Parasitic Persistence: The Diagnostic Challenge of Strongyloidiasis in a Nonendemic Setting.

Strongyloidiasis is a parasitic infection endemic to warm and moist climates, which is caused by the intestinal nematode Strongyloides stercoralis. In the United States, most cases are seen in immunocompromised patients residing in the Northeast and East South-Central regions and in travelers from endemic regions. While often underrecognized, Strongyloides infection should remain a key consideration in patients presenting with atypical gastrointestinal symptoms, with a relevant exposure history. We present a case of a 45-year-old male initially diagnosed with Helicobacter pylori whose symptoms…

Strongyloidiasis Diagnosed by ELISA in Brussels: A Bicentric Retrospective Study.

Despite a significant population of migrants from endemic countries and of Europeans traveling to these regions, no Belgian study and few European studies have reported on strongyloidiasis. This retrospective study, conducted between January 2012 and December 2021, aimed to compare demographic data, clinical characteristics and management appropriateness of adult patients from Hôpital Universitaire de Bruxelles-Erasme and Centre Hospitalier Universitaire Saint-Pierre who had a positive serology for Strongyloides stercoralis. Serology results were classified as weakly positive (WP) or positive …

Respiratory Manifestations of Strongyloidiasis: Significance of Diagnosis, Treatment, and Screening.

Strongyloidiasis is a widespread disease characterized clinically by acute or chronic manifestations, which, in high-risk individuals and immunocompromised individuals, can progress to hyperinfection syndrome and disseminated forms with significant mortality rates. Among extraintestinal organs, the lungs are most commonly affected. Clinical presentations are very diverse, contributing to the challenges of diagnosis and timely treatment initiation. We present the case of a 57-year-old patient who was admitted to the intensive care unit (ICU) due to bilateral pneumonia and septic shock. During a…

Histopathological Features of the Colon in Chronic Strongyloidiasis: A Case Series.

Strongyloidiasis, which is caused by Strongyloides stercoralis (a nematode), is a prevalent human intestinal parasitic disease, particularly in tropical and subtropical regions. Okinawa Prefecture exhibits the highest prevalence of strongyloidiasis in Japan. This nematode has the unique ability to replicate for years within a human host with minimal or no symptoms, a phenomenon known as autoinfection. Patients with Strongyloides stercoralis autoinfection are often asymptomatic or experience uncomplicated infections, and, hence, are considered to have chronic strongyloidiasis. In this study, we…

Strongyloidiasis in humans and dogs in Southern Italy: an observational study.

Strongyloidiasis is a clinical issue both in humans and in dogs. Moreover, there are concerns about its zoonotic potential. We aimed to explore Strongyloides stercoralis epidemiology in Southern Italy in humans and dogs sharing the same environment in three different settings: (1) kennels (group K); (2) livestock farms (group L) and (3) agricultural farms (group A). For humans, a commercial ELISA test was used for screening. RT-PCR on faecal samples was done for people testing positive or equivocal at serology. On dog's faecal samples, Baermann test and RT-PCR were performed. A total of 145 do…

Hospitalizations Associated With Strongyloidiasis in the United States, 2003-2018.

BACKGROUND: Strongyloides stercoralis is considered to be historically endemic in Appalachia and the American South, but recent surveillance data, especially data evaluating strongyloidiasis associated with hospitalization, are lacking in most parts of the United States. METHODS: We performed a population-based retrospective analysis on strongyloidiasis using the National Inpatient Sample from 2003 to 2018. Geographic distribution of strongyloidiasis associated hospitalization was assessed. Logistic regression was used to identify risk factors associated with strongyloidiasis. RESULTS: We id…

Current pharmacotherapeutic strategies for Strongyloidiasis and the complications in its treatment.

INTRODUCTION: Strongyloidiasis, an infection caused by the soil-transmitted helminth Strongyloides stercoralis, can lead immunocompromised people to a life-threatening syndrome. We highlight here current and emerging pharmacotherapeutic strategies for strongyloidiasis and discuss treatment protocols according to patient cohort. We searched PubMed and Embase for papers published on this topic between 1990 and May 2022. AREAS COVERED: Ivermectin is the first-line drug, with an estimated efficacy of about 86% and excellent tolerability. Albendazole has a lower efficacy, with usage advised when i…

Prevalence of strongyloidiasis in Peru: systematic review and meta-analysis.

BACKGROUND: Strongyloidiasis is a disease of great public health significance, caused by the parasitic nematodes Strongyloides stercoralis, Strongyloides fuelleborni, and Strongyloides fuelleborni subsp. kellyi. This systematic review and meta-analysis aimed to assess the prevalence of Strongyloides stercoralis infection in Peru. METHODS: The review was based on a literature search in PubMed, SciELO and Google Scholar using the key words or root words "strongyl*" AND "Peru" on 15 July 2020. Eligible studies were published from 1 January 1981 to 15 July 2020 and written in English, Spanish, It…

Helminth infections in the US military: from strongyloidiasis to schistosomiasis.

BACKGROUND: Helminth infections caused by parasitic worms, including nematodes (roundworms), cestodes (tapeworms) and trematodes (flukes), c … Nearly 4000 of these cases represented infections other than enterobiasis. There were 351 validated strongyloidiasis diagnoses, 317 schistosomiasis diagnoses and 191 diagnoses of cysticercosis during the study period. Incidence of intestinal nematode infection diagnoses showed an upward trend, whilst the incidence of cestode infection diagnoses decreased. CONCLUS …

High Prevalence of Strongyloidiasis in Spain: A Hospital-Based Study.

Introduction: Strongyloidiasis is a prevailing helminth infection ubiquitous in tropical and subtropical areas, however, seroprevalence data are scarce in migrant populations, particularly for those coming for Asia. Methods: This study aims at evaluating the prevalence of S. stercoralis at the hospital level in migrant populations or long term travellers being attended in out-patient and in-patient units as part of a systematic screening implemented in six Spanish hospitals. A cross-sectional study was conducted and systematic screening for S. stercoralis infection using serological tests was …

Strongyloidiasis: A Diagnosis to Consider in Previously Endemic Regions in Portugal.

Strongyloides stercoralis is a parasite capable of surviving for decades in a single host, due to its ability of auto-infection. Strongyloidiasis usually presents a chronic and asymptomatic evolution, and isolated eosinophilia, cutaneous or gastrointestinal abnormalities are the most frequent findings. Between the 1910s and 1980s, several cases of autochthonous strongyloidiasis in Portugal have been described. We report the case of a patient with abdominal pain, ascites and marked eosinophilia. The diagnostic investigation excluded frequent causes of eosinophilia. The diagnosis of strongyloidi…

A Case of Ulcerated Gastric Mass and Anemia due to Strongyloidiasis.

Strongyloidiasis is an intestinal infection caused by the nematode Strongyloides, and it is rarely seen in our clinical practice in the United States. Although it remains localized to the gastrointestinal tract most of the time, it can disseminate to other organs when it causes autoinfection in the setting of immunosuppression. The clinical manifestations vary depending on the involved organ. A case of disseminated strongyloidiasis presenting as a case of ulcerated gastric mass and anemia is described here along with a review of the literature on this condition.
