Endoscopic sphincterotomy in the treatment of choledocholithiasis and ampullar stenosis. Experience with 202 patients.

Endoscopic sphincterotomy (EST) was performed on 202 patients (265 EST) in the 5-year period 1978-1983 because of choledocholithiasis or benign stenosis of the ampulla of Vater. The indications for EST in choledocholithiasis included residual or recurrent stone(s) in 80 cholecystectomized patients, and 96 had choledocholithiasis with the gallbladder in situ. Ductal calculi passed spontaneously after EST in 87 cases (50%) and were actively extracted in 73 (41%). Ductal clearance failed in 16 patients. The overall success rate thus was 91%. Ampullar stenosis was successfully treated in 26 patien…

Effects of sphincteroplasty and endoscopic sphincterotomy on the bacteriologic characteristics of the common bile duct.

Forty-five patients with sphincter of Oddi stenosis had specimens of common bile duct cultured during ERCP before either sphincteroplasty or endoscopic sphincterotomy. All had sterile bile before sphincter ablation. Bile was recultured 6 to 36 months later during endoscopy at which time 70 percent of the sphincterotomy and 76 percent of the sphincteroplasty patients had bile colonized principally by enteric organisms. Growth was heavy to moderate in most of the patients and contained few nasopharyngeal organisms. Despite bactobilia, no patient had symptomatic cholangitis, presumably due to exc…

[Surgical risk of the Oddi sphincterotomy. Results of an international survey (25541 cases)].

Operative risks of transduodenal sphincterotomy (T.D.S.) are the principal subject of discussion between those surgeons who practice the operation and those that condemn it. An international survey of 25, 541 cases treated by 130 surgeons showed that the incidence of specific complications such as acute pancreatitis (0.6 p. 100), hemorrhage (0,65 p. 100), and early cholangitis (0,5 p. 100), and relative mortality were lower than those usually reported by surgeons attributing an excessive operative risk to the procedure. Data was also obtained indicating that certain "risk factors" due to teh s…

Early endoscopic sphincterotomy for extraction of residual stones of the common bile duct.

Retained calculi of the common bile duct are a significant cause of morbidity after operations upon the biliary tract. When percutaneous extraction is not possible or delay undesirable, endoscopic sphincterotomy is a safe and effective technique even when used in the early postoperative period. Five such patients treated at Beth Israel Medical Center within the past year are presented and discussed. Successful stone extraction was done in all patients, and there was no morbidity or mortality related to the procedure.

Treatment of pancreatitis associated with pancreas divisum by dorsal duct sphincterotomy alone.

Of 480 patients seen with pancreatitis at the Sunnybrook Medical Centre, Toronto, in the past 5 years, 5 had pancreas divisum, demonstrated by pancreatography. Clinical presentations included recurrent acute pancreatitis, chronic pancreatitis and recurrent subcutaneous fat necrosis with steatorrhea. Pancreatography demonstrated dorsal pancreatic drainage through the duct of Santorini in all cases. The luminal diameter at the orifice of the duct of Santorini, assessed at operation, was inadequate to provide normal drainage from the gland. Sphincterotomy of the duct of Santorini alone, without s…

Botulinum toxin injections vs lateral internal sphincterotomy in chronic anal fissure management.

Among the various treatment modalities for chronic anal fissures, lateral internal sphincterotomy, acknowledged as the gold standard, is associated with superior healing rates but entails a higher risk of complications due to its invasive nature. Conversely, botulinum toxin injection, a minimally invasive alternative, remains a viable therapeutic option for select patient populations despite its relatively higher recurrence rate, owing to its favorable safety profile. The objective of this study was to compare the botulinum toxin and sphincterotomy efficacy in the management of chronic anal fi…

Post-ERCP Pancreatitis Risk Factors: Is Post-Sphincterotomy Bleeding Another Risk Factor?

BACKGROUND: Despite advancements in technology and expertise, ERCP carries risks of significant complications, such as pancreatitis, bleedin … iate and multivariate analyses. RESULTS: Age, urgent indication, ERCP history, cholangitis, precut sphincterotomy, common bile duct diameter, pancreatic cannulation, pancreatic stent, and bleeding were statistically significant risk factors for post-ERCP pancreatitis (+). In multivariate analysis, the effects of urgent indication, ERCP history, cholangitis, pre …

Gas spread following endoscopic sphincterotomy: Challenging diagnosis and management: A case reports.

INTRODUCTION AND IMPORTANCE: Subcutaneous emphysema, pneumomediastinum and pneumoperitoneum simultaneously are a rare complication of endoscopic retrograde cholangiopancreatography (ERCP) that usually indicates free perforation to the peritoneal cavity or the retroperitoneal space. CASE PRESENTATION: We report an unusual case of a subcutaneous emphysema, pneumomediastinum and pneumoperitoneum following an ERCP for removal of a common bile duct stone. There was no radiological evidence of peritoneal or retroperitoneal perforation. CLINICAL DISCUSSION: This complication seems to be relative to

Endoscopic papillary large balloon dilation with minor sphincterotomy for periampullary diverticular papilla.

Background To observe the outcome of endoscopic papillary large balloon dilation (EPLBD) with minor sphincterotomy (mEST) for periampullary diverticular papilla related to stone removal. Methods Patients with confirmed periampullary diverticulum (PAD) during stone removal from May 2016 to April 2018 were reviewed retrospectively. The Chi-square test with Yates correction or Fisher's exact test was used for the analysis of categorical data and a normality test was applied for continuous data. Results A total of 154 consecutive patients (89 males and 65 females, aged 51-87 years) with confirmed …
