Visual marker-based augmented reality for intraoperative ultrasound in minimally invasive surgery.

BACKGROUND: Intraoperative ultrasonography (IOUS) is essential in minimally invasive hepatobiliary-pancreatic surgery but requires surgeons to alternate attention between laparoscopic and ultrasound monitors, disrupting workflow. We developed Flag AR, an augmented reality system that superimposes real-time ultrasound images onto laparoscopic displays using visual marker tracking. METHODS: This prospective observational study included 14 patients undergoing minimally invasive hepatobiliary-pancreatic procedures between March and April 2025. Three experienced surgeons performed target identific

Which delivery model innovations can support sustainable HIV treatment?

The rapid scale-up of antiretroviral treatment (ART) for HIV since the mid-2000s, mostly through disease-specific or "vertical" programmes, has been a highly successful undertaking, which averted millions of deaths and prevented many new infections. However, the dynamics of the HIV epidemic and changing political and financial commitment to fight the disease will likely require new models for the delivery of ART over the coming decades if the promises of universal treatment are to be met. Delivery model innovations for ART are intended to improve both the effectiveness and efficiency of the HI

Neuropathic Pain and Functional Reorganization in the Primary Sensorimotor Cortex After Spinal Cord Injury.

UNLABELLED: Refractory to most types of treatment, neuropathic pain (NP) is a major problem for people living with spinal cord injury (SCI). The underlying mechanisms among problems related to treatment are poorly understood. The aim of the present study was to investigate the association between cortical reorganization and NP after SCI. Twenty-four individuals with sensorimotor complete and incomplete paraplegia and tetraplegia (12 with NP, 13 pain free) and 31 healthy individuals were examined. Functional magnetic resonance imaging was used to assess activation in primary somatosensory and m

Ipsilateral-Dominant Control of Limb Movements in Rodent Posterior Parietal Cortex.

It is well known that the posterior parietal cortex (PPC) and frontal motor cortices in primates preferentially control voluntary movements of contralateral limbs. The PPC of rats has been defined based on patterns of thalamic and cortical connectivity. The anatomical characteristics of this area suggest that it may be homologous to the PPC of primates. However, its functional roles in voluntary forelimb movements have not been well understood, particularly in the lateralization of motor limb representation; that is, the limb-specific activity representations for right and left forelimb moveme

Electrophysiological signatures of visual temporal processing deficits in developmental dyslexia.

Developmental dyslexia (DD) is a common neurodevelopmental disorder that affects reading ability despite normal intelligence and education. In search of core deficits, previous evidence has linked DD with impairments in temporal aspects of perceptual processing, which might underlie phonological deficits as well as inefficient graphemic parsing during reading. However, electrophysiological evidence for atypical temporal processing in DD is still scarce in the visual modality. Here, we investigated the efficiency of both temporal segregation and integration of visual information by means of eve

Do older adults make more risky decisions in the Hungry Donkey Task or in the Iowa Gambling Task?

The Iowa Gambling Task (IGT) and the Hungry Donkey Task (HDT) are well-known tasks employed to assess decisions under ambiguity. Although the two tasks are equal in terms of wins and losses, they differ in terms of the recipient: while in the IGT participants make decisions for themselves, in the HDT decisions are made to help a hungry donkey. Decisions for themselves versus another one in a situation of ambiguity are particularly important in the field of aging because of older adults' changes in motivational and other-oriented behavior. The present study aimed to test whether older adults ma

Choline Plus Working Memory Training Improves Prenatal Alcohol-Induced Deficits in Cognitive Flexibility and Functional Connectivity in Adulthood in Rats.

Fetal alcohol spectrum disorder (FASD) is the leading known cause of intellectual disability, and may manifest as deficits in cognitive function, including working memory. Working memory capacity and accuracy increases during adolescence when neurons in the prefrontal cortex undergo refinement. Rats exposed to low doses of ethanol prenatally show deficits in working memory during adolescence, and in cognitive flexibility in young adulthood. The cholinergic system plays a crucial role in learning and memory processes. Here we report that the combination of choline and training on a working memo

Uncovering the spatio-temporal dynamics of value-based decision-making in the human brain: a combined fMRI-EEG study.

While there is a growing body of functional magnetic resonance imaging (fMRI) evidence implicating a corpus of brain regions in value-based decision-making in humans, the limited temporal resolution of fMRI cannot address the relative temporal precedence of different brain regions in decision-making. To address this question, we adopted a computational model-based approach to electroencephalography (EEG) data acquired during a simple binary choice task. fMRI data were also acquired from the same participants for source localization. Post-decision value signals emerged 200 ms post-stimulus in a

Interprofessional Care in Obstetrics and Gynecology.

Interprofessional care relates to providing care to an individual in an integrated system of professionals who share and shift the care given, depending on the individual and population need. By broadening the scope of care given by health-care workers (HCWs) or shifting tasks through interprofessional care in Obstetrics and Gynecology, more women and their newborns can benefit from safe deliveries, decreased perinatal morbidity and mortality and through screening and early treatment prevent morbidity and mortality secondary to gynecologic diseases.

Fatigue-induced alterations in the spatial distribution of lumbar erector spinae activity in older versus young adults.

PURPOSE: This study examined differences in the spatial distribution of lumbar erector spinae (LES) muscle activity during a submaximal isometric trunk extension contraction between older and younger adults. METHODS: Thirteen older adults (OLDER) and thirteen young adults (YOUNG) participated. High-density surface electromyography signals were recorded from the LES muscle during an isometric trunk extension task at 30% of maximal voluntary isometric force until failure. The spatial distribution of muscle activity was assessed via the x and y coordinates of the centroid of the root mean square

Line-bisectioning and obstacle avoidance: evidence for separate strategies.

Previous studies have frequently applied a combination of line-bisection tasks (in which participants indicate the middle of a line) and obstacle avoidance tasks (in which participants move their hand between two obstacles) with the aim of revealing perception-action dissociations in certain neurological disorders, such as visual form agnosia and optic ataxia. However, valid conclusions about the underlying processing pathways can only be drawn if participants apply the same strategy in both tasks (i.e. finding the middle between the obstacles). Yet, this assumption has never been tested direc

Divergent effects of high-frequency rTMS on cognitive performance in sleep-deprived nurses: An EEG brain network study.

BACKGROUND: Sleep deprivation (SD) is a common occupational hazard, particularly for shift workers like nurses, leading to significant impairments in cognitive functions such as sustained attention and working memory. High-frequency repetitive transcranial magnetic stimulation (rTMS) is a promising neuromodulation technique for cognitive enhancement, but its effects in sleep-deprived individuals and the underlying neural mechanisms remain poorly understood. This study aimed to investigate the efficacy of high-frequency rTMS over the left dorsolateral prefrontal cortex (DLPFC) in modulating sus

Anxiety Evokes Hypofrontality and Disrupts Rule-Relevant Encoding by Dorsomedial Prefrontal Cortex Neurons.

UNLABELLED: Anxiety is a debilitating symptom of most psychiatric disorders, including major depression, post-traumatic stress disorder, schizophrenia, and addiction. A detrimental aspect of anxiety is disruption of prefrontal cortex (PFC)-mediated executive functions, such as flexible decision making. Here we sought to understand how anxiety modulates PFC neuronal encoding of flexible shifting between behavioral strategies. We used a clinically substantiated anxiogenic treatment to induce sustained anxiety in rats and recorded from dorsomedial PFC (dmPFC) and orbitofrontal cortex (OFC) neuron

The Calm after the Storm: Implications of Sepsis Immunoparalysis on Host Immunity.

The immunological hallmarks of sepsis include the inflammation-mediated cytokine storm, apoptosis-driven lymphopenia, and prolonged immunoparalysis. Although early clinical efforts were focused on increasing the survival of patients through the first phase, studies are now shifting attention to the long-term effects of sepsis on immune fitness in survivors. In particular, the most pertinent task is deciphering how the immune system becomes suppressed, leading to increased incidence of secondary infections. In this review, we introduce the contribution of numerical changes and functional reprog

Potential Resilience to Post-traumatic Stress Disorder and Common Mental Disorders among Lay Health Workers Working on the Friendship Bench Programme in Zimbabwe.

PURPOSE: The Friendship Bench is a successful task-shifting intervention for addressing common mental disorders in Zimbabwe. The intervention takes a cognitive behavioural therapy and problem-solving approach provided by lay health workers (LHWs). The LHWs live in the same environment as their clients and are thus exposed to the same traumas and stressors as are their clients. Little is known about the long-term psychological effects on LHWs involved in this type of work. METHODS: A random sample of LHWs (n=182) was assessed using the Posttraumatic Stress Disorder Checklist (PCL-5) and the Sh

Unintended Consequences of Community Health Worker Programs in South Africa.

Task shifting from trained clinicians to community health workers (CHWs) is a central, primary health care strategy advocated by global health policy planners in resource-poor settings where trained health professionals are scarce. The evidence base for the efficacy of these programs, however, is limited-in particular, research that identifies their potential unintended consequences. Based on sustained ethnographic study of CHWs working for AIDS projects in South Africa at the height of the country's AIDS epidemic, this article identifies how structural and local factors produced unintended co…

Global maternal health and newborn health: Looking backwards to learn from history.

The late appearance of the 'M' on the international health agenda - in its own right and not just as a carrier of the intrauterine passenger - is thought-provoking. The 'M' was absent for decades in textbooks of 'tropical medicine' until the rhetoric question was formulated: 'Where is the "M" in MCH?' The selective antenatal 'high-risk approach' gained momentum but had to give way to the fact that all pregnant women are at risk due to unforeseeable complications. In order to provide trained staff to master such complications in impoverished rural areas (with no doctors), some countries have em

The promise of computer-assisted auscultation in screening for structural heart disease and clinical teaching.

Cardiac auscultation has been the central clinical tool for the diagnosis of valvular and other structural heart diseases for over a century. Physicians acquire competence in this technique through considerable training and experience. In Africa, however, we face a shortage of physicians and have the lowest health personnel-to-population ratio in the world. One of the proposed solutions for tackling this crisis is the adoption of health technologies and product innovations to support different cadres of health workers as part of task shifting. Computer-assisted auscultation (CAA) uses a digita…

Learning and cognitive flexibility: frontostriatal function and monoaminergic modulation.

Learning in a constant environment, and adapting flexibly to a changing one, through changes in reinforcement contingencies or valence-free cues, depends on overlapping circuitry that interconnects the prefrontal cortex (PFC) with the striatum and is subject to several forms of neurochemical modulation. We present evidence from recent studies in animals employing electrophysiological, pharmacological and lesion techniques, and neuroimaging, neuropsychological and pharmacological investigations of healthy humans and clinical patients. Dopamine (DA) neurotransmission in the medial striatum and P
