The foramen spinosum, one of the important openings at the base of the cranium, is the opening through which the middle meningeal artery enters the cranium. The variations of the foramen spinosum should be well known to be an important landmark in middle fossa surgeries and to understand better the clinical conditions related to the middle meningeal artery passing through it. A total of 35 bones (32 cranial base and 3 separate sphenoid bones) of individuals of unknown age, sex, and ethnicity in the Laboratory of the Department of Anatomy, Gaziantep University, Faculty of Medicine were examined…

Skull base osteomyelitis is a severe complication of malignant otitis externa that affects the marrow of the temporal, sphenoid, and occipital bones. Skull base osteomyelitis is usually diagnosed based on clinical, microbiological, and radiological findings. Here, we present the imaging findings of a 76-year-old man who initially presented with right-sided malignant otitis externa, with the involvement of the otomastoid structures and ipsilateral temporal bone. Over the following 3 years, despite specific extended antibiotic therapy, the skull base osteomyelitis entirely involved the skull bas

To investigate the characteristics of imaging changes with time of facial fractures, patients with facial fractures who had computed tomographic scan were enrolled including 500 patients who were divided into six groups based on the time of scanning: super early (<3 d), early (4-7 d), early-to-medium (8-14 d), medium (15-21d), medium-to-late (22d-2 months) and late stage (>2 months). The data were compared and analyzed. Forty two patients with frontal bone fractures had high-energy impact as the reason of fractures. The fracture line was clear and sharp within one week but blunt and sclerotic

Fibrous dysplasia of the facial bones frequently occurs in the maxilla around the time of puberty and becomes inactive when skeletal growth is completed. A case of fibrous dysplasia of the maxilla and sphenoidal-temporal bone in a 19 year-old male was studied, in terms of the presence of estradiol, testosterone and dihydrotestosterone, using the peroxidase-antiperoxidase method. This case underwent partial surgical resection, for cosmetic deformity, of the maxillary tumor which, however, relapsed to preoperative size within three weeks. Estradiol and testosterone were strongly positive and dih

Surgery for anterior clinoidal meningioma (CMs) can be daunting, especially when encased in the internal carotid artery (ICA).1 In particula … ral fossa dura from the lateral wall of the cavernous sinus. We then drilled the lesser wing of the sphenoid bone and the roof of the orbital apex to a thin eggshell layer. Finally, we removed the remaining bone piece. Complete extradural drilling of the anterior clinoid process was performed, which allowed us to process the tumor base at the start of dissectio …

OBJECTIVE: Endoscopic endonasal transsphenoidal surgery (EETS) is a common first-line approach for resection of sellar lesions such as pituitary adenomas. While these procedures are largely successful, rare or unexpected complications can occur. One such complication is tension pneumosella (TP). Here, we conducted a systematic review of the literature to provide recommendations on how best to identify, manage and prevent TP post-EETS.
METHODS: A systematic review was conducted using PubMed, Google Scholar, Embase, Cochrane and Web of Science electronic databases according to the Preferred Rep

The anatomy of the paranasal sinuses is variable and it is important to appreciate their clinical and surgical significance. During both posterior ethmoid and sphenoid sinus surgery, but also for endoscopic approach of pituitary, a good knowledge of the anatomy is essential for understanding the pathological process and to prevent surgical complications (optic nerve, internal carotid artery injury during surgery). Although coronal Computerized Tomography (CT) is the routine procedure performed for functional endoscopic sinus surgery (FESS), axial CT is most useful in demonstrating the anatomy

BACKGROUND: There are multiple approaches to reach the sellar region using microscope or endoscope, which are the old sublabial, with its post-operative morbidities, or the uni-nostril transseptal, which has a limited working field. Another technique is the direct transnasal, which is associated with tissue sacrifice, and lastly the bi-nostril (one side transseptal-other side transnasal), aiming to preserve one of the septal flaps.
METHOD: We have 27 case studies using a new technique in which we access the sphenoid endoscopicaly through the nasal septum from both nostrils under the mucoperic

BACKGROUND: Meningitis occurs in 0.8-1.5% of patients undergoing neurosurgery. The aim of the study was to evaluate the characteristics of meningitis after endoscopic endonasal transsphenoidal surgery (EETS) comparing the findings retrieved to those highlighted by literature search.
MATERIALS AND METHODS: Patients treated by EETS during an 18-year period in the Department of Neurosurgery of 'Federico II' University of Naples were evaluated and included in the study if they fulfilled criteria for meningitis. Epidemiological, demographic, laboratory, and microbiological findings were evaluated.

PURPOSE: To evaluate the efficacy of multi-detector row CT (MDCT) findings in the diagnosis of fibrous dysplasia (FD) in orbital bone.
METHODS: The MDCT manifestations of 18 cases of FD in orbital bone (6 cases from the 458th hospital of PLA, 12 cases from the Zhongshan ophthalmic center of Sun Yat-Sen university) collected from April 2002 to August 2009 were reviewed. Six cases were confirmed by biopsy, 5 cases were confirmed by operation and postoperative pathology, the remaining 6 cases were confirmed based on typical CT findings.
RESULTS: FD in orbital bone could be originated from monos

Spheno-orbital meningiomas (SOMs) are complex tumors that grow and extend into nearby structures. SOM tumor growth is often associated with hyperostosis caused by tumor cell infiltration and bone alteration. We describe the case of a 64-year-old man with SOM that extended into the sphenoid sinus without a direct connection between the intracranial and extracranial lesions. This report emphasizes the importance of identifying the growth patterns of SOMs and assessing the paranasal sinuses adjacent to the hyperostotic orbit walls from preoperative images.

We report the case of a 5-year-old boy who presented with a draining cutaneous pit temporal to the lateral canthus, with recurrent periorbital infections. MRI and CT revealed a sinus tract leading from the pit at the skin surface to a 5 mm lesion located in the sphenoid bone near the left sphenofrontal suture. Intraoperative facial nerve monitoring and a lacrimal probe inside the sinus tract were used to guide dissection to the cyst through a minimally invasive temporal approach, without need for neurosurgical intervention.

A 2-year-old boy with blindness as an isolated symptom was found to have no light perception binocularly because of compression of both optic nerves by a neuroblastoma infiltrating the walls of the optic canals and medial sphenoid bone. Imaging disclosed a primary tumor near the kidney and multiple osseous metastases. Although neuroblastoma commonly causes blindness by metastasis to the orbit, it rarely causes bilateral blindness from intracranial compression of the optic nerves. This is the first report of bilateral blindness as the presenting feature.

We present the clinical, radiological, and pathological findings in an elderly man who developed a progressive superior orbital fissure syndrome due to idiopathic hypertrophic cranial pachymeningitis. The unique aspect of this case concerned the increased density of the sphenoid ridge and lateral orbital wall observed by using computed tomography, and the enhancement of the marrow signal seen on magnetic resonance imaging. These neuroimaging abnormalities of bone resulted from an indirect nonspecific response of the marrow to the adjacent soft tissue and dural inflammatory process.

The authors present a surgical approach that incorporates the frontal sinus and extends a supraorbital craniotomy to include the lateral orbital rim and zygoma. The craniotomy provides wide exposure of the anterior fossa, orbit, ipsilateral middle fossa, and cavernous sinus. The procedure can be performed easily, and the bone flaps can be secured rapidly back into the anatomical position at the time of closure. This modified supraorbital craniotomy is ideal for large benign lesions originating along the sphenoid wing or orbit that expand into the anterior fossa.

OBJECTIVE: Although arachnoid cysts and intracranial aneurysms are very common lesions, their association in the same patient is rare. We present a case of a middle cerebral artery aneurysm ruptured into an arachnoid cyst. We found only six cases with intracystic hemorrhage reported in the literature. The presence of an arachnoid cyst can mislead clinical presentation. The patient presented a paradoxically small temporal fossa and thickening of the temporal and sphenoid bone. The authors suggest that this uncommon association (arachnoid cyst, atypical cranial vault, and "mirror-like" cerebral …
