INTRODUCTION: Syncope is an abrupt loss of consciousness in response to reduced perfusion to the brain. Neurocardiogenic or vasovagal syncope results from a complex neurologic reflex, and treatments to prevent recurrence attempt to modulate aspects of that reflex.

Syncope is a frequently observed symptom in pediatrics with teenagers being the age group most often affected. In contrast to older age, organic cardiac causes of syncope in child-hood are observed in a minority of only 2-5% of cases, in their majority pediatric syncopes thus are neurocardiogenic in origin. We present the case of a 28-year-old woman admitted to our department with respiratory distress, profound hypotension, and tenderness in the left upper abdominal quadrant and left calf. Ultrasound examination revealed left lower extremity thrombus as well as a floating thrombus in the right atrium attached to a patent foramen ovale. Spiral computed tomography revealed bilateral pulmonary artery embolization, while an abdominal computed tomographic scan disclosed spleen infarction due to a paradoxical embolism through the patent foramen ovale.

We report on 4 children who experienced a syncopal episode while being treated with guanfacine without any other evident cause. Syncope appears to be an uncommon side effect of guanfacine and is probably due to drug-induced hypotension or bradycardia.

Neurocardiogenic and arrhythmic syncope are very common and may be found in many patients in Emergency Departments or admitted to hospitals. BACKGROUND: Syncope is a common disorder that is potentially disabling and affects both young and old. Once neurological, cardiological, and metabolic causes have been excluded, there remains a group in which diagnosis is unclear; some may have an autonomic basis. We therefore did a retrospective study on consecutive patients referred to our tertiary referral autonomic centres between 1992 and 1998 with recurrent syncope and presyncope, in whom non-autonomic causes, before referral, had been sought and excluded. This study examined the potential clinical contribution of noninvasive brain and skeletal muscle oximetry as a diagnostic aid in neurally mediated syncope. Tilt table testing was performed in 15 patients with a history of syncope and 9 healthy volunteers. Spatially resolved reflectance near-infrared spectroscopy was used to examine regional oxyhemoglobin saturation in the frontal cerebral cortex and pectoral muscle.
