Continued lodging of retinal emboli in a patient with internal carotid artery and ophthalmic artery occlusions.

Internal carotid artery or ophthalmic artery occlusions are devastating ophthalmological events which lead to severe impairment of vision. A case of multiple branch retinal artery occlusions in a 63-year-old male with internal carotid artery and ophthalmic artery occlusions on brain angiography is presented. Emboli lodging in branches of the retinal arteries were bright, glistening, yellow or orange in appearance. Such a distinctive ophthalmoscopic appearance led to the diagnosis of cholesterol emboli. Fluorescein and indocyanine green angiography disclosed delayed filling of the retinal vesse…

Retinal arterial and vein occlusion: is surgery ever indicated?

PURPOSE OF REVIEW: To highlight the recent progression in surgical treatments for central retinal vein occlusion (CRVO) and central retinal artery occlusion (CRAO). RECENT FINDINGS: Anti-VEGF treatment, accepted as a primary treatment for CRVO, is unable to effectively treat all types of the diseases. Regarding CRAO, there are not any accepted therapies available. There have however been recent innovations in surgery, such as utilizing robotics-assisted tools in cannulation procedures for central retinal artery occlusion, or micro-cystotomy for refractory macular edema resulting from ischemic…

Acute changes in the retina and central retinal artery with methamphetamine.

Methamphetamine (METH), an addictive stimulant of neurotransmitters, is associated with cardiovascular and neurological diseases. METH-induc … cell adhesion molecule-1 (PECAM-1) protein expression level was decreased in the retina and central retinal artery (CRA) by METH treatment, along with the endothelial proteoglycans glypican-1 and syndecan-1. Moreover, a regulator of the extracellular matrix, matrix metalloproteinase-14 (MMP-14) in the retina, and MMP-2 and MMP-9 in plasma, were increased by METH …

Central retinal arterial occlusion in a patient with pyoderma gangrenosum.

A 74-year-old male presented to us with a history of vision loss for 36 hours in the right eye (RE). The RE had a visual acuity of hand movements. The fundus revealed a pale retina, cattle tracking in the retinal vessels, and a cherry-red spot at the macula. The patient was a known case of pyoderma gangrenosum (PG) and had received intravenous methylprednisolone and cyclophosphamide at the onset of visual symptoms. An emergency anterior chamber paracentesis was performed following unsuccessful attempts of ocular massage. The patient improved to 6/9 in the RE 4 months after paracentesis. The pa

Branch retinal arterial occlusion associated with toxoplasmic chorioretinitis.

BACKGROUND: Ocular toxoplasmosis can cause a variety of retinal vascular changes including branch retinal arterial occlusion, which is a rare complication of the disease. PATIENT AND METHODS: We report a case of toxoplasmic chorioretinitis in a pregnant woman, who developed branch retinal arterial obstruction adjacent to the active chorioretinitis lesion. RESULTS: The patient received an appropriate steroid and antibiotic treatment and the retinitis lesion resolved over a six-week period. At two months after diagnosis, visual acuity in her right eye was 20/30 and there was a hyperpigmented s

CO2 dependence of retinal arterial and capillary blood velocity.

OBJECTIVE: Blood flow to the brain is extremely sensitive to changes in PCO2. While animal studies show a similar potent PCO2 dependence in retinal and choroidal vessels, the PCO2-retinal blood flow relationship has never been adequately studied in humans. METHODS: Video scanning laser ophthalmoscopy after fluorescein angiography was used to analyze retinal arterial and capillary blood velocity under conditions of mild hypercapnia and hypocapnia. Control conditions (end-tidal PCO2 = 38.3 +/- 0.4 mmHg) were contrasted with hyperventilation-induced hypocapnia (PCO2 = 34.0 +/- 0.4 mmHg) and hype

A case of sudden retinal artery occlusion and blindness in pregnancy.

Central retinal artery occlusion is generally a disease of old age associated with loss of vision in one eye. In older patients, it is usually secondary to emboli from atherosclerotic plaques; in younger patients valvular heart disease is a common precursor. Several other conditions have been associated with central retinal artery occlusion, but to our knowledge, there have been no previously reported cases with pregnancy as the sole associated factor. Such a case is presented.

Digital subtraction carotid angiography and retinal arterial obstruction.

Thirty-one consecutive patients with retinal arterial obstruction were studied with digital subtraction angiography (DSA). Among the 29 patients in whom DSA was technically acceptable, 45% (13/29) demonstrated evidence of carotid stenosis or an atherosclerotic plaque ipsilateral to the eye with the retinal arterial obstruction. In 21% of cases (6/29 patients) the stenosis was 60% or greater. The incidences of carotid atherosclerosis were similar among the central and branch retinal arterial groups.

Unilateral branch retinal vein occlusion and contralateral branch retinal artery occlusion: a case report and comprehensive literature review.

Branch retinal vein occlusion (BRVO) and branch retinal artery occlusion (BRAO) are two distinct yet serious retinal vascular disorders that often present with acute visual impairment. While both conditions are commonly encountered independently, their simultaneous or sequential occurrence in opposite eyes of the same patient is exceedingly rare and poses unique diagnostic and therapeutic challenges. Here, we present the case of a 65-year-old male patient with a history of systemic hypertension who developed BRAO in the right eye and BRVO in the left eye within a short time interval. Detailed …

Association of Retinal Artery and Other Inner Retinal Structures With Distribution of Tapetal-like Reflex in Oguchi's Disease.

PURPOSE: To report novel ophthalmoscopic features of patients with Oguchi's disease, and to describe how they may be related to the unusual tapetal-like fundus appearance. METHODS: Twenty-one eyes of 11 patients who were diagnosed with Oguchi's disease were investigated. Genetic screening of seven cases showed homozygous mutations in the SAG gene (c.926delA). The retinal appearance was retrospectively assessed in the fundus photographs, and the optical coherence tomographic (OCT) and fundus autofluorescence (AF) images. RESULTS: In 11 eyes of 7 patients, clearly demarcated dark regions witho
