[Differential diagnosis of pain experience. Chronic pain and depression].

Chronic pain and depressive symptoms often occur together. The relationships between these two conditions are, however, still controversial. On the neurobiological level, features common to both disorders are to be found. Mental coping patterns favor the development of depressive symptomatology in patients suffering from pain, or of pain within the framework of a psychiatric illness. For the diagnostic work-up, the case history, exploration, organic investigations and, where indicated, psychological tests are employed. A well-proven strategy in the areas of diagnosis and therapy is interdiscip

Psychosocial correlates of pain attributions in women with dyspareunia.

The relationship between patients' causal attributions for pain and biopsychosocial measures was investigated in a sample of 100 women with dyspareunia. Independently of findings from the gynecological examinations, causal attributions were related to adjustment. More specifically, the women who made psychosocial attributions reported higher pain scores, higher levels of psychological distress, lower levels of marital adjustment, more problems with sexual function, and more frequent reports of sexual assault. The relationship between psychosocial causal attributions for pain and psychosocial d

[Can psychosomatic concepts make a contribution in surgery?].

All medical reasoning and action is based on a medical theory. Johannes Müller, the renowned 19th century physiologist, stated: "What we experience with our senses is not a mirror of objects characteristics, but rather depends on the living organism's interpretation of sense-receptor stimulation." A mechanistic model of medicine which does not reflect on and include individual reality and the influence of personality and personal history subjects patients to the danger of surgery; psychosomatic concepts reduce this danger. If the surgeon integrates (not adds!) the psychological and social dime

Eating disorders and insulin-dependent diabetes mellitus.

The eating disorders anorexia nervosa and bulimia nervosa have been reported to occur in Type I diabetes mellitus. Although prevalence estimates vary, the most rigorous studies yield rates similar to the population at large. Intentional insulin omission is more common, especially in young diabetic women, and at times may indicate an eating disorder in Type I diabetic patients. Both diagnosable eating disorders and intentional insulin omission are associated with worse glycemic control and higher rates of secondary diabetic complications. Recognition of these conditions, followed by carefully c

[Adjusting to illness and secondary prevention in unemployed patients from the medical psychology viewpoint].

Due to the demands on medical and psychosocial facilities, an above average accumulation of unemployed patients was investigated. General practitioners and internists have reported high stress experiences of patients and the difficulties encountered with their illness reflection. It was clear in the examinations, that there was no fidderence in the way unemployed and employed reflected their illness, although a difference was shown in their coping of critical life experiences. In scope of the medical conversation, unemployed patients could convey specific coping abilities as a secondary preven

Depression in fatiguing illness: comparing patients with chronic fatigue syndrome, multiple sclerosis and depression.

Because depression is commonly observed in the chronic fatigue syndrome (CFS), the present study sought to determine whether the symptom pattern is similar to that seen in clinically depressed subjects (DEP). Individuals with multiple sclerosis (MS) were chosen as an additional comparison group because MS is a fatiguing illness of known organic etiology. The Beck Depression Inventory (BDI) was used to compare categories of depressive symptomatology. Absolute scores on the BDI were higher for the depressed group on mood and self-reproach symptoms, but were not higher than the CFS group on somat

Family support and coping with cancer: some determinants and adaptive correlates.

This paper studies the interrelationships between the coping behaviours of cancer patients and perceived amount and adequacy of family support, as well as the issue of how these predict psychosocial adjustment to cancer. Based on questionnaire data from a sample of 169 patients with cancers of various sites, three questions were considered in detail: (1) How might cognitive and behavioural modes of coping with cancer affect perceptions of support provided by one's family in terms of the amount as well as adequacy of various supportive acts? (2) Are coping modes to be considered when explaining

An unusual presentation of multiple sclerosis.

Two male patients who presented with unusual pictures of dementia in the absence of other obvious symptoms or signs are reported. Investigations demonstrated changes highly suggestive of multiple sclerosis (MS) on magnetic resonance imaging, cerebrospinal fluid analysis and electrophysiological tests. We suggest this represents a rare presentation of multiple sclerosis.

[Fatigue and signs of depressive symptoms in general practice in Aquitaine].

UNLABELLED: An epidemiologic study of patients consulting General Practitioners (GPs) was conducted among GPs who usually collaborate on Aquitaine Sentinel Network, supported by the Department of Medical Informatics of the University of Bordeaux II. One day in June and one day in December 1992, each GP who participated in this investigation had to collect age, sex and reason for consulting about every patient who took medical advice. For every patient over 15 year old, on the day of the consultation or the previous week, the GP had to collect signs of depression, dysphoria, loss of interest or

[Chronic fatigue syndrome. Clinical, social psychological problems and management].

Fatigue chronic syndrome (SFC) is the heir-at-law of neurasthenia. Both are seen like physical diseases and share certain therapeutic measures, such as sleep; they have the same symbolic function and enable patients as well as doctors reluctant to psychological dimensions of pathology, to get and express sympathy and attention. A strong controversy developed these last years concerning the SFC physiopathology particularly concerning the responsibility of viral infectious agents or psychiatric troubles. The SFC fatigue is unlikely hysterical or neuromuscular but it probably depends on several a

Sickle cell disease pain in children and adolescents: change in pain frequency and coping strategies over time.

Examined 9-month follow-up data obtained from children and adolescents with sickle cell disease (SCD) and their parents participating in a longitudinal study of pain coping strategies. Of 87 subjects completing the baseline assessment of pain coping strategies, 70 (80%) of their parents completed a structured pain interview assessing their child's health care use and activity reduction during painful episodes over the follow-up period. Regression analyses controlling for age and pain frequency revealed that baseline Coping Attempts were associated with higher levels of school, household, and s

Asbestos exposure and posttraumatic stress disorder.

In 48 cases of claims of psychic injury due to exposure to asbestos, posttraumatic stress disorder (PTSD) was alleged as a basis for damages in nine (19%). PTSD as currently defined refers to a specific syndrome following exposure to a life-threatening traumatic event, one outside the general range of people's experiences such as military combat, floods, earthquakes, bombing, torture, death camps--events either natural or man-made. The symptoms include reexperiencing of the event with intrusive thoughts or nightmares of the event, startle reactions, psychic numbing, and survival guilt. The app

[Subjective discomfort of stomach cancer and esophageal cancer patients in after-care].

In the framework of a study on the rehabilitation needs in gastric and oesophageal cancer patients, 100 gastric and 100 oesophageal cancer patients were interviewed on their subjective emotional state and physical functioning. The potentially cured patients were admitted to our in-patient aftercare programme an average 9.5 months following primary treatment. Major psychological distress was reported by 69.5% of the patients. Brooding, unrest, nervousness, and insomnia were the most frequently named complaints in both groups. 93% reported physical limitations, and 80% felt their conditions of l

[Lay etiologic theories and coping with illness in severe physical diseases. An empirical comparative study of female myocardial infarct, cancer, dialysis and multiple sclerosis patients].

In a multicenter approach 451 female patients, suffering from severe somatic diseases, were investigated on strains, disease-related attributions, coping and quality of life. Four diagnostic groups were involved, myocardial infarction (MI), Cancer (CA), end-stage renal disease (ESRD), and multiple sclerosis (MS). With respect to coping with the onset of illness or the diagnosis, all four groups accordingly reported placing trust in the doctors, compliance-related strategies, fighting spirit and information seeking as most markedly applied coping modes. But in each group a different predominant

Children with physical handicaps and their mothers: the interrelation of social support, maternal adjustment, and child adjustment.

Examined the interrelation of maternal adjustment, mother-child interaction, and child adjustment in 29 families of children with spina bifida and without mental retardation and in 28 families of children without handicaps. A multivariate, ecological model proposed that adjustment of mother and child depends on the adaptiveness of maternal response to the stress of the physical handicap and on the ability of mothers to create an optimal caregiving environment through mother-child interaction. Analyses examining the relationships among maternal social support, maternal psychological adjustment,

Congenital gastrointestinal anomalies: psychosocial functioning of children with imperforate anus, gastroschisis, and omphalocele.

This study assessed the psychosocial status of 56 children aged 6-16 years with imperforate anus, gastroschisis, or omphalocele. Results indicate that (a) children were of average intellectual ability; (b) approximately 9% of the sample had learning disabilities; (c) although the mean maternal ratings of behavioral adjustment did not differ from those reported for children without anomalies, approximately 18% of the children had deviant adjustment scores; (d) a multivariate risk and resistance model accounted for a significant amount of the variance in children's adjustment; and (e) child, mat

Depressive symptomatology and medical co-morbidity in a primary care clinic.

Most primary care patients exhibiting significant depressive symptomatology fail to meet DSM-III criteria for a major depressive disorder (MDD). Yet, such patients have substantial morbidity and dysfunction attributable to their affective syndrome. Since surprisingly little is known about this group's clinical characteristics, we studied 618 general medicine patients aged eighteen to sixty-four years. In this population, fifty-seven (9.2%) scored quite high when screened on the Center for Epidemiological Studies Depression Scale (greater than or equal to 27) while not meeting MDD criteria on t
