Carotid cavernous sinus fistula caused by dental implant-associated infection.

A 61-year-old woman presented with painful ophthalmoplegia, Tolosa-Hunt syndrome. The patient had undergone a placement of dental implant 5 months before the presentation and had a local maxillary sinusitis 1 month later. She had not been aware of any preceding head trauma or infection. On examination, the patient showed serious right oculomotor nerve paresis and retro-orbital pain. Blood examination showed normal findings. Magnetic resonance imaging identified abnormal structure in the right cavernous sinus with flow void signals. Angiography revealed a carotid cavernous sinus fistula fed by

Anatomic variations of paranasal sinuses-clinical case report.

This article describes the clinical case of an 86-year-old female patient with an asymptomatic clinical condition, without presenting a history of previous traumatisms. When routine radiographic exam-panoramic radiograph-was performed, the following extensions of the paranasal sinuses were found: maxillary sinus (palate), sphenoid sinus (pterygoid process, major wing, and anterior clinoid process), and supraorbital portion of the frontal sinus. Thus computerized tomography was performed to confirm the diagnosis.

[Osteomas of the paranasal sinuses in the Department of Otolaryngology of Karol Marcinkowski University of Medical Sciences in Poznań in 1959-1995].

In years 1959-1995 in the Department of Otolaryngology of University of Medical Sciences in Poznań 112 patients were treated for osteoma within paranasal sinuses. In our material there was 59 men and 52 women. The age range was 13 to 79. Quantitative analysis showed most cases of osteoma in frontal sinus, the second place was taken by ethmoidal cells and very rarely was osteoma observed in maxillary sinus and sphenoid sinus. Operation procedure was performed on all the patients.

Transorbital injury from a harpoon involving the paranasal sinuses.

A harpoon accidentally entered the right eye of a 15-year-old boy. No loss of consciousness occurred. After neuro-radiological assessment, the object which had penetrated the sphenoid sinuses was removed without mishap. He made a good recovery but has, as permanent deficits, an inferotemporal field defect, diminished visual acuity with optic atrophy, a sensory pupillary defect and right ptosis. The need for hospitalisation and a team approach to penetrating injuries of the face is discussed.

Radiographic density profiles link frontal and anterior ethmoid sinuses behavior in chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) may occur through heterogeneous disease processes. It is possible that more than 1 inflammatory process underlies CRS in any given patient. If so, heterogeneity in processes may be a function of the spatial organization of the paranasal sinuses. Density characteristics of sinus opacities on computed tomography (CT) scans offer insight into the nature of sinus opacities and disease, in general, and may thus be used to detect spatial heterogeneity of sinus disease within a given patient. METHODS: The study was a retrospective chart review of CRS patients

Invasive fungal sinusitis and meningitis due to Arthrographis kalrae in a patient with AIDS.

We report the first described case of Arthrographis kalrae pansinusitis and meningitis in a patient with AIDS. The patient was initially diagnosed with Arthrographis kalrae pansinusitis by endoscopic biopsy and culture. The patient was treated with itraconazole for approximately 5 months and then died secondary to Pneumocytis carinii pneumonia. Postmortem examination revealed invasive fungal sinusitis that involved the sphenoid sinus and that extended through the cribiform plate into the inferior surfaces of the bilateral frontal lobes. There was also an associated fungal meningitis and vascul

Endoscopic identification of the pharyngeal (palatovaginal) canal: an overlooked area.

Objective The pharyngeal or palatovaginal canal (PC) is a small tunnel that lies between the sphenoid process of the palatine bone and the vaginal process of the sphenoid bone. Currently, little endoscopic information is available about this region. Design and Setting Endoscopic endonasal cadaveric study. Subjects and Methods Twenty sides in 10 adult cadaver heads were studied endoscopically. The sphenopalatine foramen (SPF) and the adjacent pterygopalatine fossa were exposed. Dissection medial to the vidian canal demonstrated a tunnel that runs posteromedially in the sphenoid floor. Endoscopi

Rare Case of Metastatic Breast Cancer to the Sphenoid Sinus: Diagnosis and Treatment.

INTRODUCTION: Breast cancer (BC) is the most common cancer among women worldwide. While metastases typically affect the lungs, liver, and bones, spread to the paranasal sinuses, especially the sphenoid sinus, is extremely rare. CASE REPORT: A 57-year-old woman with a history of infiltrative ductal carcinoma of the breast diagnosed 12 years earlier presented with progressive left-sided vision blurring and headaches for 3 weeks. Imaging revealed a heterogeneous lesion in the left sphenoid sinus compressing the optic nerve. The metastatic breast carcinoma with histopathological and immunohistoch

Invasive Sphenoid Sinus Aspergillosis in a Patient With Chronic Neutropenia: A Case Report.

Invasive sinus aspergillosis is an uncommon but potentially life-threatening infection typically observed in immunocompromised individuals. However, increasing reports highlight its occurrence in patients with transient or borderline immunosuppression. We present a case of a 75-year-old female with treated mantle cell lymphoma and chronic neutropenia who developed invasive sphenoid sinus aspergillosis. Diagnosis was confirmed via imaging and histopathology following surgical intervention. She was treated with liposomal amphotericin B, followed by a successful transition to oral voriconazole, w

[Septic thrombosis of cavernous sinus: report of 6 cases].

The cavernous sinus is most frequently involved by septic thrombosis. The common sites of primary infection are the medial face, orbits, tonsils, soft palate, sphenoid and ethmoid sinuses. The usual clinical presentation begins with fever and periorbital edema followed by headache, ptosis and ocular muscles palsy. The diagnosis is usually made on clinical grounds. Treatment consists of eradication of the primary source of infection and the administration of antibiotics and anticoagulants. We report six cases of septic thrombosis of cavernous sinus.

A case of the inferior meatus fungus ball.

A fungus ball is usually found in just one sinus, often the maxillary sinus, followed by the sphenoid, ethmoid, and frontal sinuses. We report the case of a fungus ball in the inferior meatus of a 50-year-old woman, which was successfully removed by nasal endoscopic surgery. This is the first case to our knowledge that has been reported at this unusual site. In addition, we hypothesise that "inferior meatus fungus ball" is a disease entity similar to a sinus fungus ball.

Anatomical variations in the human paranasal sinus region studied by CT.

A precise knowledge of the anatomy of the paranasal sinuses is essential for the clinician. Conventional radiology does not permit a detailed study of the nasal cavity and paranasal sinuses, and has now largely been replaced by computerised tomographic (CT) imaging. This gives an applied anatomical view of the region and the anatomical variants that are very often found. The detection of these variants to prevent potential hazards is essential for the use of current of endoscopic surgery on the sinuses. In the present work, we have studied the anatomical variants observed in the nasal fossae a

Localized Amyloidosis of the Sphenoid Sinus: A Case Report and a Descriptive Literature Review.

Amyloidosis is the process of extracellular deposition of protein fibrils and manifests pathologically as a systemic or localized process. Localized amyloidosis of the head and neck is uncommon, and involvement of the sphenoid sinus is exceedingly rare. We describe a case of localized amyloidosis isolated from the sphenoid sinus. A descriptive literature search was conducted to highlight presentation, management, and outcomes related to this pathology. Our patient was a 65-year-old male who presented to our clinic with nasal congestion and an incidental finding of a large expansile mass within

A new plastic model of endoscopic technique training for endonasal transsphenoidal pituitary surgery.

BACKGROUND: Endoscopic transsphenoidal approach is a minimally invasive surgical technique for the removal of sellar and parasellar lesions, which has been progressively accepted by neurosurgeons. However, frustration is often expressed by neurosurgeons when first attempting endoscopic endonasal pituitary surgery. To overcome the learning curve from microscope to endoscope in a smooth way, a new human nasal model has been developed. The present study assessed this new model of the human paranasal sinuses for endonasal surgery training, particularly for endonasal pituitary surgery training. ME

Macroscopic anatomy, radiography and computed tomography of normal paranasal sinuses of the adult one-humped dromedary (Camelus dromedarius).

In order to improve the current knowledge of the topography and the anatomy of the paranasal sinuses of the one-humped dromedary (Camelus dromedarius), we applied both conventional and advanced imaging techniques, namely, radiography and computed tomography (CT). Twelve heads of healthy dromedaries were used; eight heads were dissected to obtain skulls for the fenestration of the sinuses, two heads underwent anatomical sections, and two heads were imaged respectively by radiography and CT. Sinus fenestration allowed observation of sinuses and their communications. In each dissected dromedary's

Results of oral steroid treatment in nasal polyposis.

Twenty-five patients with massive nasal polyposis were treated during four days with 60 mg oral prednisolone (Deltacortril) followed by a progressive reduction of the dose (minus 5 mg per day). Therapeutic efficacy was evaluated by means of symptoms, anterior rhinoscopy, and a CT scan of the sinuses before and at the first visit after the treatment. Seventy-two per cent of the patients showed subjective improvement due to the involution of polyps in the nasal cavity. However, on CT of the paranasal sinuses only 52% showed a clear improvement. In general, the frontal and sphenoid sinuses were m
