Sixty-one cases of acute epiglottitis at the Royal Alexandra Hospital for Children, Sydney, from July 1968 to December 1974 are reviewed. The average age of 2.7 years is lower than previously reported. The average time from the initial symptom to arrival at the hospital was 14 hours. The average time from arrival to performance of tracheotomy was two hours. The diagnosis was made when first seen and assessed in 50 of the 61 cases. Mechanical insufflation exsufflation-expiratory flows (MIE-EFs) correlate with upper airway patency. A patient aged 67 years, with severe dyspnoea caused by supraglottitis, necessitating intubation and tracheotomy was admitted to hospital. Although the respiratory condition initially improved, dyspnoea recurred. This could be explained by noncardiogenic pulmonary oedema, a complication of upper airway obstruction. The clinical symptoms, pathophysiology and therapy are discussed.

Gastroesophageal reflux (GER) plays a causative role in the development of subglottic stenosis (SGS) in children. This study examined the impact of aggressive antireflux therapy on the clinical outcomes of 35 children. We report a recently encountered case of vascular leiomyoma in the larynx. The patient was a 76-year-old man with a chief complaint of hoarseness. Laryngoscopy revealed a bean-sized, reddish tumor with a smooth surface, suspended from the right vocal cord toward the subglottis. Its border was clear on MR imaging. Laryngotracheal stenosis should be suspected in children with recurrent, prolonged, or atypical croup; a history of endotracheal intubation; or a history of stridor, feeding difficulties, and failure to thrive. Tracheotomy-dependent patients with acquired laryngotracheal stenosis are candidates for surgical intervention to provide the child with the earliest opportunity to develop normal oral communication.
