Although obstructive sleep apnea (OSA) has been studied in detail for over a decade, the mortality of this disorder is unclear. We calculated cumulative survival in 385 male OSA patients. We found that those with an apnea index (AI) greater than 20 had a much greater mortality than those with AI = less than 20. The probability of cumulative eight-year survival was .96 +/- 0.02 (SE) for AI = less than 20 vs. 63 +/- 0.17 for AI greater than 20 (p less than .05). This difference in mortality related to AI was particularly true in the patients less than 50 years of age in whom mortality from other

OBJECTIVE/BACKGROUND: Previous studies have shown that obstructive sleep apnoea (OSA) is associated with reduced delta EEG and increased beta EEG power and increased EEG slowing ratio. There are however no studies that explore differences in sleep EEG between positional obstructive sleep apnoea (pOSA) and non-positional obstructive sleep apnoea (non-pOSA) patients.
PATIENTS/METHODS: 556 of 1036 consecutive patients (246 of 556 were female) undertaking polysomnography (PSG) for the suspicion of OSA met the inclusion criteria for this study. We calculated power spectra of each sleep epoch using

OBJECTIVE: Obstructive Sleep Apnea (OSA) in children is treated primarily with adenotonsillectomy (AT). When clinical exam demonstrates small tonsils, the success of AT in resolving OSA is uncertain. The purpose of this study is to determine the utility of Drug induced Sleep Endoscopy (DISE) for children with OSA and small tonsils (Brodsky scale 1+) and to identify what obstructive trends exist in this subset of patients and to determine the utility of DISE-directed surgical intervention in patients with small tonsils.
METHODS: A retrospective chart review was performed for patients who under

BACKGROUND: Obstructive sleep apnea (OSA) is caused by partial or complete obstruction of the upper airways. Corrective surgeries aim at removing obstructions in the nasopharynx, oropharynx, and hypopharynx. OSA is associated with an increased risk of various metabolic diseases. Our objective was to evaluate the effect of surgery on the plasma metabolome.
METHODS: This study included 39 OSA patients who underwent Multilevel Sleep Surgery (MLS). Clinical and anthropometric measures were taken at baseline and five months after surgery.
RESULTS: The mean Apnea-Hypopnea Index (AHI) significantly

During the past decades obesity and diabetes have become increasingly common in modern, industrialized societies. At the same time sleep disorders, chronic sleep loss and sleep deprivation have also become more and more prevalent. There may be a positive feed back circle between the two disorders: sleep problems may affect endocrine function and metabolic conditions, while metabolic abnormalities potentially interfere with sleep regulation. Sleep-disordered breathing, obstructive sleep apnea in particular, has the strongest association with glucose metabolism. Prevalence and severity of obstru

The aims of this study are to clarify whether patients with obstructive sleep apnea syndrome (OSAS) have a decline in verbally or visually-based cognitive abilities and whether the possible decline is related to particular sleep depth changes. In addition, the effect of continuous positive airway pressure (CPAP) on the possible changes is investigated. Fifteen OSAS patients and 15 healthy controls joined two full-night polysomnographies, including a computational measure of deep sleep percentage (DS%) bilaterally from the frontal, central and occipital channels, and a neuropsychological assess

Sixty patients diagnosed with obstructive sleep apnea syndrome (OSAS) underwent uvulopalatopharyngoplasty (UPPP). The effects of surgery were studied based on endoscopic findings during drug-induced sleep and determination of the apnea-hypopnea index (AHI) before and after the operation. Changes in the form of the airway during sleep in the recumbent position were observed, and the role of upper airway endoscopy in the diagnosis and surgical treatment of OSAS was determined. The site of airway obstruction during sleep induced by i.v. injection of 10 mg of diazepam was classified into five type

Abnormal central regulation of upper airway muscles may contribute to the pathophysiology of the childhood obstructive sleep apnea syndrome (OSAS). We hypothesized that this was secondary to global abnormalities of ventilatory control during sleep. We therefore compared the response to chemical stimuli during sleep between prepubertal children with OSAS and controls. Patients with OSAS aroused at a higher PCO2 (58 +/- 2 vs. 60 +/- 5 Torr, P < 0.05); those with the highest apnea index had the highest arousal threshold (r = 0.52, P < 0.05). The hypercapnic arousal threshold decreased after treat

Obstructive sleep apnea (OSA) is the most common sleep-related breathing disorder and frequently prompts referral to ENT practices. Following a structured medical history and clinical examination, ambulatory diagnosis is typically performed using cardiorespiratory polygraphy which records key parameters such as the apnea-hypopnea index (AHI), oxygen desaturation index (ODI), and oxygen saturation. Modern devices allow for patient-friendly use and computer-assisted data analysis but still require clinical expertise for accurate review and interpretation of the data. New diagnostic tools such as

BACKGROUND: Obstructive sleep apnea (OSA) is a highly prevalent and heterogeneous disorder associated with substantial cardiometabolic morbidity. Although continuous positive airway pressure (CPAP) remains first-line therapy, long-term effectiveness is frequently limited by suboptimal adherence. Advances in airway devices, surgical techniques, neuromodulation, and pharmacologic therapies have expanded the therapeutic landscape and created opportunities for individualized, mechanism-based treatment.
METHODS: We conducted a selective, narrative review with structured quantitative synthesis of r

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent but often underdiagnosed sleep disorder associated with intermittent hypoxia and fragmented sleep. It has been increasingly implicated in the pathogenesis of cardiac rhythm disorders (CRDs), including atrial fibrillation (AF), bradyarrhythmia's, and ventricular arrhythmias. Despite substantial global evidence, limited Indian data exist regarding the burden and characteristics of CRDs among OSA patients.
OBJECTIVE: The objective is to determine the prevalence and types of CRD in patients diagnosed with OSA and explore their correlation w

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent sleep disorder characterized by intermittent hypoxia and oxidative stress, which can impair mitochondrial function and accelerate cellular aging. Mitochondrial DNA copy number (mtDNA-CN) is a useful biomarker of mitochondrial health. We conducted a cross-sectional study investigated differences in mtDNA-CN between individuals with severe OSA and those without severe OSA.
METHODS: We analyzed 97 individuals who underwent overnight polysomnography and blood tests at Taichung Hospital, Taiwan, between March 1 and December 31, 2022. Leukocy

This overview examines the craniofacial and myopathic endotypes of obstructive sleep apnea in relation to the spatial matrix hypothesis. Recently, attention has been drawn to the phenotype of obstructive sleep apnea since this condition includes neurologic, metabolic, craniofacial and myopathic endotypes. Most often, practitioners of dental sleep medicine focus their attention on the manipulation of the mandible for therapeutic purposes, but the etiology of the craniofacial endotype may reside elsewhere. Specifically, in line with the cranio-caudal gradient of development, consideration needs

Obstructive sleep apnoea-hypopnea syndrome (OSAHS) is characterized by the recurrent occurrence of apnoea and hypopneas during sleep. OSAHS is an independent risk factor for cardiovascular diseases, including hypertension, coronary artery disease, arrhythmias, and stroke. The high prevalence of OSAHS among patients with psychiatric disorders, along with the reciprocal links between these two categories of conditions, deserve emphasis. This association indeed carries diagnostic, prognostic, and therapeutic implications. Diagnosis poses specific challenges, including difficulties to perform slee

Obstructive sleep apnea (OSA) is a heterogeneous disorder with varying clinical presentations and pathophysiologic mechanisms. Multiple studies demonstrated increased cardiovascular morbidity and mortality, impaired daytime function and quality of life in moderate to severe OSA. New sleep study metrics are being developed to better predict OSA-related cardiovascular risk that may help to target therapy to patients most likely to benefit. Novel treatments, including hypoglossal nerve stimulation, as well pharmacologic approaches, provide options to personalize therapy.
