Topical Losartan in the Treatment of Stromal Scarring After Refractive Surgery.

PURPOSE: To review current guidelines for the prophylactic and therapeutic use of topical losartan to prevent and treat corneal scarring fibrosis in refractive surgery. METHODS: PubMed was used to search the medical literature. RESULTS: Scarring fibrosis of the cornea is typically associated with injury to, and defective regeneration of, the adjacent overlying epithelial basement membrane that controls transforming growth factor (TGF)-β passage into the stroma, leading to the development and persistence of myofibroblasts. Topical losartan's effects in the cornea are mediated by inhibiting ac

Corneal-Based Surgical Presbyopic Therapies and Their Application in Pseudophakic Patients.

Purpose. The purpose of this review is to provide a summary of laser refractive surgery and corneal inlay approaches to treat presbyopia in patients after cataract surgery. Summary. The presbyopic population is growing rapidly along with increasing demands for spectacle independence. This review will focus on the corneal-based surgical options to address presbyopia including various types of corneal intrastromal inlays and laser ablation techniques to generate either a multifocal cornea ("PresbyLASIK") or monovision. The natural history of presbyopia develops prior to cataracts, and these pres

[Non-freeze epikeratoplasty in recurrent pterygium].

We present five patients with recurrent pterygium. Prior to our surgical procedure the patients had had several surgical interventions as well as radiotherapy. Up to four-fifths of their corneal surface was covered by a multilayered pterygium including symblepharon scarring in the lower and upper lid, completely immobilizing the globe. The pterygium was removed, the conjunctival strains excised, and corneal abrasion was carried out with a rotating diamond tool. Finally, an optical non-freeze epikeratoplasty processed by the Non-freeze Barraquer-Krumreich-Swinger refractive set (BKS-1000) was s

Late-onset traumatic laser in situ keratomileusis (LASIK) flap dehiscence.

PURPOSE: To report a case of laser in situ keratomileusis (LASIK) flap dehiscence following focal trauma six months after uneventful refractive surgery. METHODS: Case report. A 37 year old man was seen one day after a tree branch snapped tangentially against his left cornea causing a dehiscence of his LASIK flap. RESULTS: The flap was repositioned after treating the exposed flap stroma with a 50:50 mixture of distilled water and balanced salt solution. The patient regained 20/20 uncorrected visual acuity. CONCLUSIONS: Patients should be informed about the potential for traumatic flap dehisc

How does the Surgical Repair of Ptosis Impact Refractive Error?

PURPOSE: To identify a correlation between the degree of congenital ptosis and levels of refractive error, visual stimulus deprivation and anisometropic amblyopia, and any improvement with surgical intervention. METHODS: A prospective investigation of 30 patients aged 3 months to 8 years, with a diagnosis of unilateral or bilateral congenital ptosis. A full orthoptic assessment and cycloplegic refraction were performed and patients requiring surgical intervention for ptosis were at surgeon discretion. RESULTS: The mean age at presentation was 27.1 months. The prevalence of amblyopia ranged f

Repeatability and comparability of the Galilei-G4 and Cassini in measuring corneal power and astigmatism in normal and post-refractive surgery eyes.

To assess the repeatability and comparability of the Galilei G4 versus the Cassini topographer in post-refractive eyes and in normal eyes, including older patients representative of an initial cataract evaluation. Simulated keratometric (simK), total corneal and posterior corneal power and astigmatism were evaluated in both post-refractive and normal eyes. Repeatability was measured by calculating within-subject standard deviation (Sw), coefficient of variation (CoV), and intraclass correlation coefficient (ICC). Vector analyses and Bland-Altman plots were employed to assess agreement between

[Phototherapeutic keratectomy. Undesirable effects, complications, and preventive strategies].

The conceptual advantage of phototherapeutic keratectomy (PTK) include the option to remove diseased tissue on a micrometer scale. Although less invasive than other forms of corneal surgery, it remains a surgical procedure which carries inherent risks. In phototherapeutic indications this risk is higher than in otherwise healthy eyes undergoing photorefractive surgery. Potential general complications include wound-healing problems, infection, inflammation, or a recurrence of the underlying disease. More specifically, PTK is associated with unwanted refractive changes, such as a hyperopic shift

Postoperative Astigmatism after Keratoplasty: A Systematic Review Meta-Analysis Based on PRISMA.

Background: The number of corneal transplants is rising, with the aim to treat a spectrum of diseases ranging from dystrophies to corneal opacities caused by trauma or chemical burns. Refractive outcomes after this procedure are often impossible to predict and associated with high levels of astigmatism. However, there are many techniques that affect the reduction of astigmatism and improve the quality of life of patients. Objectives: The aim was to compare the improvement in postoperative visual acuity (logMAR) and amount of corneal astigmatism (Diopters) after corneal keratoplasty in patients

A Questionnaire Study on Patient Perspectives of Sedation for Ophthalmic Procedures.

Introduction Ophthalmic procedures, such as cataract and refractive surgery, are prevalent in the United States and are often associated with significant preoperative anxiety among patients, which can impact patient comfort and surgical outcomes. Sedation is used in the vast majority of these procedures to reduce anxiety and improve patient cooperation. However, there is limited information on patient-reported experiences and perspectives. This study thus aimed to explore the opinions of patients on sedation for their eye procedures. Methods We administered an anonymous Qualtrics survey to gat
