous in children and can be very difficult to treat. We aimed to determine the safety and efficacy of the transnasal placement of a mediastinal drainage catheter and a nasojejunal feeding tube, with or without gastric decompression, in the treatment of the above perforations in children. METHODS: We placed transnasal mediastinal drainage catheters and nasojejunal feeding tubes in 14 pediatric patients. Patients with esophageal perforation also underwent the placement of a gastric decompression tube. Four of

Improved Dose Estimates for Fluoroscopically Guided Lumbar Epidural Injections.

OBJECTIVE: The goal of the study was to determine the potential impact of system inaccuracies and table attenuation on fluoroscope-reported dose values. DESIGN: An Institutional Review Board-approved study was conducted to collect detailed acquisition and patient exposure data for fluoroscopy-guided lumbar epidural injections. BACKGROUND: System-reported dosimetry values, especially the air Kinetic Energy Released per unit MAss and dose-area product metrics, are routinely used for estimating the radiation burden to patients undergoing fluoroscopy-guided procedures. However, these metrics do
