Use of a bivalve finger fracture orthosis for a new treatment protocol of a PIP comminuted fracture and dorsal dislocation.

As therapists and physicians, we often need to work with our patients to cater treatment accordingly. These authors describe how they modified a treatment protocol for a patient that sustained a proximal interphalangeal joint comminuted fracture and dorsal dislocation, but refused surgery. Their modification allowed the patient to return to full activities. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

Premature termination of treatment in an inpatient eating disorder programme.

This retrospective study was conducted to explore rates, timing and predictors of two forms of premature termination of treatment (PTT) in an inpatient eating disorders programme: patient dropout (DO) and administrative discharge (AD). A chart review was conducted to obtain demographic, Eating Disorder Inventory-2 (EDI-2), and Resident Assessment Instrument-Mental Health (RAI-MH) data for 186 patients being treated for bulimia nervosa (BN), anorexia nervosa (AN), or eating disorder not otherwise specified (EDNOS). Overall, of the 37.6% of patients who terminated treatment prematurely, 22.1% of

Substernal goitre: a rare cause of pulmonary hypertension and heart failure.

Benign substernal goitres usually extend into the upper anterior mediastinum and are easily extractable through a cervical approach. Very infrequently these tumours extend into the thoracic cavity causing compression of mediastinal structures. The authors report a case of pulmonary hypertension and severe cardiac failure secondary to a long-standing substernal goitre, and support the surgical management of this disease.

Basic care, pain relief, and advance healthcare directives in Ireland.

The Assisted Decision-Making (Capacity) Act 2015 (ADMCA) provides a legislative basis for advance healthcare directives (AHDs) in Ireland. AHDs offer important benefits to patients, including preserving autonomy and allowing for greater control over end-of-life care. However, they can also create difficult ethical dilemmas, especially in respect of AHDs refusing basic care, pain relief and palliative symptom management. We use a hypothetical case study to interrogate how such directives should be addressed under the ADMCA and identify some uncertainties and ambiguities within the ADMCA concern

Attention to diseases that re-emerge due to refusal of vitamin K: an infant case.

Intramuscular vitamin K injection is recommended for all newborns to prevent bleeding. However, the number of parents who reject vitamin K is at an increase. We present a 1-month girl who presented with haemorrhagic shock due to extraordinary intra-thoracic bleeding. The patient was treated with thoracentesis and blood transfusion. Parents were informed the about the benefits of Vitamin K and they were convinced to continue a routine immunization programme.

Did the call for boycott by the Catholic bishops affect the polio vaccination coverage in Kenya in 2015? A cross-sectional study.

INTRODUCTION: Polio eradication is now feasible after removal of Nigeria from the list of endemic countries and global reduction of cases of wild polio virus in 2015 by more than 80%. However, all countries must remain focused to achieve eradication. In August 2015, the Catholic bishops in Kenya called for boycott of a polio vaccination campaign citing safety concerns with the polio vaccine. We conducted a survey to establish if the coverage was affected by the boycott. METHODS: A cross sectional survey was conducted in all the 32 counties that participated in the campaign. A total of 90,157

Refusal of recommended travel-related vaccines among U.S. international travellers in Global TravEpiNet.

BACKGROUND: International travellers are at risk of travel-related, vaccine-preventable diseases. More data are needed on the proportion of travellers who refuse vaccines during a pre-travel health consultation and their reasons for refusing vaccines. METHODS: We analyzed data on travellers seen for a pre-travel health consultation from July 2012 through June 2014 in the Global TravEpiNet (GTEN) consortium. Providers were required to indicate one of three reasons for a traveller refusing a recommended vaccine: (1) cost concerns, (2) safety concerns or (3) not concerned with the illness. We ca

Commentary: Involuntary Antipsychotics in Prison—Extending Harper, Contracting Care?

This commentary on Salem et al. provides background for their study by reviewing Washington v. Harper and outlining some areas that were not addressed by that decision. It contrasts Harper holdings with those in other U. S. Supreme Court decisions in parallel cases and in United States v. Loughner. It provides cautions about extensions of some holdings in Loughner regarding the use of Harper-type administrative procedures. This article also reviews the methods and findings of Salem et al. and encourages further work. Finally, it raises cautions and voices a call to action concerning potential

Potentially incapable patients objecting to treatment: doctors' powers and duties.

Generally, a patient may only be assessed and treated with his or her consent. It is difficult to interpret the law regarding doctors' powers and duties to assess and treat in cases where a patient refuses to accept medical advice but it is unclear whether the patient has decision-making capacity (DMC) or whether they are mentally ill. An examination of legal principles in this area suggests that there is a limited justification for doctors to detain a person for the purpose of assessment if they have good reason to suspect the person may either lack DMC or be mentally ill and if failure to de

Court-ordered immunisations: a review of the issues.

Following an outbreak of measles in Swansea in 2013, the U.K. courts were asked to order the immunisation of children after an unresolved dispute over the merits of the vaccination between the parents of two children. This article reviews the involvement and approach of the courts in cases of hotly disputed immunisation and argues that going to litigation and obtaining a court order is no guarantee that the vaccination will be given to the child.

Infantile fibrosarcoma of the penis in a 2-year-old boy.

Fibrosarcoma is rare in the pediatric age group. It generally involves the extremities and the trunk but rarely involves the genital area. We report a case of a large fungating infantile fibrosarcoma of the penis in a 2-year-old Malay boy. Partial recovery of the penile structure was achieved after chemotherapy. The difficulty in managing the social and surgical aspect of this case is discussed in our report. To the best of our knowledge, this is the first case report of infantile fibrosarcoma involving the penis in an Asian region.

Exploring symptoms in patients managed without dialysis: a qualitative research study.

Little is known about the prevalence and burden of symptoms in patients managed without dialysis. This study was the result of a larger study exploring the experiences of 30 such patients and their trajectories to death. Data were analysed relating to symptoms once the patients had been referred to a Renal Supportive Care Service based in the East End of London, UK. A high symptom prevalence was found with 30 different symptoms reported at first consultation. Widely reported symptoms impacting on daily living included breathlessness, oedema, pruritus, nausea and vomiting and pain. Findings ind

Obstructive sleep apnea caused by carotid body tumor: case report.

Obstructive sleep apnea (OSA) results from a structural compromise of the upper airway combined with decrease in muscle tone during sleep. Overt upper airway pathology is rare, however a variety of pharyngeal tumors have been well described as a cause of OSA. We describe a case of a mass originating in the carotid body resulting in severe OSA with hypersomnia resistant to positive pressure ventilation.

Peripheral osteoma of the mandible mimicking a parotid mass.

We report the case of a peripheral osteoma mimicking a benign tumor in the left parotid gland of a 75-year-old woman. The clinical presentation of the painless mass on the lateral surface of the mandibular ascending ramus was suggestive of a benign parotid tumor such as a pleomorphic adenoma. It was diagnosed using computed tomography (CT) and further delineated using 3D spiral CT scanning. Mandibular osteoma should be considered in differential diagnosis of parotid masses, which can be revealed by conventional radiographs.

Chronicle of a death foretold: a case of catastrophic vascular Behcet's disease.

A 20-year-old man with Behcet's disease characterized by recurrent arterial aneurysms presented with a new aortic root aneurysm. This patient previously had aneurysms of the coronary arteries and vein, as well as ruptured renal artery aneurysm. Chronic maintenance immunosuppressive therapy was recommended due to the catastrophic nature of the disease, which the patient refused to take. The patient died shortly after admission. This case demonstrates the unique catastrophic natural history of vascular Behcet's disease with recurrent life-threatening arterial events, and this case stresses the t

Unaffordable drug prices: the major cause of non-compliance with hypertension medication in Ghana.

PURPOSE: The prevalence of hypertension is increasing in Ghana. In addition hypertension has been identified as the most common cause of heart failure, stroke, chronic renal disease and spontaneous sudden deaths in Ghana. A major concern arising from this increasing hypertension prevalence is that many patients in this relatively poor country find it difficult to afford the standard hypertension medications. The purpose of this study was to evaluate access to hypertension medication and assess non-compliance with hypertension medication in Ghana. METHODS: Patient interviews were conducted on
