Immunoneutralization of nerve growth factor in lumbosacral spinal cord reduces bladder hyperreflexia in spinal cord injured rats.

PURPOSE: We investigated the effects of intrathecal application of nerve growth factor (NGF) antibodies (Ab) on bladder hyperreflexia in chronic spinalized rats. MATERIALS AND METHODS: In adult female rats an intrathecal catheter was implanted at the level of the L6 to S1 spinal cord, followed by complete transection of the Th8 to 9 spinal cord. At 10 days after spinalization the intrathecal catheter was connected to an osmotic pump for continuous delivery of vehicle or NGF Ab (10 microg daily) for 2 weeks. Awake cystometry was then performed. NGF levels in the L5 to S1 dorsal root ganglia, L…

Spinal cord seizures: a possible cause of isolated myoclonic activity in traumatic spinal cord injury: case report.

Spinal cord seizures are infrequently reported. They have been associated with intravenous dye placement, transverse myelitis and multiple sclerosis, but never with traumatic spinal cord injury (SCI). We report the case of a 48-year-old SCI male with complete C6 quadriplegia, and apparent spinal cord seizures. These seizures were characterised by myoclonus simplex activity involving the upper extremities only. The lower extremities were spared. The patient was conscious throughout the myoclonic activity and an electroencephalogram of the brain obtained during an event revealed no cortical epil…

Metastatic spinal cord compression. Occurrence, symptoms, clinical presentations and prognosis in 398 patients with spinal cord compression.

We reviewed all medical records concerning patients suffering from spinal cord or cauda equina compression (SCC) secondary to cancer, in the eastern part of Denmark, from 1979 through 1985. During the period the incidence of SCC in cancer patients went up from 4.4% to 6%. However, this increase was not significant. The series comprised 398 cases, with carcinoma of the prostate (19%), lung (18%), breast (14%) and kidney (10%) accounting for 61%. The symptoms were evaluated in accordance with the patients rating of pain, motor deficits, sphincter control and paraesthesia, whereas the clinical ma…

Contractures complicating spinal cord injury: incidence and comparison between spinal cord centre and general hospital acute care.

One hundred and eighty-one spinal cord injured patients admitted to the Rehabilitation Institute of Chicago after acute care following a spinal cord injury were studied. The presence or absence of contractures as well as significant abnormalities with loss of range of motion greater than 15% was recorded. Patients were either admitted from general hospitals or the acute care unit of our spinal centre, Northwestern Memorial Hospital. Patients treated in the general hospitals had a statistically significant increased incidence of contractures compared to spinal centre patients. Patients treated …

Breaking barriers: Understanding and managing blood-spinal cord barrier dysfunction in spinal cord injury: A systematic review.

BACKGROUND: Spinal cord injury (SCI) has life-changing consequences, not just from the initial trauma, but also from a series of damaging processes that unfold afterwards, such as oxidative stress, inflammation, and nerve cell loss. At the heart of these events is the breakdown of the blood-spinal cord barrier (BSCB). When BSCB fails, harmful substances start flooding in, leading to progressive tissue damage. METHODS: In this review, we explore findings from 38 studies - including animal models and patient research - to shed light on how and why the BSCB fails after SCI, as well as what can b…

Processing and sectioning of organ donor spinal cord tissue for electrophysiology on acute human spinal cord slices.

Acute spinal cord slice electrophysiology is a powerful technique used in preclinical basic science research to investigate sensory and motor neuron function and pathophysiology. A major barrier that stands between implementing these findings into effective clinical treatments is the translational gap between rodent models and human patients. To date, no methods or protocols describe how to prepare viable human spinal cord slices for acute electrophysiological recordings. To bridge this translational divide, we describe here a protocol for the extraction of spinal cord tissue from consenting h…

Anti-Nogo-A NG101 treatment induces changes in spinal cord micro- and macrostructure following spinal cord injury.

NG101 is a recombinant antibody that neutralizes the nerve growth inhibitor Nogo-A, promoting neural repair and improving upper extremity motor function in spinal cord injury (SCI). This study evaluated spinal cord MRI biomarkers to detect treatment-related structural changes and enhance patient stratification using data from 106 participants with acute cervical SCI in the phase 2b NISCI trial. We assessed lesion volume, tissue bridges, and remote changes in cross-sectional cord area (CSA), and tract-specific myelin-sensitive magnetization transfer saturation (MTsat) over six months. Compared …

Activity-based recovery training with spinal cord epidural stimulation improves standing performance in cervical spinal cord injury.

BACKGROUND: Individuals with a clinically complete spinal cord injury are unable to stand independently without external assistance. Studies have shown the combination of spinal cord epidural stimulation (scES) targeted for standing with activity-based recovery training (ABRT) can promote independence of standing in individuals with spinal cord injury. This cohort study aimed to assess the effects of stand-ABRT with scES in individuals with cervical chronic spinal cord injury. We evaluated the ability of these individuals to stand independently from physical assistance across multiple sessions…

Temporal transcriptomic dynamics in rat spinal cord after tibial fracture unveil crosstalk between spinal cord and bone.

Spinal cord injury induces progressive bone loss and increases fracture susceptibility. While the neurological consequences of skeletal trauma remain poorly characterized, underlying molecular mechanisms require systematic investigation. To systematically characterize the temporal dynamics of spinal cord responses, L4-L5 spinal segments were harvested from Sprague-Dawley rats at 0, 3, 7, 14, and 28 days post-fracture for RNA sequencing (Illumina platform). Integrated bioinformatics analyses revealed temporally stratified differentially expressed genes (DEGs). Quantitative real-time reverse tra…

Effectiveness of transcutaneous spinal cord stimulation for lower limb rehabilitation in spinal cord injury: a systematic review.

BACKGROUND: Spinal cord injury (SCI) results in devastating motor and sensory deficits below the level of injury. Transcutaneous spinal cord stimulation (tSCS) has emerged as a promising non-invasive neuromodulation technique for improving motor function in individuals with SCI. However, the effectiveness of tSCS for lower limb rehabilitation remains unclear. OBJECTIVE: To systematically review the effectiveness of tSCS for improving lower limb motor function, walking ability, spasticity, and functional independence in individuals with SCI. METHODS: We conducted a comprehensive search of Pub…

Functional synaptic connectivity of engrafted spinal cord neurons with hindlimb motor circuitry in the injured spinal cord.

Spinal cord injury (SCI) results in significant neurological deficits, and curative therapies are lacking. Neural progenitor cell (NPC) transplantation shows promise, as graft-derived neurons (GDNs) can integrate into host spinal cord and support axon regeneration. Here, we examined the synaptic integration of GDNs into hindlimb motor circuits in a mouse thoracic contusion SCI model. Transsynaptic tracing revealed that GDNs form synaptic connections with host motor circuits. Axon mapping showed distinct termination patterns of cholinergic and V2a interneurons within host spinal cord. Chemogene…

Teriparatide: an innovative and promising strategy for protecting the blood-spinal cord barrier following spinal cord injury.

The blood-spinal cord barrier (BSCB) is disrupted within minutes of spinal cord injury, leading to increased permeability and secondary spinal cord injury, resulting in more severe neurological damage. The preservation of blood-spinal cord barrier following spinal cord injury plays a crucial role in determining the prognosis. Teriparatide, widely used in clinical treatment for osteoporosis and promoting fracture healing, has been found in our previous study to have the effect of inhibiting the expression of MMP9 and alleviating blood-brain barrier disruption after ischemic stroke, thereby impr…

Spinal Cord Injury Pain Instrument and painDETECT questionnaire: Convergent construct validity in individuals with Spinal Cord Injury.

BACKGROUND: Neuropathic pain (NeuP) is a frequent sequel of spinal cord injury (SCI). The SCI Pain Instrument (SCIPI) was developed as a SCI-specific NeuP screening tool. A preliminary validation reported encouraging results requiring further evaluation in terms of psychometric properties. The painDETECT questionnaire (PDQ), a commonly applied NeuP assessment tool, was primarily validated in German, but not specifically developed for SCI and not yet validated according to current diagnostic guidelines. We aimed to provide convergent construct validity and to identify the optimal item combinati…

Electrical neuromodulation of the cervical spinal cord facilitates forelimb skilled function recovery in spinal cord injured rats.

Enabling motor control by epidural electrical stimulation of the spinal cord is a promising therapeutic technique for the recovery of motor function after a spinal cord injury (SCI). Although epidural electrical stimulation has resulted in improvement in hindlimb motor function, it is unknown whether it has any therapeutic benefit for improving forelimb fine motor function after a cervical SCI. We tested whether trains of pulses delivered at spinal cord segments C6 and C8 would facilitate the recovery of forelimb fine motor control after a cervical SCI in rats. Rats were trained to reach and g…

Mst3b promotes spinal cord neuronal regeneration by promoting growth cone branching out in spinal cord injury rats.

Spinal cord injury is a severe clinical problem, and research searching activity molecular that can promote spinal cord injury repairing is very prevalent. Mst3b can promote repair of damaged peripheral nerves and the optic nerve, but has been rarely reported in spinal cord injury research. Through detecting its expression in different periods of injured spinal cord, we found that the expression of Mst3b was significantly upregulated in injured spinal cord neurons. Increasing Mst3b expression using adenovirus in vivo and in vitro promoted axonal regeneration of spinal cord neurons, which led t…

Diagnosis of incomplete conduction block of spinal cord from skin surface using spinal cord evoked magnetic fields.

BACKGROUND: We previously reported the usefulness of neuromagnetic recordings for the diagnosis of disorders in peripheral nerves or the spinal cord. However, there have been no reports on incomplete conduction block of the spinal cord, which is clinically common in conditions such as cervical myelopathy. Here, we estimated the usefulness of measuring spinal cord evoked magnetic fields for evaluating incomplete conduction block. METHODS: Incomplete conduction block models of the spinal cord of the rabbit were established using a Fogarty balloon catheter that was inserted into the epidural spa…
