We evaluated the functional outcome 3 (2-12) years after arthroplasty in 29 proximal humerus fractures. The patients' average age at the time of surgery was 71 (47-87) years. The follow-up included a full clinical examination by an independent observer, a questionnaire concerning activities of daily living and pain, and radiographs. The shoulders had a marked reduction of performance, with a Constant score mean of 38 (16-69). All patients were able to eat with utensils, 10 could comb their hair, 19 managed to wash the contralateral armpit, 3 could reach the back pocket and 12 were able to take

A person of advanced age usually has degenerative changes of bone, joint and ligament, which can be causes of cervico-omo-brachial pain and disability. He or she may also suffer from metastatic bone tumor of cervical spine or upper extremity. This article described pathology, signs and symptoms and recent treatment of these diseases. Cervical myelopathy and radiculopathy, which are most common causes of cervico-omo-brachial symptoms, are sometimes accompanied by peripheral entrapment neuropathy such as cubital tunnel syndrome or carpal tunnel syndrome (double crush syndrome). In this complicat

BACKGROUND: Aseptic glenoid loosening is the most common cause of revision after anatomic total shoulder arthroplasty (aTSA). Inlay glenoid components have been shown to reduce edge loading and opposite-edge lift-off forces compared with onlay glenoids. Early clinical results of inlay glenoids have been promising, although there is a paucity of literature detailing mid-term to long-term follow-up. We report the minimum 5-year clinical and radiographic outcomes after aTSA with the Arthrosurface inlay glenoid component and a stemless, aspherical humeral head in an active, young patient populatio

PURPOSE: Shoulder arthroplasty (SA) utilization has increased substantially, with a corresponding rise in revision procedures, many driven by periprosthetic joint infection (PJI). Herpes zoster (HZ), or shingles, is increasingly prevalent among older and immunocompromised patients and shares overlapping risk factors with PJI. However, the association between shingles and postoperative complications following SA is unknown. This study evaluated the relationship between peri-operative shingles and postoperative complications after SA.
METHODS: Patient-level claims from adults ≥ 18 years undergo

BACKGROUND: Anatomic total shoulder arthroplasty (aTSA) is a well-described technique for addressing glenohumeral osteoarthritis. Little has been written on outcomes for newer stemless humeral implants in older patients, with none looking specifically at an implant relying on screw fixation. The purpose of this study is to evaluate the clinical and radiographic outcomes for patients over 70 years of age undergoing aTSA with the Eclipse (Arthrex Inc., Naples, FL, USA) stemless humeral component.
METHODS: A retrospective review using a multicenter shoulder arthroplasty registry was performed ev

BACKGROUND: The optimal management of posterior glenoid bone loss in glenohumeral osteoarthritis with anatomic total shoulder arthroplasty (aTSA) remains unknown, as the degree of bone loss and retroversion can vary across a wide spectrum in glenoids with Walch B2 and B3 morphology. The objectives of the current study were to evaluate the clinical and radiographic outcomes of utilizing standard, all-polyethylene, nonaugmented anchor-peg glenoid (APG) components in aTSA for patients with mild to moderate B2 or B3 morphology.
METHODS: Between January 2010 and September 2019, we identified 79 sh

BACKGROUND: The purpose of this study was to compare preoperative radiographic evaluation with intraoperative video and explant analysis in patients undergoing revision of a previous anatomic total shoulder arthroplasty (TSA).
METHODS: We evaluated the preoperative radiographs of 165 revisions of failed TSAs for component loosening and glenohumeral registry (ie, the spatial relationship of the glenoid component and the prosthetic humeral head). Seventy-nine intraoperative videos were evaluated for component stability, rotator cuff (RC) integrity, synovitis, and glenoid bone loss. Eighty-seven

BACKGROUND: There is limited information assessing outcomes after total shoulder arthroplasty (TSA) with a press-fit short stem. The purpose of this study was to evaluate early clinical and radiographic outcomes and complications after TSA with an anatomic press-fit short stem.
METHODS: We identified 118 TSAs completed with press-fit short stem and minimum 2-year follow-up; 85 of these patients had a grit-blasted (GB) short stem placed, whereas 33 patients had short stems with proximal porous coating (PPC). Shoulder function scores, active mobility measurements, and radiographs were assessed.

Glenoid component loosening is a common cause of failed total shoulder arthroplasty. Many different treatment options exist for the management of a symptomatic loose polyethylene glenoid component, ranging from component removal to revision arthroplasty. Arthroscopic removal and conversion to hemiarthroplasty provides a minimally invasive management option that avoids takedown of the subscapularis and allows for humeral prosthesis retention. Frequently, however, osteolysis is encountered concomitantly leading to cavitary glenoid bone deficits at the time of glenoid implant removal, limiting bo

OBJECTIVES: To evaluate the effect of oral antimicrobial suppression on the outcome of streptococcal periprosthetic joint infection (PJI).
METHODS: Consecutive patients with streptococcal PJI receiving antimicrobial suppression for >6 months were prospectively included and compared to a retrospective control group without suppression. Outcome was assessed with Kaplan-Meier analysis and compared by the log-rank Mantel-Cox test. Multivariate analysis was used to identify factors associated with treatment failure.
RESULTS: Of 69 streptococcal PJI episodes (37 knee, 31 hip and one shoulder PJI),

INTRODUCTION: To evaluate the effect of prosthetic lateralization on tuberosity healing following reverse shoulder arthroplasty (RSA) for proximal humerus fractures (PHFs) in older adults.
MATERIAL AND METHODS: This single-center retrospective study included patients aged >65 years with displaced Neer 3- or 4-part PHFs treated with RSA within four weeks of injury. Patients were allocated according to the global lateralization of the prosthesis: very highly lateralized (VHL; 21.4mm, Arrow®, n=53) and lateralized (L; 14.7mm, Unique®, n=20). Tuberosity healing was assessed radiographically and c

BACKGROUND: The prevalence of failed reverse total shoulder arthroplasty (rTSA) is increasing. This can often present a challenging clinical situation with substantial bone loss and limited reconstruction options. This study reports a single tertiary referral center's experience with revision of failed rTSA managed with revision rTSA of bone-interfacing components.
METHODS: After institutional review board approval, all revision shoulder arthroplasty cases performed at a single institution between 2012 and 2020 were reviewed. Cases in which rTSA was revised to a new rTSA construct with revisi

Three centers' experience with 93 comminuted humeral head fractures were clinically (Constant score) and radiologically (Neer) analyzed. The results revealed fundamental differences between the various types of fractures depending on the number of fragments. For three-part fractures open reduction and internal fixation (mean Constant scores 83 and 91 points, respectively) or conservative treatment (78 points) seem to be indicated. The prognosis for four-part fractures is determined largely by the vascular supply of the head fragment with a high risk of humeral head osteonecrosis. For this reas

Upper-limb prosthesis users continue to reject devices despite continued research efforts. Today, the passive topology of body-powered prehensors, which physically transmits grasp force and position data between user and device, results in improved performance over myoelectric alternatives. However, the loads and postures on the user's body also result in discomfort, fatigue, and worsened grasp force control. Despite the long history and everyday adoption of body-powered prehensors in society, the measurement of how specific body loads and postures affect grasp performance and user experience

INTRODUCTION: Indications for anatomic (aTSA) and reverse (rTSA) total shoulder arthroplasty are well defined and dependent on the function of the rotator cuff; however, indications for rTSA have gradually extended to complex fractures, revisions, and primary arthritis in very elderly patients. The risk of secondary rupture of a weakened or degenerative rotator cuff is difficult to assess and can lead the orthopaedic surgeon to hesitate between aTSA or rTSA. It, therefore, seems appropriate to compare these two types of prostheses in terms of pain, functional, clinical outcome metric scores, a

INTRODUCTION: While tuberosity reinsertion significantly improves the short-term functional outcomes of reverse shoulder arthroplasty done for proximal humerus fracture, we do not know how well these results hold over the long term. The objective of this study was to analyze the effect of tuberosity reinsertion on the quality of life of patients and the functional outcomes of the operated limb after a minimum follow-up of 5 years.
METHODS: Sixty-two patients were included. Their mean age at the final review was 79 ± 10 years. The Katz and Lawton scales, Constant score, DASH and SSV were colle
