Bleeding control in catastrophic blunt pelvic trauma using the abdominal aortic and junctional tourniquet in a civilian level I trauma center: A case report.

BACKGROUND: In non-compressible blunt pelvic trauma with hemodynamic instability there are few options to ensure bleeding control in the prehospital setting or the emergency bay. These include insertion of a balloon into the descending aorta to achieve occlusion and loss of blood flow distal to the device known as resuscitative endovascular balloon occlusion of the aorta (REBOA). An alternative is the use of an external abdominal aortic and junctional tourniquet (AAJT) which is inflated to produce compression of the aorta. CASE PRESENTATION: We present the case of a 27-year-old patient, who w

Does treatment at a level I trauma center reduce disparities in patient outcomes for open tibia fractures? A retrospective analysis of the National trauma Databank.

BACKGROUND: Race and insurance status are independent predictors of healthcare outcomes following lower-extremity trauma. Level 1 trauma centers show better outcomes overall, but it is has not been extensively studied as to whether they specifically lower complication rates and shorten length of stay in those with Black race, with low socioeconomic status, and/or a lack of private health insurance. We performed a study with the objective of determining whether Level I trauma centers can improve the complication rate of those shown to be at high risk of experiencing adverse outcomes due to soci…

Pancreatic fistula and mortality after surgical management of pancreatic trauma: analysis of 81 consecutive patients during 11 years at a Korean trauma center.

PURPOSE: Pancreatic trauma is infrequent because of its central, deep anatomical position. This contributes to a lack of surgeon experience and many debates exist about its standard care. This study aimed to investigate the postoperative pancreatic fistula (POPF) and mortality of pancreatic trauma after operation. METHODS: We reviewed records in the trauma registry of our institution submitted from January 2006 to December 2016. The grade of pancreatic injury, surgical management, morbidity, mortality, and other clinical variables included in the analyses. RESULTS: Data from a total of 26,07

Effect of a new urgent care center on the amount and characteristics of upper limb injuries treated in a level Ⅰ trauma center in southern Israel.

PURPOSE: Reducing nonemergent use of the emergency department (ED) is an important public health policy goal. Urgent care centers (UCCs) have recently increased in number and their impact on the characteristics and number of cases presenting to the ED is not yet fully understood. In this retrospective study, we attempt to characterize changes in upper limb trauma admissions to the ED in southern Israel after the opening of a nearby UCC. METHODS: Data were collected from the medical records at ED, Soroka University Medical Center. Patients with related upper limb traumas during the years 2011-

Application of a dynamic wound closure device in orthopaedic trauma patients following fasciotomy: A level-1 trauma center perspective and preliminary case series.

The challenge of achieving delayed primary closure of fasciotomy wounds is well known, and multiple techniques of achieving either delayed primary closure or alternative forms of soft-tissue reconstruction have been described in the literature. However, a universally accepted consensus of the optimal management strategy has yet to be established. We describe our experience with a device designed to use the mechanical concept of dermatotraction-a surgical technique that uses the skin's natural elasticity to close wounds. This device (ABRA Surgical, Southmedic, Inc) promotes a delayed primary wo

Antimicrobial susceptibilities of respiratory pathogens in the surgical/trauma intensive care unit compared with the hospital-wide respiratory antibiogram in a level I trauma center.

BACKGROUND: Unit-specific antibiograms have developed to guide clinicians to appropriate antibiotic choices for subsets of patients. The additional level of a unit- and respiratory-specific antibiogram for surgical and trauma patients defines key differences in susceptibility information for treating hospital-acquired pneumonia. METHODS: This was a three-year, retrospective single-center study. A total of 729 positive respiratory specimens from 612 patients were analyzed using Quality Compass Pathfinder(®), the antibiotic-reporting software. Culture and susceptibility reports were compared fo

High Mortality and Medical Costs in Geriatric Trauma Patients: Surgical Treatment and Risk Factors from a Retrospective Cohort Study at a Level I Trauma Center.

BACKGROUND: Trauma remains a leading cause of death and disability. With the rapidly growing aging population, geriatric patients face heightened trauma risks due to physiological decline and comorbidities. Despite the growing burden, data on the clinical characteristics, injury mechanisms, patterns, and healthcare utilization in this group remain limited. This study analyzed trauma outcomes and associated factors in geriatric patients. METHODS: We conducted a retrospective cohort study using data from a Level I trauma center. Patients aged ≥65 years were compared with younger adults (<65

New Trauma Score versus Kampala Trauma Score II in predicting mortality following road traffic crash: a prospective multi-center cohort study.

INTRODUCTION: Mortality due to injuries disproportionately impact low income countries. Knowledge of who is at risk of poor outcomes is critical to guide resource allocation and prioritization of severely injured. Kampala Trauma Score (KTS), developed in 1996 and last modified in 2002 as KTS II, is still widely being used to predict injury outcomes in resource-limited settings with no further revisions in the past two decades, despite ongoing criticism of some of its parameters. The New Trauma Score (NTS), a recent development in 2017, has shown potential in mortality prediction, but a dearth

A new pediatric trauma center quality benchmarking metric: Center-level variability in postinjury functional impairment using the Functional Status Scale (FSS).

BACKGROUND: Over 7 million children are injured annually in the United States, leading to 500,000 disability-adjusted life years. Functional outcomes after injury, however, are not routinely measured. Whether disability is modifiable by trauma center care remains unclear. The purpose of this study was to assess variability between trauma centers in functional impairment rates. METHODS: The "Assessment of Health-Related Quality of Life and Functional Outcomes after Pediatric Trauma" prospective observational dataset was analyzed. Functional status among injured children (≤14 y) was assessed at…

Association between delayed thromboprophylaxis and the incidence of deep vein thrombosis in patients with pelvic trauma: a retrospective cohort adjusted for severity at a level I trauma center.

BACKGROUND: Patients with pelvic trauma are at high risk for deep vein thrombosis (DVT), but early initiation of thromboprophylaxis may increase bleeding risk in hemodynamically unstable individuals. The optimal timing for initiation of anticoagulation remains controversial. OBJECTIVES: To evaluate whether delayed initiation of pharmacologic prophylaxis is associated with a higher incidence of DVT in patients with pelvic trauma, after adjustment for age and activation of the massive transfusion protocol. METHODS: This retrospective observational cohort study was conducted at a Level I trauma

Trauma center performance and outcome disparities in severe childhood traumatic brain injury: A Trauma Quality Improvement Program study including a causal mediation analysis.

BACKGROUND: That Black children die at higher rates from traumatic injuries has been recognized for years, but race as a social construct cannot itself be a cause of death. The effect of race must be mediated. METHODS: This observational, cross-sectional study was based on data from the Trauma Quality Improvement Program of the American College of Surgeons for the years 2014 through 2022. Severe traumatic brain injury was defined as an Abbreviated Injury Scale head score of 4 or greater. Exclusion criteria were age older than 18 years, transfer to another acute care facility, and discharge fr

Impact of blood alcohol concentration on hematologic and serum chemistry parameters in trauma patients: Analysis of data from a high-volume level 1 trauma center.

BACKGROUND: Alcohol (EtOH) intoxication is common among trauma patients. While providers are familiar with the clinical aspects of acute EtOH intoxication, few studies have investigated the effects that EtOH levels may have on common laboratory markers. The aim of this study was to identify hematologic and serum chemistry parameters that may be affected by the blood alcohol concentration (BAC), hypothesizing that BAC influences both comprehensive blood count (CBC) and comprehensive serum chemistry (CSC) components. METHODS: We performed an IRB-exempt institutional registry review of all traum

A Selection of Trauma Scores Might Not Correlate with Coagulation Factor Activity following Multiple Injuries: A Retrospective Observational Study from a Level 1 Trauma Center.

Loss and dilution of coagulation factors have been observed following multiple trauma. Timely recognition of reduced clotting factor activity might facilitate therapeutic action to restore normal coagulation function. This study investigates the potential role of some well-known trauma scores in predicting coagulation factor activity after multiple injuries. A dataset comprising the coagulation factor activities of 68 multiply injured adult patients was analyzed. The following trauma scores were evaluated: AIS, ISS, NISS, GCS, RTS, TRISS, RISC, and TASH score. To investigate the effect of trau

Cumulative effective radiation dose received by blunt trauma patients arriving to a military level I trauma center from point of injury and interhospital transfers.

BACKGROUND: Trauma patients sustaining blunt injuries are exposed to multiple radiologic studies. Evidence indicates that the risk of cancer from exposure to ionizing radiation rises in direct proportion to the cumulative effective dose (CED) received. The purpose of this study is to quantify the amount of ionizing radiation accumulated when arriving directly from point of injury to San Antonio Military Medical Center (SAMMC), a level I trauma center, compared with those transferred from other facilities. METHODS: A retrospective record review was conducted from 1st January 2010 through 31st
