We have reviewed the incidence, demography, and outcome of children who have received a repeat transplant in the North American Pediatric Renal Transplant Cooperative Study registry. From 1987 through 1994, 3290 primary and 573 first repeat transplants were performed on North American children. Living donor grafts were used in 52% of primary and 23% of first repeat transplants. Of primary transplants, 23.7% were among children under the age of 6 years. For repeat transplants, the percentage was 15% for the same category. Gammopathies were found to be present in 25 (13%) of 192 HIV-negative renal transplant recipients with more than 30 months follow-up prospectively investigated for monoclonal or oligoclonal immunoglobulins (mIg) by agarose gel electrophoresis and immunofixation. One goal of transplantation experiments involving damaged spinal cords is to reconstruct a functional innervation to muscles in the periphery. Embryonic spinal cord grafts have been shown to survive transplantation into adult spinal cord lacking motoneurones. Motoneurones from the graft appear to be able to innervate muscle tissue by being encouraged to grow across a bridge of peripheral nerve. As a result of introduction of quadruple immunosuppression using cyclosporine, antithymocyte globulin, azathioprine and prednisone in immunological high-risk patients with a presensitization greater than or equal to 80% and/or multiple grafts the rate of immediate graft function could significantly increased, the frequency of rejection and graft rupture was reduced and the patient survival rate could improved to 100%. The problem of vascular rejection should resolved by a more aggressive biopsy approach.

Acute deterioration of renal function occurred shortly after the angiotensin-converting enzyme (ACE) inhibitor, enalapril, was administered to two renal transplant patients who were also receiving cyclosporine. Renal function recovered completely in both cases upon discontinuation of enalapril. Esophageal replacement using transverse colon with left colic pedicle is reviewed. 18 were done for caustic stenosis with one death, 4 for atresia without mortality. For caustic stenosis the authors advocate early transplant since the third month if dilatations failed. Improvement of technique using staplers prevented any complication in the last 8 patients. These good results allowed the authors to switch to isoperistaltic gastric tube. In esophageal atresias, the authors did not find any indication of replacement in the 12 last years.

This report compares endocrine and exocrine responses of "free-duct" and "duct-ligated" whole pancreas isografts in Lewis diabetic rats during a 1-year period of observation. Fasting blood sugar, serum amylase, serum insulin, pancreozymin-secretin test, and L-arginine tolerance test assays were performed prior to and at periodic intervals after transplantation. Two cases of relapse of pneumonia caused by cytomegalovirus (CMV) in recipients of renal transplants were studied. Between episodes of pneumonia, both patients recovered completely as judged by X-ray clearing and by resolution of fever and respiratory symptoms. CMV was isolated during each episode of pneumonia from lung tissue, pleural fluid, or buffy coat cultures. Serologic studies revealed a rise in titer of antibody, confirming recent infection with CMV during the first epidose of pneumonia in each patient. Disseminated cryptococcal infection is described in eight patients, seven of them with verified meningeal involvement. Six of the eight patients were recipients of a renal homograft and submitted to the classical immunosuppressive treatment. Consideration is given to predisposing factors and to problems in the clinical, biological and mycological diagnosis. Some comments are presented on the often disappointing results of antifungal therapy of cryptococcal meningitis.

Eighteen children aged 6 to 17 years received 24 cadaveric renal transplants between January 1965 and July 1971, and a further child received a kidney donated by her father. 12 children are alive with good functioning grafts and another 2 children are alive on haemodialysis awaiting a further renal graft. The clinical problems of renal transplantation in children are discussed with particular reference to the side effects of immunosuppressive therapy.
