Skull Base Plasmacytoma in a Patient with Light Chain Myeloma.

Skull base involvement of plasmacytoma is reported in a patient with light chain myeloma. A 39-year-old man was admitted after experiencing paresthesia on the left side of the face and left arm, intermittent diplopia, and hoarseness for 2 years. Cranial magnetic resonance imaging revealed a large midline mass extending from the middle and posterior skull base into the upper two cervical vertebrae. An extramedullary plasmacytoma associated with light chain multiple myeloma was diagnosed after biopsy of the mass and laboratory investigations. The imaging findings and clinical features associated…

[Common errors of intraoperative navigation in lateral skull base surgery].

BACKGROUND AND OBJECTIVE: The knowledge of disturbing factors for navigated surgical procedures in lateral skull base surgery will help the user to identify, avoid, and correct these errors. PATIENTS/METHODS: Based on our experiences with 85 patients undergoing surgery of the lateral skull base, errors of hard- and software, of image acquisition and image transfer, of patient registration, user-related errors as well as strategic errors were analyzed. RESULTS: Any impairment of the camera field leads to functional limitation of optical systems to the same extent as electromagnetic systems ca…

[The subcranial approach for tumors involving the anterior skull base].

The technical approach for anterior skull base tumors has progressed considerably during the last decade. In the classical combined craniofacial resection (CFR) technique, anterior skull base lesions have been approached through bi-frontal craniotomy coupled with Weber-Ferguson and related modifications incisions. The purpose of this work is to present our experience with the subcranial approach for treatment of anterior skull base tumors. During the years 1994-2002, 64 patients underwent 72 procedures of anterior skull base tumor resection via the subcranial approach. The ages of the patients…

Lateral skull base surgery for glomus tumors: long-term control.

OBJECTIVE: The age of modern microsurgery has made resection of glomus tumors with extensive skull base involvement possible. Resection of extensive lesions is not without risk of major complication or new cranial nerve deficit. Because glomus tumors are rare and slow growing, data reflecting recurrence risk after resection using modern skull base techniques are lacking. These factors complicate the accurate definition of efficacy of surgical resection and its functional cost. The object of this review is to determine the current incidence of major complications, the surgical cranial nerve def…

A huge osteoblastoma with aneurysmal bone cyst in skull base.

We reported a case of a huge osteoblastoma in the skull base of a young girl who had developed blindness in her right eye 1 year prior to examination. CT showed a large expansile bony lesion with eggshell margin over the skull base. MR imaging showed mixed solid and cystic parts with multiple fluid-fluid levels in the lesion. The optic chiasma and bilateral optic nerves were severely compressed by the tumor.

Early arytenoid adduction for vagal paralysis after skull base surgery.

OBJECTIVES: To evaluate the efficacy of early arytenoid adduction in the management of vagal paralysis after skull base surgery. STUDY DESIGN: Retrospective evaluation at a tertiary care skull base center. METHODS: Aggressive surgical management of skull base lesions has become increasingly popular owing to advances in surgical technique and intraoperative monitoring. Temporary and permanent lower cranial neuropathies occur frequently, especially after the surgical management of lesions involving the vertebrobasilar system and the jugular foramen. An injury to the proximal vagus nerve is usu…

Transciliary subfrontal craniotomy for anterior skull base lesions. Technical note.

Microsurgical approaches are the procedures of choice for high-risk patients with lesions requiring surgical treatment. The use of a microscope reduces the extent of the surgical invasion, thus minimizing the handling of healthy tissues. The authors present a surgical approach described for the first time in 1981, which has been used for the past 17 years in more than 260 patients with different tumors and vascular lesions of the anterior cranial fossa. The modification set forth in this article makes better exposure possible, allows more space for instrument handling, and improves cosmetic re

Management of the jugular bulb during lateral skull base surgery.

The intraoperative management of a patent jugular bulb presents a formidable challenge during the treatment of lateral skull base lesions. Whether to preserve, partially occlude, or completely sacrifice this critical structure is a decision best made following a multifactorial analysis of preoperative clinicoradiographic data, tumor histopathology, and intraoperative findings. Twenty-six patients with tumors requiring dissection near a patent jugular bulb were reviewed. Ten patients had superior neck tumors, nine had primary temporal bone lesions, and seven presented with recurrent parotid mal…

Chondrosarcoma of the skull base: a series of eight cases.

Chondrosarcomas of the skull base are indolent, locally invasive tumors with a marked tendency to recur. Surgery is the mainstay of treatment because these tumors are generally resistant to other forms of treatment. A surgical approach with wide access to the skull base and one that is easily repeatable is required, because recurrence is common. We have used the LeFort I maxillotomy or mobilization of the zygoma at the time of craniotomy to obtain wide access to the skull base in eight cases of chondrosarcoma. Three patients have undergone subsequent procedures by us for recurrent disease. One…

Rehabilitation of cranial nerve deficits after neurotologic skull base surgery.

Damage to any one of these nerves results in sufficient morbidity to warrant therapy; however, most patients will compensate for isolated loss of function. It is with multiple nerve injuries, as often seen in glomus surgery with the resection of cranial nerves IX, X, and XII that the full efforts of the rehabilitation team are called on. After vocal cord medialization and palatal adhesion, younger healthier patients will eventually resume adequate oral intake. However, the time it requires to return to a reasonably enjoyable diet often extends up to 1 year postoperatively. A few never attain t

Free rectus abdominis muscle reconstruction of the anterior skull base.

Pericranial flaps and galeal frontalis myofascial flaps can be useful for separating the cranial space from the paranasal sinuses and the nasopharynx, but they cannot provide reliable separation and protection of the brain from bacterial flora of the upper airway in patients who previously have received high dose radiation therapy and undergone craniotomy, or required extensive resection of the cranial base. The free rectus abdominis muscle flap, on the other hand, can provide a good alternative for reconstruction of the anterior skull base in such difficult cases, and give good results. In si…

Operative approach to the frontal skull base: extensive transbasal approach.

An operative technique called the extensive transbasal approach is reported with the operative results of 11 cases. This is an operative mode in which en bloc osteotomy of the orbital roofs and frontal sinus is performed after ordinary bifrontal craniotomy. Through this approach, a far wider operative space than that afforded by conventional operative techniques is possible, and reconstruction of the frontal base can be made securely. We consider this approach of major clinical value.

Beyond Bell's Palsy: Skull Base Pathology Presenting with Facial Paralysis.

BACKGROUND: Bell's palsy is the most common diagnosis in patients presenting with acute facial paralysis. It is important to understand how occult skull base pathology may mimic idiopathic facial paralysis. This study evaluates the incidence and clinical predictors of occult skull base pathology among patients initially diagnosed with Bell's palsy. METHODS: A retrospective review was performed of patients who presented between January 2002 and January 2024 with an initial diagnosis of Bell's palsy and had at least 12 months of follow-up. Patients with known malignancy, trauma, infectious etio…

Diagnostic Tools and Imaging for Skull Base Cerebrospinal Fluid Leak.

The diagnosis of skull base cerebrospinal fluid (CSF) leaks remains challenging and requires a multidisciplinary approach. Clinical evaluation and targeted endoscopic examination are essential, but laboratory confirmation with beta-2 transferrin or beta-trace protein provides the most reliable noninvasive evidence. Imaging modalities such as high-resolution computed tomography (CT), magnetic resonance cisternography, CT cisternography, and radionuclide studies complement detection and localization, guiding surgical planning. Intraoperative fluorescein enhances defect identification. Each diagn…
