Epstein-Barr virus-associated smooth muscle tumor (EBV-SMT) is a rare mesenchymal tumor that almost exclusively occurs in immunocompromised hosts. Here, we report a 75-year-old Taiwanese woman without definite immune-deficient history presenting with progressive occipital neuralgia, low cranial nerve deficits (CN9-12) and cervical (C1-C5) radiculopathy. Magnetic resonance imaging revealed a 4.5*4.0*6.7 cm infiltrating mass occupying posterior skull base and C1-C2 vertebra and C1-5 epidural extension with bone destruction and vertebral artery (VA) encasement. There was also a synchronous 2.7 cm

The usual clinical presentation of sinonasal tumours includes symptoms that are indistinguishable from inflammatory sinus disease, namely nasal airway obstruction, pain, and epistaxis. Abnormal V1 and/or V2 sensations are a strong indication of the possibility of tumour. Computed tomography is the most reliable and informative imaging tool for evaluating the cancers of the paranasal sinuses. Magnetic resonance imaging is essential for tumour mapping because of the excellent tissue characterisation and the possibility of differentiating between neoplasms and retained secretions. A wide variety

BACKGROUND: Angiosarcoma is a rare neoplasm. It is most likely to affect the scalp in elderly people and involvement of the cranium is uncommon. We report a case of primary malignant angiosarcoma of the cranial vault.
CASE DESCRIPTION: An 82-year-old man presented with right hemiparesis, deformity of the cranium and mildly swollen scalp. Further studies disclosed bilateral parietal lesions, with destruction of the dura matter and infiltration of the brain parenchyma. Thrombocytopenia was found. Faint enhancement of the lesions was seen on magnetic resonance imaging (MRI) after i.v. administra

Paragangliomas of the head and neck are preferably treated surgically. Planning the surgical approach for temporal bone paragangliomas is performed according to the Fisch classification. Small temporal paragangliomas can be removed in a transtympanic or transmastoidal procedure. Locally advanced paragangliomas of the head and neck have to be embolized presurgically. An occlusion test is also recommended to check the possibility of a resection of the internal carotid artery. Type C and D temporal bone paragangliomas can be removed by different infratemporal approaches. Alternatively, some type

Lymphoplasmacytic lymphoma (LPL) and multiple myeloma (MM) are distinct B-cell neoplasms whose synchronous occurrence is extremely rare. We report a case of a 68-year-old man presenting with fatigue and bone pain. Laboratory studies showed hyperglobulinemia (71.1 g/L), severe normocytic anemia (hemoglobin 44 g/L, MCV 79.5 fL), and thrombocytopenia. Serum immunofixation revealed dual IgG-kappa and IgM-kappa monoclonal proteins. Imaging showed diffuse osteolytic lesions in the skull and spine. Bone marrow flow cytometry identified two immunophenotypically distinct populations: CD19+/CD20+/CD5-/C

Neuroendocrine tumors (NET) are a heterogeneous group of neoplasms that arise from neuroendocrine cells and commonly originate in the GI tract, lungs, and pancreas, but can also present in the skull base and paranasal sinuses. While fluorodeoxyglucose positron emission tomography (FDG PET) scans are routinely used in the evaluation of sinonasal malignancies, their limitations in detecting recurrences near the cranium and cervical lymph node metastases in NETs such as olfactory neuroblastoma (ONB) and sinonasal neuroendocrine carcinomas (SNEC) have prompted the search for an alternative imaging

Neuroblastomas are the most common extracranial solid tumor in the pediatric age group (~8%-10% of childhood neoplasms). Most cases of intracranial neuroblastomas occur due to metastasis from some primary extracranial sites and are known as secondary neuroblastomas. However, the occurrence of primary central nervous system neuroblastomas (PCN-NB) is very rare, and only a few cases and case series have been reported in the literature. PCN-NB is mainly an intra-axial pathology, and extra-axial involvement is mainly due to metastasis from some extracranial primary site with involvement of the sku

Ectopic pituitary tumors are neoplasms with no connection to the pituitary gland and are commonly deposited in other areas of the anterior skull base. A 32-year-old woman presented with a 3-month history of right-sided facial weakness, sensorineural hearing loss, diplopia, and severe headaches. Physical examination revealed a mid-dilated sluggishly reactive right pupil with slight limitation in all gazes, as well as right-sided orbicularis weakness, lagophthalmos, and decreased facial sensation. A magnetic resonance imaging (MRI) of the head without contrast revealed a 3.7 × 1.8 × 2.6 cm mildl

Paragangliomas are extra-adrenal catecholamine-secreting neuroendocrine tumors that can present with adrenergic signs and symptoms. The urinary bladder is a rare location for a paraganglioma, and these tumors must be distinguished from other more common bladder neoplasms. In this case report, we discuss a 59 year-old woman who initially presented with tachycardia, palpitations, chest tightness, shortness of breath, and weight loss. Laboratory evaluation showed significantly elevated catecholamines in the plasma and urine. A CT (Computed Tomography) scan of the abdomen and pelvis revealed an en

The transoral approach is considered the gold-standard surgical route for performing anterior odontoidectomy and ventral decompression of the craniovertebral junction for pathological conditions that result in symptomatic cervicomedullary compression, including basilar invagination, rheumatoid pannus, platybasia with retroflexed odontoid processes, and neoplasms. Extended modifications to increase the operative corridor and exposure include the transmaxillary, extended "open-door" maxillotomy, transpalatal, and transmandibular approaches. With the advent of extended endoscopic endonasal skull

The first United States 201 cobalt-60 source gamma knife for stereotactic radiosurgery of brain tumors and arteriovenous malformations became operational at the University of Pittsburgh on August 14, 1987. Four and one-half years of intensive planning, regulatory agency review, and analysis of published results preceded the first radiosurgical procedure. Installation of this 18,000-kg device and loading of the 201 cobalt-60 sources posed major challenges in engineering, architecture, and radiophysics. In the first 4 months of operation, we treated 52 patients (29 with arteriovenous malformatio

OBJECTIVE: Chordomas are locally aggressive neoplasms of the spine or skull base that arise from embryonic remnants of the notochord. Intradural chordomas represent a rare subset of these neoplasms, and few studies have described intradural chordomas in the spine. This review evaluates the presentation, management, and outcomes of intradural spinal chordomas.
METHODS: A systematic review of PubMed/MEDLINE, EMBASE, Cochrane Library, Scopus, and Web of Science was performed. Studies describing at least 1 case of intradural chordomas anywhere in the spine were included. Extracted details include

BACKGROUND: Aneurysmal bone cysts (ABCs) of the skull are rare, particularly those arising in the occipital bone. Intracranial extension or dural destruction is extremely uncommon. Here, we report an occipital ABC-like lesion that recurred 19 months after gross-total resection with dural defects, intradural extension, and molecular evidence supporting aggressive neoplastic evolution.
CASE DESCRIPTION: A 37-year-old woman presented with 2 months of headache and 1 month of binocular diplopia. CT and MRI demonstrated a large expansile lytic lesion of the occipital bone with poorly defined interf

Surgical procedures for pediatric suprasellar tumors include craniotomy and nasal surgery. Craniotomies are increasingly performed with a microscope, exoscope, and endoscope, while nasal surgery is performed mainly with endoscopes. In some cases, craniotomy and nasal surgery are performed together. This article describes craniotomy. Due to its large surgical field, craniotomy is often chosen for large tumors in the suprasellar region, tumors located above the optic chiasm and optic nerve, tumors extending laterally, and hypervascularized tumors. It is the surgical technique of choice for many
