A case of vertebrobasilar transient ischemia and posterior cerebral infarction caused by a ruptured cervical disc is reported. The causes of extrinsic vertebral artery compression are reviewed.

Whether the posterior urethra can successfully be reconstructed depends mainly on the primary care, especially in the case of strictures secondary to trauma. A 50-year-old woman complained of upper abdominal pain for several weeks, getting worse and radiating towards the left shoulder in the 24 hours preceding her hospital admission. A patient with bilateral staghorn calculi treated by ESWL and endourologic placement of a double-J catheter is described. The patient was lost to follow-up for 18 months after catheter placement. When the patient was seen again, examination revealed a fractured catheter which warranted retrieval by combined nephrectomy and URS.

Three cases of complex aneurysms are presented in which balloon embolization therapy was associated with subsequent aneurysmal rupture, causing subarachnoid hemorrhage in two cases and a carotid-cavernous fistula in one. Two of these patients were treated directly by balloon embolization following surgical exploration. The third patient developed the fistula during postembolization volume expansion and heparinization.

Angiosarcoma of the spleen is a rare tumor with a very poor prognosis. Review of the world literature reveals only 57 reported cases of this neoplasm. Sixteen of these presented with an acute abdomen secondary to splenic rupture. Patients with this tumor have a mean survival time of 14.4 months after detection, and this decreases to 4.4 months after splenic rupture. The only rational treatment is splenectomy prior to splenic rupture. This paper describes another case presenting as spontaneous splenic rupture.

A 31 years old multiparous woman was delivered by caesarean section at her first pregnancy. For her third, spontaneous, full-term labour an epidural analgesia was realized with 0.125% epinephrined bupivacaine and fentanyl (1 mg per kg). Four cases of spontaneous splenic rupture after infectious mononucleosis (IM) have been treated at this institution since 1978. The condition is rare, occurring in 0.1-0.5 per cent of patients with proven infectious mononucleosis. Splenectomy is considered the treatment of choice for these patients. The authors describe a rare case of spontaneous rupture of a renal liposarcoma which imitated the clinical picture of an acute abdomen. Urgent transabdominal left nephrectomy was performed. The entire lower half of the kidney was altered by the tumour and ruptured and there was an extensive perirenal and retroperitoneal haematoma. Ten patients with traumatic lesions of the thoracic aorta were seen in a hospital. Most were victims of traffic accidents and presented severe associated lesions along with their vascular trauma. We found that the vascular injuries were clinically manifest in only a minority of patients. The remaining ruptures were discovered through CT-scanning of the mediastinum or angiography. We believe that in every major trauma victim aortic lesions should be actively sought for by complimentary examinations to guarantee maximum survival of the patients.
