Efficacy and safety of oral tapentadol for cancer-related pain: an updated systematic review and meta-analysis.

BACKGROUND: This systematic review aims to evaluate the efficacy, safety, and tolerability of oral tapentadol in the management of cancer-related pain in adults. It updates the 2015 Cochrane review on the same topic by including newer trials up to March 2024. METHODS: A systematic search of Medline (via PubMed), the Cochrane Central Register of Controlled Trials, and references from the previous review was conducted. Randomized controlled trials comparing oral tapentadol with placebo or other opioids (e.g., morphine, oxycodone, tramadol/paracetamol) in adults with chronic cancer pain were inc

Sleep Apnea and Stroke: A Narrative Review.

TOPIC IMPORTANCE: Stroke is the second-leading cause of death worldwide. OSA is an independent risk factor for stroke and is associated with multiple vascular risk factors. Poststroke OSA is prevalent and closely linked with various stroke subtypes, including cardioembolic stroke and cerebral small vessel disease. Observational studies have shown that untreated poststroke OSA is associated with an increased risk of recurrent stroke, mortality, poorer functional recovery, and longer hospitalizations. REVIEW FINDINGS: Poststroke OSA tends to be underdiagnosed and undertreated, possibly because

The effectiveness and promising strategies of obesity prevention and treatment programmes among adolescents from disadvantaged backgrounds: a systematic review.

This review aimed to summarize the evidence on the effectiveness of obesity prevention and treatment programmes for adolescents from socioeconomically disadvantaged backgrounds. A secondary aim was to identify potential successful intervention strategies for this target group. PubMed, EMBASE, PsycINFO and Cochrane Library were searched from January 2000 up to February 2016. Intervention studies targeting adolescents from disadvantaged backgrounds were included, with body mass index as outcome. Secondary outcomes were other adiposity measures, physical activity, diet, sedentary behaviour and sc

Preventing tuberculosis with bacillus Calmette-Guérin vaccine: a meta-analysis of the literature.

This article reviews a previously published meta-analysis of 1264 titles or abstracts and 70 selected studies for evaluation of the efficacy of bacillus Calmette-Guérin (BCG) vaccine in preventing tuberculosis. Following review, data from 26 studies were included in the analysis. These 26 studies reveal that vaccination with BCG significantly reduces the risk of tuberculosis by an average of 50%. This level of protection persists across a number of subgroups defined by age at vaccination and study design. Vaccination with BCG was significantly associated with a reduction in the incidence of pu

Advanced trauma life support training for ambulance crews.

BACKGROUND: There is an increasing global burden of disease from injuries. Models of trauma care initially developed in high-income countries are also being adopted in low and middle-income countries (LMIC). Amongst these ambulance crews with Advanced Life Support (ALS) training are being promoted in LMIC as a strategy for improving outcomes for victims of trauma. However there is controversy as to the effectiveness of this health service intervention, and the evidence has yet to be rigorously appraised. OBJECTIVES: To quantify the effectiveness of ambulance crews with ALS training versus cre

When randomization is not random: Allocation bias in small sample, group sequential randomized clinical trials.

Even in rare diseases, where the sample size is limited and blinding is less frequently implemented, randomized controlled trials are considered the gold standard to prove efficacy. Randomization is used to mitigate bias and regulatory guidance recommend the investigation of the impact of bias on the test decision. We quantified how allocation bias affects the test decision in small-sample two-arm group sequential trials under a biasing policy based on the Blackwell-Hodges convergence strategy. Type I error and power were evaluated under Lan-DeMets spending (Pocock-, O'Brien-Fleming-, Wang-Tsi

Cannabinoids in dermatology: a scoping review.

The therapeutic applications of cannabis and cannabinoids are an increasingly conspicuous topic as de-criminalization and legalization of these products continues to expand. A limited number of cannabinoid compounds have been approved for a specific set of conditions. However, the current role of cannabinoids for the treatment of dermatologic conditions remains to be defined. We conducted a review of the current literature to determine the applications of cannabinoids for the therapy of various skin diseases. After conducting our analysis, we found that cannabinoid products have the potential

Efficacy of myofascial induction compared with its simulation on joint amplitude in people with axial spondylarthritis: Protocol of a randomized controlled clinical trial.

BACKGROUND: Axial spondyloarthritis (AxSpA) produces structural changes that cause alterations in body functions. One tissue that seems to have a predictive role in the etiology and progression of the disease is the soft tissue, particularly the fascia. However, little is known about the use of myofascial induction in people with AxSpA, and clinical evidence from physiotherapy regarding potential strategies is limited. OBJECTIVE: To evaluate the efficacy of myofascial induction compared with its simulation on joint amplitude in people with AxSpA. METHODS: In this randomized controlled parall

Analysis of adaptive platform trials using a network approach.

BACKGROUND: Adaptive platform trials allow randomized controlled comparisons of multiple treatments using a common infrastructure and the flexibility to adapt key design features during the study. Nonetheless, they have been criticized due to the potential for time trends in the underlying risk level of the population. Such time trends lead to confounding between design features and risk level, which may introduce bias favoring one or more treatments. This is particularly true when experimental treatments are not all randomized during the same time period as the control, leading to the potenti

Examining heterogeneity in meta-analysis: comparing results of randomized trials and nonrandomized studies of interventions for low back pain.

STUDY DESIGN: Literature review. OBJECTIVE: To assess the influence of various factors in statistical heterogeneity of meta-analyses of interventions for low back pain. One of these factors was study design: randomized controlled trial (RCT) versus nonrandomized study (NRS). SUMMARY OF BACKGROUND DATA: The presence of statistical heterogeneity poses a challenge to the conduct and interpretation of meta-analyses. METHODS: We searched MEDLINE, EMBASE, and The Cochrane Library up to May 2005 for comparative studies of interventions for low back pain. The interventions with the highest number o

Treatment of hiccup during general anaesthesia or sedation: a qualitative systematic review.

BACKGROUND AND OBJECTIVE: Acute hiccup is a minor complication that can occur during sedation or general anaesthesia. The disorder can disturb the surgical field, might interfere with lung ventilation or could hamper diagnostic procedures. The objective was to perform a systematic search for interventions aimed at treating hiccup occurring during anaesthesia or sedation. METHODS: A systematic search for reports describing interventions to treat hiccup in conjunction with anaesthesia was carried out (MEDLINE, EMBASE, Cochrane-Library, manual screening of reference lists and review articles, up

"The unbearable lightness" of the primary end point in clinical trials.

In this Perspective, I discuss the limitations of a soft primary endpoint that is used in some of the recent randomized phase II/III clinical trials. Unfortunately, many clinicians and investigators do not interpret the data critically to recognize the limitations of such findings. I advise against over-interpreting the effects of an intervention on a soft primary endpoint.

Including nonrandomized evidence in living systematic reviews: lessons learned from the COVID-NMA initiative.

BACKGROUND AND OBJECTIVES: Randomized controlled trials (RCTs) are more likely to be included in evidence syntheses of health interventions due to their methodological rigor. However, the integration of nonrandomized studies (NRSs) may be necessary, as was seen during the COVID-19 pandemic due to the emergence of variants of concern. We aimed to examine the body of evidence, randomized and nonrandomized, on COVID-19 vaccine effectiveness (VE) during the emergence of the Delta variant and to share lessons learned from including nonrandomized evidence alongside randomized evidence in the COVID-N

Non-inferiority clinical trials: What they are and when they should be done.

Randomized controlled trials (RCTs) are the gold standard design of studies evaluating novel interventions, pharmaceutical and otherwise. The standard RCT design is a 'superiority' study to assess if the tested intervention is better than existing treatments. However, a new intervention that has additional benefits, such as better adverse effect profile, may be useful even if it is not more effective than existing treatments. In such cases, a non-inferiority (NI) design may be preferable. In NI designs, once NI is demonstrated, sequential analysis for superiority may be done, provided it is pr

Immunotherapy in multiple myeloma: when, where, and for who?

PURPOSE OF REVIEW: Immunotherapy is transforming treatment of multiple myeloma patients in all stages of their disease. This review will discuss recent developments in immunotherapy in multiple myeloma with a focus on antibodies, antibody-drug conjugates, and T-cell-redirection strategies. RECENT FINDINGS: CD38-targeting antibodies have single agent activity in multiple myeloma, and especially when combined with other drugs, are improving the clinical outcome of patients with newly diagnosed or relapsed/refractory multiple myeloma. Also the SLAMF7-targeting antibody, elotuzumab, improves the

Pharmacotherapy of trigeminal neuralgia.

The efficacy of the anticonvulsant drug carbamazepine in the management of trigeminal neuralgia is evidenced in several controlled trials, and the numbers needed to treat to obtain one patient with at least 50% pain relief (NNT) is 1.7. Single small trials have shown that baclofen alone provides pain relief (NNT = 1.4) and that lamotrigine has an additional effect in patients with insufficient relief using carbamazepine or phenytoin (NNT = 2.1). Uncontrolled observations and clinical practice indicate that phenytoin, clonazepam, sodium valproate, gabapentin, and lidocaine will also relieve tri
