Functional correlates of the speech-in-noise perception impairment in dyslexia: an MRI study.

Dyslexia is a language-based neurodevelopmental disorder. It is characterized as a persistent deficit in reading and spelling. These difficulties have been shown to result from an underlying impairment of the phonological component of language, possibly also affecting speech perception. Although there is little evidence for such a deficit under optimal, quiet listening conditions, speech perception difficulties in adults with dyslexia are often reported under more challenging conditions, such as when speech is masked by noise. Previous studies have shown that these difficulties are more pronou

The role of the binaural test in filtered speech audiometry.

The value of the binaural resynthesis test administered according to Matzker's principle can be estimated only when a comparable monaural test can be used as a control. The most common positive finding in the filtered speech test is asymmetrical discrimination. Both parts of the message delivered to either ear must therefore have the same intelligibility in normal material. Binaural intelligibility is highly resistant to degenerative changes in the auditory system, for example. Positive findings in the binaural test are encountered frequently in brain stem lesions with vascular or traumatic ae

Electropalatography in home training of retracted articulation in a Swedish child with cleft palate: effect on articulation pattern and speech.

The aim was to evaluate the effectiveness of electropalatography (EPG) in home training of persistent articulation errors in an 11-year-old Swedish girl born with isolated cleft palate. The /t/ and /s/ sounds were trained in a single subject design across behaviours during an eight month period using a portable training unit (PTU). Both EPG analysis and perceptual analysis showed an improvement in the production of /t/ and /s/ in words and sentences after therapy. Analysis of tongue-contact patterns showed that the participant had more normal articulatory patterns of /t/ and /s/ after just 2 m

Radial forearm-palmaris longus tenocutaneous free flap; implication in the repair of the moderate-sized postoncologic soft palate defect.

BACKGROUND: Moderate resection of the soft palate results in suboptimal outcomes in terms of postoperative velopharyngeal function. We propose the radial forearm tenocutaneous free flap incorporating the palmaris longus tendon for reconstruction of the levator sling in these cases. METHODS: Twenty-six patients underwent reconstruction with this method. Group I defect involved up to one-fourth of the soft palate. Group II defect involved up to one-half of the soft palate whether or not including the uvula. Group III defect involved more than three-fourths of the soft palate. Postoperative func

Influence of speech aid prosthesis with speech therapy on speech outcomes in adult patients with persisting velopharyngeal insufficiency: A retrospective analysis.

STATEMENT OF PROBLEM: Prosthetic intervention with a speech aid prosthesis (SAP) along with speech therapy has been reported to improve speech outcomes in patients with persisting velopharyngeal insufficiency (VPI). However, little is known regarding the impact of this treatment on different speech parameters over time in adult patients with VPI. PURPOSE: The purpose of this retrospective study was to evaluate the change in speech parameters after rehabilitation with an SAP and speech therapy in adult patients with VPI using subjective and objective tests. MATERIAL AND METHODS: Patients abov

Development and validation of a digits-in-noise hearing test in Persian.

OBJECTIVE: The prevalence of unrecognised and late-diagnosed hearing loss is higher in low- and middle-income than in high-income countries, due in part to lack of access to hearing services. Because hearing screening is important for early identification of hearing loss, development of an accessible, self-screening test that can detect hearing loss reliably and quickly would provide significant benefits, especially for underserved populations. This study aimed to develop and validate a new version of the digits-in-noise (DIN) test for Persian speaking countries. DESIGN: Recordings of Persian

Speech characteristics one year after first Belgian facial transplantation.

OBJECTIVES/HYPOTHESIS: Facial transplantation has progressed over the past 8 years. We did the first Belgian facial transplantation by vascularized composite tissue allotransplantation and report the 1-year follow-up regarding speech and oromyofunctional behavior. STUDY DESIGN: Outcome study. METHODS: The recipient, a 56-year-old man, had his face severely injured due to a ballistic injury. In January 2012, in a 20-hours surgical procedure, a digitally planned facial composite tissue allotransplantation was performed consisting of a large amount of bone together with the soft tissue of the e

Social perception of speech in individuals with oropharyngeal reconstruction.

Oral cancer affects approximately 5% of the Canadian population every year. One option for treatment of oropharyngeal cancer includes resection of the diseased tissue with primary reconstruction of the defect using a microvascular free flap, followed by post-operative adjuvant radiation therapy. The aim of reconstructive surgery is to maintain functional speech and swallowing. While the literature provides support for the maintenance of speech intelligibility following reconstructive procedures, certain aspects of resonance may be altered when the palatal structures are involved. Little is kno

Randomised Controlled Trial Evaluating Active versus Passive Waiting for Speech-Language Pathology.

INTRODUCTION: High demand for speech-language pathology means children sometimes wait over 12 months for services, missing out on timely support. Waiting can be a time of stress, concern, and powerlessness for caregivers. Provision of information via a website may support families and encourage active waiting. OBJECTIVE: The aim of this study was to compare children's speech, intelligibility, language, and literacy outcomes, and caregivers' satisfaction and empowerment in active versus passive waiting conditions. METHODS: Ninety-seven preschool-aged children referred to a community health sp

A Comparison of Personal Sound Amplification Products and Hearing Aids in Ecologically Relevant Test Environments.

PURPOSE: The aim of this study was to compare the benefit of self-adjusted personal sound amplification products (PSAPs) to audiologist-fitted hearing aids based on speech recognition, listening effort, and sound quality in ecologically relevant test conditions to estimate real-world effectiveness. METHOD: Twenty-five older adults with bilateral mild-to-moderate hearing loss completed the single-blinded, crossover study. Participants underwent aided testing using 3 PSAPs and a traditional hearing aid, as well as unaided testing. PSAPs were adjusted based on participant preference, whereas the

Speech and language therapists' insights into severity of speech sound disorders in children for developing the speech sound disorder severity construct.

BACKGROUND: Children with speech sound disorders (SSD) are at higher risk of communication breakdown, but the impact of having an SSD may vary from child to child. Determining the severity of SSD helps speech-language therapists (SLTs) to recognise the extent of the problem and to identify and prioritise children who require intervention. AIMS: This study aimed to identify severity factors for SSD in order to develop a multifactorial Speech Sound Disorder Severity Construct (SSDSC) using SLTs' views and the International Classification of Functioning, Disability and Health (ICF). METHOD: In

Prosthetic management of soft palate cleft--a case report.

The treatment of cleft in the soft palate can be achieved essentially by surgical reconstruction or prosthetic obturation. The goals of prosthetic intervention are to prevent food and liquid leakage into the nose and to improve speech intelligibility by ensuring functional velopharyngeal port closure. In this report, the prosthetic management of a 5-year-old child with soft palatal cleft is presented. The impression of the cleft was taken with tissue conditioner, Visco -gel, TM (De Trey, Amalgamated Dental London) and the pharyngeal obturator (speech bulb) was made in clear acrylic resin. Pati

An examination of the practicality of the simplex procedure.

OBJECTIVE: The practical importance of the simplex procedure, a subjective technique used to refine the frequency gain characteristic (FGC) of a hearing aid according to listener preference, was determined for individual listeners by measuring hearing aid benefit using both laboratory studies and field studies. DESIGN: A digital research hearing aid with two memories was used as the test hearing aid. The modified simplex procedure was used to select the FGC judged to yield the best speech clarity in the presence of low-level vent noise and again in higher-level cafeteria noise by 10 experienc

Pathophysiological effects of Möbius syndrome on speech and hearing.

The effects of Möbius syndrome (MoS) on the functioning of the peripheral speech and hearing mechanisms has not hithertofore been described, to my knowledge. Speech and hearing function of an 8-year-old MoS speaker was studied. Data were obtained from several sources. These included: electromyography of the facial muscles, physical examination of the peripheral speech mechanism, speech and language test batteries, audiological evaluation, and acoustic analyses of the speech signal. A pathologic condition was found to exist in a number of cranial nerves and in structures in the oral cavity. Thi

Children and young adults with profound and multiple learning disabilities: Evidence of intelligible subvocal language.

INTRODUCTION: Literature to date describes people with Profound and Multiple Learning Disabilities (PMLD) as pre-linguistic. In contrast, this study explores the existence and use of meaningful sub vocal (SV) language by twenty PMLD participants. METHOD: The SV utterances of 20 PMLD participants were recorded and amplified. Recordings were investigated for evidence of language content and structure, listener intelligibility, and acoustic and phonetic features relative to normal speech and whisper. RESULTS: Language content and structure was identified. Listener intelligibility was demonstrat
