Evaluation of Antibiotic Duration after Surgical Debridement of Necrotizing Soft Tissue Infection.

Background: Necrotizing soft tissue infection (NSTI) is known to be a medical emergency with high morbidity and mortality. Guidelines do not specify the optimal duration of antibiotic agents after completion of surgical debridements of NSTI, which has created variable practice. It was hypothesized that patients with NSTI who receive 48 hours or less of post-operative antibiotic agents after final debridement have similar rates of subsequent intervention or infection recurrence, suggesting that a shorter duration of antibiotic agents may treat NSTI adequately after final surgical debridement. Diagnostic Challenge of Necrotizing Soft Tissue Infection in a Patient With B Lymphocytic Leukemia.

The necrotizing soft tissue infection is a rare surgical emergency. Early diagnosis and timely treatment can contribute to better survival and the preservation of the limb. Diagnosis of this pathology remains a challenge to the clinician at the initial stage of the disease, especially if the patient is immunocompromised. We present the case of a 75-year-old man with B lymphocytic leukemia who was diagnosed with necrotizing soft tissue infection but failed to exhibit typical clinical and laboratory measurements. This case highlights the difficulty of early diagnosis in hematology patient, alter

Necrotizing Soft Tissue Infection and Perforated Viscus After Suction-Assisted Lipectomy.

Suction-assisted lipectomy (SAL) is a commonly performed cosmetic surgery in the United States and has been steadily increasing in popularity over the past few years. As more of these surgeries are performed, several rare but life-threatening complications are being recognized, including necrotizing soft tissue infections (NSTIs). NSTIs require rapid surgical intervention but can be challenging to diagnose, as skin manifestations may be difficult to differentiate from normal post-SAL changes. We present a case of a 44-year-old female who presented with signs of septic shock after SAL of her ab…

Hepatic/splenic abscess and/or skin and soft tissue infection as predictors of melioidosis in children.

INTRODUCTION: Melioidosis can have multiple organ involvement which can then mimic other infections. The aim of this study was to determine if there are any factors significantly associated with melioidosis which can inform diagnostic evaluations before receiving the results of confirming laboratory testing. METHODOLOGY: The charts of patients aged < 16 years admitted to Songklanagarind Hospital during 2002-2014 with a clinical presentation suspicious of melioidosis were reviewed. RESULTS: Of the 145 suspected cases, 27 patients had a confirmed diagnosis of melioidosis by either serology and

Death related to Cedecea lapagei in a soft tissue bullae infection: a case report.

BACKGROUND: Cedecea lapagei bacterium was discovered in 1977 but was not known to be pathogenic to humans until 2006. In the medical literature there are very few clinical case reports of Cedecea lapagei; none have reported a catastrophic death secondary to a soft tissue hemorrhagic bullae infection. As well as soft tissue infection, rare cases of pneumonia, urinary tract infections, peritonitis, osteomyelitis, bacteremia, and sepsis have been documented with the majority having good outcomes. Here, we present the first case of a fatal outcome in a Cedecea lapagei soft tissue infection with mu

Retroperitoneal Necrotizing Soft Tissue Infection in a Healthy Adult Masquerading as Acute Pancreatitis.

Necrotizing soft tissue infections (NSTIs) are polymicrobial infections with high morbidity and mortality. We report a case of retroperitoneal NSTI in an immunocompetent young male, with clinical symptoms and signs mimicking acute pancreatitis (pseudo-Cullen's and pseudo-Grey-Turner's signs. The initiating lesion was a pustule which progressed and mimics acute pancreatitis. CT scan showed features of retroperitoneal NSTI. After extensive debridement and antibiotics, the patient improved. NSTI of the retroperitoneum is rare. The absence of external clinical signs and anatomical barriers make th…

Clinical Guidelines for the Antibiotic Treatment for Community-Acquired Skin and Soft Tissue Infection.

Skin and soft tissue infection (SSTI) is common and important infectious disease. This work represents an update to 2012 Korean guideline for SSTI. The present guideline was developed by the adaptation method. This clinical guideline provides recommendations for the diagnosis and management of SSTI, including impetigo/ecthyma, purulent skin and soft tissue infection, erysipelas and cellulitis, necrotizing fasciitis, pyomyositis, clostridial myonecrosis, and human/animal bite. This guideline targets community-acquired skin and soft tissue infection occurring among adult patients aged 16 years a

Severe monobacterial necrotizing soft tissue infection by group A Streptococcus: A surgical emergency.

Eight percent of necrotizing soft tissue infections (NSTI) are attributable to group A Streptococci (GAS), and among these, 50% develop streptococcal toxic shock syndrome. The reported mortality associated with NSTI reaches 32%. We present cases of two healthy individuals with minor GAS skin infection which developed to a rapidly progressed NSTI and sepsis despite of the antibiotic treatment, aiming to discuss the lessons learned from the course and management of these patients.

Shortening and secondary relengthening for chronically infected tibial pseudarthroses with poor soft tissues.

BACKGROUND: The treatment of chronically infected tibial pseudarthroses with poor soft tissues ends with amputation on many occasions. Aggressive débridement of bone and soft tissue and reconstruction of the extremity, performed as a limb salvage procedure, is an alternative treatment option to amputation. METHODS: Our patients had a mean age of 42 years. According to the Paley classification, one of the patients had A2 pseudoarthrosis, four had B2, and three had B3. One had localized infection, whereas the other seven had diffuse infection, according to the Cierny-Mader system. The mean dura

Skin and soft tissues. Management of four common infections in the nursing home patient.

Common skin and soft tissue infections in nursing home patients include herpes zoster, cellulitis, pressure ulcer infections, and scables. Treatment of shingles with an oral antiviral should be started within 24 hours of symptom onset. Dissemination and bacterial superinfection require antibiotic therapy. Use of corticosteroids to prevent post-herpetic neuralgia remains controversial. Cellulitis is most often caused by Staphylococcus aureus and beta-hemolytic streptococci (groups A and B). Therapy for cellulitis is empiric; gram-negative bacilli should be covered in diabetic patients. Most pre…

[The studies on clinical efficacy of Tomiron in the surgical patients: skin and soft tissue infection].

We studied the clinical efficacy and prophylactic effect of Tomiron (cefteram pivoxil; CFTM-PI), a new oral cephalosporin, in the surgical patients. CFTM-PI was administered at a daily dose of 300 or 600 mg after meals. Infectious diseases in 151 patients consisted of infected atheroma, wound infection, subcutaneous abscess, furuncle, phlegmon, mastitis, lymphangitis, periproctal abscess and biliary tract infection. Fifty patients were administered CFTM-PI to prevent secondary wound infection. The clinical efficacy rate was 75.5% in the treatment of infection and 86.0% in the prophylactic use.

Soft tissue infection around a skin penetrating osseointegrated implant. A case report.

A case is reported where a female patient with bilateral otosclerosis received a bone-anchored and skin-penetrating titanium implant on which a hearing aid was mounted to improve conductive hearing loss. The patient developed an infection that did not cease despite intensive local treatment and skin-grafting. Eventually the implant was removed. The histological examination of the interface between implant and surrounding bone and soft tissues showed an inflammatory reaction in the superficial parts of the soft tissues whereas the deeper portions of the soft tissues and all of the bone tissue w

Necrotizing Soft Tissue Infections: Intensive Care Unit (ICU) Survivor's Long-Term Functional Outcomes and Quality of Life.

Introduction Necrotizing soft tissue infections (NSTIs) are rare, rapidly progressing infections of the skin, fascia, and muscle causing necrosis, frequently requiring intensive care unit (ICU) admission. Treatment includes surgical debridement, organ support, antibiotics, and hyperbaric oxygen therapy (HBO). NSTIs require aggressive debridement, leaving survivors with wounds and functional deficits. Quality of life (QoL) is decreased in NSTI survivors, including physical and mental health. Our goal was to analyze the long-term outcomes of patients admitted to the ICU for NSTIs. We evaluated s…
