Treatment withdrawal and distress: Recognising the need for better support in critical care-A scoping review.

BACKGROUND: Intensive care units house critically acute patients requiring extensive treatments and specialised care. Highly trained healthcare providers work tirelessly to perform life-sustaining measures, but when all possible treatment options have been exhausted, it sometimes becomes necessary to withdraw treatment. This process places critical care nurses and the interprofessional teams at the centre of emotionally and ethically challenging end-of-life care. OBJECTIVES: The objective of this review was to synthesise what is known about the types of moral, emotional, and psychological dis

Parental hope in paediatric palliative care: a systematic review of the ethical issues in evidence-based literature.

BACKGROUND: This systematic review aims to raise awareness of the concept of parental hope, which is distinct from the idea of agency – acting on behalf of someone else. Parental hope is an important factor in end-of-life care, particularly in light of recent advances in perinatology, neonatology, and paediatric palliative care (PPC). Parenting involves autonomous agency – acting and thinking on one's own behalf – as well as making decisions for one's children. Consequently, parental permission, authority and decision-making are integral aspects of parental attitudes towards their children in

["Accompanying the dying process" - A guide to dealing with the dying and their relatives].

Recognizing the dying phase is often a major challenge for professional carers. The associated adaption of medical and nursing measures and knowledge of options for drug and non-drug symptom control, including off-lable-use (OLU), are the basis of care for the dying. Treating the dying also always means caring for the informal carers as a unit of care. With the help of a multi-professional team, it is possible to meet the needs of patients and their family caregivers in all dimensions of a human being. Meeting the existential needs of the dying and their relatives requires a benevolent attitud

Promoting the use of advance care planning in people from minority ethnic backgrounds.

Advance care planning supports decision-making autonomy for patients at the end of life. However, research has shown that the uptake of advance care planning is often low among people from minority ethnic backgrounds. This article explores the systemic and intracultural barriers experienced by people from minority ethnic backgrounds when planning care at the end of life. The author also offers a conceptual framework to support greater equity and access to advance care planning in these populations.

Spirituality in Serious Illness Education: A Narrative Review of Medical School Curricula From 2000 to 2024.

INTRODUCTION: Spirituality is increasingly recognized as an integral component of patient-centered care in serious illness. Despite substantial evidence supporting its benefits, the integration of spiritual care into medical education remains inconsistent. This narrative review examines how spiritual care is incorporated into serious illness curricula in medical schools and evaluates its impact on student learning. METHODS: Articles published between 2000 and 2024 describing medical school curricula addressing spiritual care in palliative care, serious illness, hospice, or end-of-life care we

Developing palliative and end-of-life care research partnerships in the North East and North Cumbria: A report on the first year of the RIPEN network.

BACKGROUND: The North East is one of the most disadvantaged areas of England with end-of-life care needs shaped by high levels of disability, physical and mental ill health. This programme of work aimed to set the foundation for the development of a palliative and end-of-life care research community, equipped to meet local and national challenges. OBJECTIVES: Develop a self-sustaining research culture in North East palliative and end-of-life care. Build research collaborations around older age, frailty, multiple long-term conditions and inequalities and submit at least one research proposal.

Community-based neuropalliative care.

Community-based palliative care is defined as palliative care delivered outside of the hospital and outpatient clinics. These settings include the home, nursing homes, day programs, volunteer organizations, and support groups. There is strong evidence outside of the neuropalliative context that community-based palliative care can reduce hospital costs and admissions at the end of life. Research that focuses on specialized community-based palliative care for neurologic disease have similar findings, although with significant variability across conditions and geographic locations. Several of the

Family experience of palliative sedation therapy: proportional vs. continuous deep sedation.

PURPOSE: Some patients experience intense symptoms refractory to intensive palliative care, and palliative sedation is sometimes used. Palliative sedation may be classified into proportional and continuous deep sedation (CDS). The primary aim of this study was to compare family experience between families of patients who received proportional or CDS. METHODS: A multicenter questionnaire survey was conducted involving bereaved families of cancer patients who received proportional or CDS based on a sedation protocol. Overall evaluation of sedation (satisfaction, family-perceived distress, appro

Multicenter Evaluation of 434 Hospital Deaths From COVID-19: How Can We Improve End-of-Life Care During a Pandemic?

CONTEXT: The pandemic has substantially increased the workload of hospital palliative care providers, requiring them to be responsive and innovative despite limited information on the specific end of life care needs of patients with COVID-19. Multi-site data detailing clinical characteristics of patient deaths from large populations, managed by specialist and generalist palliative care providers are lacking. OBJECTIVES: To conduct a large multicenter study examining characteristics of COVID-19 hospital deaths and implications for care. METHODS: A multi-center retrospective evaluation examine

Clinical practice guideline for end-of-life care in patients with cancer: a modified ADAPTE process.

PURPOSE: This study seeks to adapt a guideline for end-of-life care in patients with cancer to be used by healthcare teams. METHODS: This methodological study was conducted by modifying the ADAPTE process and adding to it a qualitative study and consensus ratings by a multidisciplinary panel of experts. A qualitative study was thus performed to identify the end-of-life needs of patients with cancer. Then, the source guidelines and the results of the qualitative study were used to draft the initial version of the guideline, with 85 adaptation recommendations. A multidisciplinary panel of healt

How good is end of life care for patients dying with cystic fibrosis?

BACKGROUND: Cystic Fibrosis (CF) is a life-limiting illness. Audit of the care of patients dying of CF has not been published to date. METHODS: Newcastle and Oxford teams adapted the National Audit of Care at the End of Life and agreed additional questions that were particularly pertinent for patients dying as a consequence of their CF. Data were extracted and analysed for 15 patients. RESULTS: On recognition that the patient was dying, the CF teams were less good at reviewing the need for physiological observations (50% vs national 70%) but better at reviewing the need for capillary blood g

Sedatives and sedation at the end of life: a nursing home retrospective cohort study.

OBJECTIVES: Sedatives are frequently used at the end of life in specialist palliative care. There is scarce information about their use in nursing homes. Therefore, we aimed to assess the use of (1) sedatives generally and (2) 'sedatives with continuous effect', based on objective operational criteria, within the last week of life in a nursing home. METHODS: This was a retrospective cohort study of residents who died in a German nursing home between 1/2015 and 12/2017, using the nursing home's medical records, which contained drug sheets and nurses' notes. Sedatives analysed were those recomm

Palliative care for patients with end-stage liver disease.

Liver disease results in over four million physician visits and over 750,000 hospitalizations per year in the USA. Those with chronic liver disease frequently progress to cirrhosis, end-stage liver disease (ESLD), and death. Patients with ESLD experience numerous complications, including muscle cramps, confusion (hepatic encephalopathy), protein calorie malnutrition, muscle wasting, fluid overload (ascites, edema), bleeding (esophagogastric variceal hemorrhage), infection (spontaneous bacterial peritonitis), fatigue, anxiety, and depression. Despite significant improvements in palliation of th

[Palliative care in otolaryngology].

The aim of palliative care is to improve the quality of life (QOL) of patients with a limited life expectancy in a comprehensive multidisciplinary approach. It encompasses consideration of medical, physical, psychosocial and spiritual problems, including treatment and help from a team of various professionals. Palliative care often extents to the patient's families and may continue after the death of the patient. Half of all head and neck cancer patients will die of their malignancy within 5 years of diagnosis. The primary medical issues affecting QOL at the end of life are communication probl

Palliative and end-of-life care in the global response to multidrug-resistant tuberculosis.

Multidrug-resistant (MDR) tuberculosis is costly, difficult to treat, and poses a global threat to tuberculosis control. The high burden of disease and treatment for patients, poor cure rates, and high mortality bring distress to patients, families, and caregivers. Despite guidance to improve treatment outcomes, little attention has been paid to palliative care of patients and families, such as for physical, psychosocial, social, and spiritual difficulties. An international expert symposium was convened to articulate an appropriate palliative care response for people with MDR tuberculosis. Sev
