Parasellar Meningiomas.

Parasellar spaces remain particularly singular, comprising the most important neurovascular structures such as the internal carotid artery and optic, oculomotor, and trigeminal nerves. Meningiomas are one of the most frequent tumors arising from parasellar spaces. In this location, meningiomas remain mostly benign tumors with WHO grade I and a meningothelial subtype. Progestin intake should be investigated and leads mostly to conservative strategies. In the case of benign nonsymptomatic tumors, observation should be proposed. Tumor growth will lead to the proposition of surgery or radiosurgery

Endonasal endoscopic transsphenoidal excision of tuberculum sellae meningiomas: a systematic review.

INTRODUCTION: The endonasal endoscopic approach (EEA) for the resection of tuberculum sellae meningiomas (TSMs) has, more recently, been advocated as an alternative approach to deal with this challenging tumor. The aim of this study was to conduct a systematic review of publications of TSMs excised through the transsphenoidal route in the past 10 years and review data on the extent of excision, visual outcomes and complication rates. EVIDENCE ACQUISITION: We performed a thorough systematic review of the medical literature following the PRISMA guidelines. A medical librarian retrieved a list o

Nasopharyngeal carcinoma with skull base invasion and hydrocephalus: a case report.

We describe a case of nasopharyngeal carcinoma (NPC) with diffuse skull base bony destruction, resulting in obstructive hydrocephalus. Previously, these lesions were reported to be NPC with brain metastasis, without diffuse bony destruction of the skull base or hydrocephalus. We found no other cases of NPC with brain metastasis and diffuse bony destruction of the skull base resulting in hydrocephalus in the literature.

Functional Outcomes and Quality of Life in Patients with Sinonasal, Nasopharyngeal, and Anterior Skull Base Tumors.

PURPOSE OF REVIEW: This article will review functional and QOL outcomes among patients treated predominantly for sinonasal and nasopharyngeal malignancies. RECENT FINDINGS: Treatment advances and interdisciplinary supportive care help to lessen the functional impairments and the reduction in quality of life (QOL) that were once accepted as inevitable tradeoffs for cure. Recent progress in QOL and Patient-Reported Outcome (PRO) instruments for this population will be covered. Sinonasal and nasopharyngeal tumors affect patients' quality of life, appearance, and critical functions. Tumors arise

Proton Therapy in Head and Neck Cancer Treatment: State of the Problem and Development Prospects (Review).

Proton therapy (PT) due to dosimetric characteristics (Bragg peak formation, sharp dose slowdown) is currently one of the most high-tech techniques of radiation therapy exceeding the standards of photon methods. In recent decades, PT has traditionally been used, primarily, for head and neck cancers (HNC) including skull base tumors. Regardless of the fact that recently PT application area has significantly expanded, HNC still remain a leading indication for proton radiation since PT's physic-dosimetric and radiobiological advantages enable to achieve the best treatment results in these tumors.

Combined Microscopic-Endoscopic Transmastoid Resection of a Petrous Meningioma: 2-Dimensional Operative Video.

Petrous meningiomas are defined as tumors with a basal dural attachment on the posterior surface of the petrous bone.1 Their insertion can be anterior to the meatus (petrous apex meningiomas), or posterior to the meatus, with associated hyperostotic bony invasion either pre- or retro-meatal.2 These meningiomas are amenable to curative surgical removal and have better surgical outcomes than more medially located true petroclival meningiomas that originate medial to the fifth nerve.2-4 They, however, remain challenging because of their close relationship to critical neurovascular structures in t

Foramen magnum meningiomas.

The foramen magnum meningioma (FMM) is one of most challenging tumors among all the meningiomas because of its distinctive location, clinical course, insidious onset, and the relative large size of the tumor during presentation. These tumors are slow-growing and clinical symptoms vary according to involvement of different structures including the spinal cord, vertebral artery, lower cranial nerves, and medulla oblongata. Controversies regarding appropriate surgical approach for resection of tumor in this unique location continue. Tumors in the posterior or posterolateral region of the foramen

Proteomic analysis of meningiomas reveals clinically distinct molecular patterns.

BACKGROUND: Meningiomas represent one of the most common brain tumors and exhibit a clinically heterogeneous behavior, sometimes difficult to predict with classic histopathologic features. While emerging molecular profiling efforts have linked specific genomic drivers to distinct clinical patterns, the proteomic landscape of meningiomas remains largely unexplored. METHODS: We utilize liquid chromatography tandem mass spectrometry with an Orbitrap mass analyzer to quantify global protein abundances of a clinically well-annotated formalin-fixed paraffin embedded (FFPE) cohort (n = 61) of mening

Case report: Epicranial pain after radiotherapy for skull base meningioma - the first symptomatic epicrania fugax?

BACKGROUND: To date, no symptomatic forms of epicrania fugax (EF) have been reported. Here, we describe the first EF-type pain to be probably caused by an underlying disorder. CASE REPORT: A 77-year-old woman started suffering from left V1-V2 trigeminal neuralgia at 72 years of age. Neurologic examination was normal. Magnetic resonance imaging (MRI) showed a left middle sphenoid wing meningioma compressing the left trigeminal nerve medially. After trying several neuromodulators, she received stereotactic radiotherapy. One month later, the episodes of facial pain were significantly diminished,

Computer-assisted resection of benign sinonasal tumors with skull base and orbital extension.

OBJECTIVE: To use freehand, real-time, intraoperative, 3-dimensional computed tomography (3-D CT) localization to assist with resection of sinonasal lesions with skull-base and/or orbital extension. DESIGN: The 3-D CT is computer-assisted technology using the preoperative CT scan as a road map for resection of lesions. Previous technology, while resulting in little intraoperative error, required use of a somewhat cumbersome rigid articulated arm and attached probe, limiting mobility and requiring rigid fixation of the head to maintain system reference. A new freehand version of this technolog

[By nasal cavity approach resection of bigger pleomorphic adenoma in left skull base with nasal endoscope].

A 46-year-old female patient presented to our hospital with history of pharyngeal discomfort for 6 months. Physical examination showed that she had facial asymmetry, loss of the left nasolabial fold, rightward de- viation of uvula, swelling of the left soft palate. Magnetic resonance imaging revealed a bigger neoplasm in left pa- rapharyngeal space which invaded into left lateral skull base. The left internal carotid artery, vein and the styloid process was jostled backward. The primary clinical diagnosis is pleomorphic adenoma of the parapharyngeal space.

Juvenile ossifying fibroma of paranasal sinuses-do we need to be radical in surgery?

Juvenile ossifying fibroma (JOF) is an uncommon, benign, bone-forming neoplasm seen in the craniofacial bones. It is distinguished from other fibro-osseous lesions primarily by its age at onset, clinical presentation, and potential aggressive behavior. Occasionally JOF may grow aggressively and extend to involve the orbits and skull base, resulting in serious cosmetic and functional problems. A radical surgery is not advisable in a pediatric age group. We describe a case of JOF of the maxilla removed by sublabial approach conservatively, but completely without compromising the aesthetic looks

Craniofacial reconstruction following oncologic resection.

The ability to reliably reconstruct complex and sizable wounds has decreased the morbidity of skull base surgery substantially, preventing major complications and allowing treatment of tumors previously considered inoperable. Addressing facial nerve function with static and dynamic procedures as well as fabrication of craniofacial prostheses to replace delicate facial landmarks has further increased surgeons' ability to restore the appearance and function of the face.

Central nervous system and head and neck imaging findings of MYC/BCL2 "double hit" B cell lymphoma.

MYC/BCL2 double hit lymphoma (DHL) is a rare, recently recognised and highly aggressive subtype of non-Hodgkin lymphoma, with an affinity to involve the central nervous system and the head and neck either at initial presentation or during relapse. We present a case of 43-year-old woman with MYC/BCL2 DHL relapse in the nasopharynx with extensive spread to the neck, skull base, and the central nervous system. To our knowledge, this is the first reported case in the literature describing the MRI and CT scan findings and the profound pattern of disease involvement of this rare neoplasm.

Extracranial skull base chondrosarcoma.

Chondrosarcoma of the skull base is a rare neoplasm. The most common presenting symptoms are hoarseness, dysphagia, and diplopia, which is associated with palsies of cranial nerves X, IX, and VI. The temporal bone is the most common site of tumor origin, followed by the sphenoid bone. These tumors must be differentiated from chordoma, chondroid chordoma, osteogenic sarcoma, enchondroma, and meningioma. Diagnosis is made by patient history, radiologic imaging, and biopsy. The treatment is surgical excision followed by radiation therapy. Five-year survival rates for grades I, II, and III are 90,

Giant-cell neoplasm of the sphenoid sinus.

A rapidly growing giant-cell neoplasm arising in the sphenoid sinus of a 26-year-old man is reported. The patient's only initial symptoms were severe headaches and left sixth-nerve palsy. Computerized tomography of the brain showed extensive tumor infiltration of the bony structures and soft tissues of the base of the skull. A transsphenoidal surgical procedure proved unsuccessful in removing the tumor. Histologic examination revealed a proliferation of osteoclast-type giant cells against a background of highly atypical mononuclear cells, raising the question of malignancy. The differential di
