Validation of a new method for the radiological measurement of rod curvature in patients with spine deformity.

PURPOSE: The relationship between rod curvature and postoperative radiographic results is a debated topic. One of the reasons of the heterogeneity of the observed results might reside in the lack of a validated and widely employed method to measure the curvature of the rods. Aim of this study was to present and validate a novel method for rod measurement, which is based on routine X-rays and utilizes a regression algorithm that limits manual measurements and the related errors. METHODS: Data from 20 adolescent idiopathic scoliosis/Scheuermann kyphosis (AIS/SK) patients and 35 adult spine defo

Complications in operative Scheuermann kyphosis: do the pitfalls differ from operative adolescent idiopathic scoliosis?

STUDY DESIGN: A prospective multicenter database of operative patients with Scheuermann kyphosis (SK) with minimum 1-year follow-up was studied for major complications compared with contemporaneous operative patients with adolescent idiopathic scoliosis (AIS) from the database. OBJECTIVE: To evaluate complications associated with current surgical techniques in SK and AIS. SUMMARY OF BACKGROUND DATA: There is a paucity of literature regarding complications associated with SK surgical treatment, but prior data suggest an elevated neurological risk. METHODS: Complication rates were compared us

Multi-functionality of computer-aided quantitative vertebral fracture morphometry analyses.

Osteoporotic vertebral fractures are an increasingly active area of research. Oftentimes assessments are performed by software-assisted quantitative morphometry. Here, we will discuss multi-functionality of these data for research purposes. A team of trained research assistants processed lateral spine radiographs from the population-based Rotterdam Study with SpineAnalyzer(®) software (Optasia Medical Ltd, Cheadle, UK). Next, the raw coordinate data of the two upper corners of Th5 and the two lower corners of Th12 were extracted to calculate the Cobb's kyphosis angle. In addition, two readers

[Omitted spinal X-ray examination after a fall from a height of 3 m].

A 59-year-old patient with right-sided chest pains after a fall from a height of 3 m was referred to hospital by an emergency physician. The chest x-rays showed fractures of the third and seventh ribs on the right side. Inpatient analgesic therapy was initiated and after 3 days the patient was discharged from hospital for further outpatient treatment. As the pain persisted the patient consulted a surgeon 5 weeks later and the first X-ray examination of the spine was carried out which revealed the formation of several wedge-shaped thoracolumbar vertebral bodies. Further magnetic resonance imagi

Is there an indication for surgery in patients with spinal deformities? - A critical appraisal.

BACKGROUND: High-quality evidence exists to support physiotherapy and brace treatment for scoliosis and other spinal deformities. However, according to previous systematic reviews, it seems that no evidence exists for surgery. Nevertheless, the number of research articles focussing on spinal surgery highly exceeds the number of articles focussing on conservative treatment. OBJECTIVE: The purpose of this study is to conduct an updated search for systematic reviews providing high-quality evidence for spinal surgery in patients with spinal deformities. METHOD: A narrative review including PubMe

High lumbar disc degeneration. Incidence and etiology.

Three hundred seventy-nine consecutive magnetic resonance images (MRIs) with dual-echo images of the entire lumbar spine were reviewed by the authors. All 379 patients presented with back pain and/or leg pain; they were interviewed and examined. Pain drawings were completed by all. There were 42 patients (11.1%) with disc pathologies involving T12-L1, L1-2, and/or L2-3 levels. Six patients (1.6%) had isolated disc degeneration and/or herniations limited only to these high lumbar segments. The remaining 36 patients had degenerative changes of the higher discs with variable involvement of the lo

Predictive value of upper instrumented level for proximal junctional Kyphosis in Scheuermann's Kyphosis.

Retrospective study. This study aimed to determine whether selecting Th3 and more distal vertebrae as upper instrumented vertebrae (UIV) affects the incidence of proximal junctional kyphosis (PJK) compared with more proximal levels or among themselves. Patients who underwent surgery for Scheuermann’s kyphosis (SK) between 2020 and 2023 were retrospectively reviewed. Individuals without a previous history of spine surgery and with a minimum follow-up of 2 year were included. A total of 34 patients were included, with 29 (85%) being male. The mean age was 17.67 ± 3.68 year. Patients with higher

Ponte osteotomies for treatment of spinal deformities: they are not all made equal.

PURPOSE: To compare surgical outcomes of Ponte's osteotomies for treatment of adolescent idiopathic scoliosis (AIS), Scheuermann's kyphosis (SK), and adult scoliosis (AdS). METHODS: We conducted a retrospective review of patients with AIS, SK, and AdIS who underwent posterior spinal instrumented fusion (PSIF) at our Institution from January 2019 to December 2022. Demographics, imaging, and intraoperative data (including number of osteotomies performed, blood losses, surgical timing, and complications) were extracted from patient charts. RESULTS: A total of 80 patients (62 AIS, 7 SK, and 11 A

Do discs "open" anteriorly with posterior-only correction of Scheuermann's kyphosis?

STUDY DESIGN: Retrospective radiographic analysis. OBJECTIVE: To investigate where and to what degree the correction of Scheuermann's kyphosis occurred in two different procedures: posterior instrumentation/fusion with an anterior release and posterior-only instrumentation/fusion with Ponte procedure. SUMMARY OF BACKGROUND DATA: Controversy remains regarding the outcome for these two procedures. The postoperative segmental disc shape change that account for deformity correction has not been described for either procedure. METHODS: Eleven patients undergoing a thoracoscopic anterior release

Scheuermann kyphosis: long-term follow-up.

BACKGROUND CONTEXT: There is considerable controversy as to the optimal treatment of Scheuermann kyphosis. Proposed modalities have included exercise, bracing and surgery. PURPOSE: The purpose of this study was to document the functional capacity and radiographic findings in adults who have been previously treated for Scheuermann kyphosis. STUDY DESIGN: A cohort study of all patients with Scheuermann kyphosis treated in a single institution using three different treatment modalities: exercise and observation, Milwaukee bracing and surgical fusion using the Harrington Compression System. Identification of human remains lacking skull and teeth. A case report with some methodological considerations.

This report deals with a case in Denmark in which close cooperation among the local police, the Central Missing and Wanted Persons Search Squad, and the Institute of Forensic Medicine resulted in the positive identification of human remains (part of a skeleton), although the deceased's skull was missing, he had no teeth (but a dental prostheses), and an attempt at blood typing produced a false result. When the remains were found, there was no idea who the deceased might be. The initial forensic examination provided a general description (age, height, and sex), and reconstruction of the vertebr

Collagen stability and cross-linking in normal and kyphoscoliotic mouse intervertebral discs.

Intervertebral discs of the cervical-thoracic region of the spine of BDL mice which are homozygous for the ky gene mutation undergo degeneration. Discs from these mice have a normal collagen content and undergo normal collagen cross linking prior to the appearance of degenerative changes. The major reducible collagen cross-link formed in discs of these mice and in normal CBA strain mice is hydroxylysino-5-ketonorleucine. These results and other previous results indicate that the discs in the ky mouse develop degenerative disease due to an extrinsic factor rather than to an intrinsic abnormalit

Validity of a one-item drawing-based instrument to assess trunk deformity perception in kyphotic deformities.

INTRODUCTION: Body image and trunk appearance perception are two crucial attributes in determining the quality of life of individuals with spinal deformities. The Trunk Appearance Perception Scale (TAPS) is a drawing-based instrument used to evaluate this feature. The TAPS does not include a sagittal view of the trunk so it is never been used to assess trunk deformity perception in hyperkyphotic patients. This study aims to analyze the effect of introducing a new drawing on the TAPS scale representing the trunk view in the sagittal plane. PATIENTS: 170 non-surgical patients were included (mea

Paraspinal muscle volume in patients with Scheuermann's Kyphosis.

To measure the cross-sectional area (CSA) of para-spinal muscles in Scheuermann's kyphosis patients. Preoperative MRI images of 16 Scheuermann's kyphosis patients were analysed and compared to 16 patients with normal MRI images (control group). The CSAs were measured at L3-4 and L4-5. Both groups showed similar demographics and patient characteristics. The multifidus muscles CSA were found to be significantly smaller at L3/4 level in Scheuermann's kyphosis patients (p = 0.022 on the left and p = 0.016 on the right side compared to control group). There was no significant change in multifidus C
