Defective regulated secretion: A trigger for Alzheimer's pathology?

Extracellular amyloid plaques formed from aggregated Amyloid-β (Aβ), a specific cleavage product of Amyloid Precursor Protein (APP), and intracellular tau-containing neurofibrillary tangles are the two key histopathological hallmarks of Alzheimer's Disease (AD). However, increasing evidence suggests that the trigger for neurodegeneration in AD involves intraneuronal defects in endolysosomes, which might be induced by both Aβ and tau. Recent high-resolution analysis of trafficking inside neuronal and non-neuronal cells suggests such defects may arise through aberrant compartmental maturation ev

Norovirus infection associated with unusual fatal case of Guillain-Barré syndrome in the community outbreak in Pune, India.

Norovirus (NoV) is a major global cause of acute viral gastroenteritis, responsible for both sporadic infections and widespread outbreaks affecting individuals across all age groups. Although typically self-limiting, gastrointestinal illness, characterized by nausea, vomiting, and diarrhea, recent evidence points to its potential role in causing nonintestinal complications. Central nervous system (CNS) manifestations such as febrile seizures, convulsions, and encephalopathy have been increasingly associated with norovirus, albeit infrequently. During a large Guillain-Barré Syndrome (GBS) outbr

The Correlation between Chronic Periodontitis and Oral Cancer.

Infections are increasingly considered as potential trigger for carcinogenesis apart from risk factors like alcohol and tobacco. The discussion about human papilloma virus (HPV) in oral squamous cell carcinoma (OSCC) points at a general role of infection for the development of oral carcinomas. Furthermore, first studies describe a correlation between chronic periodontitis and OSCC, thus, characterizing chronic inflammation as being a possible trigger for OSCC. In front of this background, we present four well-documented clinical cases. All patients showed a significant anatomical relation betw

Pain in trigeminal neuralgia: neurophysiology and measurement: a comprehensive review.

Trigeminal neuralgia (TN) is defined as sudden, usually unilateral, severe, brief, stabbing recurrent episodes of pain within the distribution of one or more branches of the trigeminal nerve. It is the most frequent cranial neuralgia, the incidence being 1 per 1,000,00 persons per year. Pain attacks start abruptly and last several seconds but may persist 1 to 2 minutes. The attacks are initiated by non painful physical stimulation of specific areas (trigger points or zones) that are located ipsilateral to the pain. After each episode, there is usually a refractive period during which stimulati…

Evoked potentials during active horizontal head rotations in patients with vertigo.

OBJECTIVES: The purpose of this study was to investigate whether evoked potentials by active head rotation help to verify and topographically differentiate patients with the major symptom vertigo. METHODS: Twenty-four healthy human subjects and 43 patients with either infratentorial or supratentorial brain lesions were analysed. RESULTS: The evoked response in normal subjects was composed of six peaks, indicated by polarization and time difference from the trigger points P100, N30, P0, N50, P155 and N320. The EEG pattern was independent of the direction, type of target and whether the eyes w…

Response of pain to static magnetic fields in postpolio patients: a double-blind pilot study.

OBJECTIVE: To determine if the chronic pain frequently presented by postpolio patients can be relieved by application of magnetic fields applied directly over an identified pain trigger point. DESIGN: Double-blind randomized clinical trial. SETTING: The postpolio clinic of a large rehabilitation hospital. PATIENTS: Fifty patients with diagnosed postpolio syndrome who reported muscular or arthritic-like pain. INTERVENTION: Application of active or placebo 300 to 500 Gauss magnetic devices to the affected area for 45 minutes. MAIN OUTCOME MEASURE: Score on the McGill Pain Questionnaire. RE

Can Real-World Data From Hospital Information Systems Be Directly Utilized for Efficacy Studies: An Analysis of Data Time Points.

BACKGROUND: Data from Hospital Information Systems are crucial components of real-world data, but concerns arise regarding the transparency of measurement time-points when directly utilizing for efficacy studies. The objective of the study was to analyze the distribution characteristics of time-points for laboratory test records from HIS. METHOD: Medical records before December 31, 2019 from Affiliated Hospital of Shandong University of Traditional Chinese Medicine, for patients primarily diagnosed with coronary heart disease (CHD), or heart failure combined with secondary diagnosis of CHD we

The auriculotemporal nerve in etiology of migraine headaches: compression points and anatomical variations.

BACKGROUND: The auriculotemporal nerve has been identified as one of the peripheral trigger sites for migraine headaches. However, its distal course is poorly mapped following emergence from the parotid gland. In addition, a reliable anatomical landmark for locating the potential compression points along the course of the nerve during surgery has not been sufficiently described. METHODS: Twenty hemifaces on 10 fresh cadavers were dissected to trace the course of the auriculotemporal nerve from the inferior border of the zygomatic arch to its termination in the temporal scalp. The compression

Trigger finger: the effect of partial release of the first annular pulley on triggering.

A prospective, randomized study was performed on 19 patients, in whom the proximal, middle, or distal third of the first annular pulley was divided to determine if a specific portion of the first annular pulley was responsible for the clinical triggering associated with restrictive flexor tenosynovitis. In all 19 patients, a partial resection of the first annular pulley resulted in continued clinical triggering with active digital flexion. At this point, a standard complete first annular pulley release was performed, with resolution of clinical triggering of the involved digit in all patients.

Luteinizing hormone dynamics during progestin-primed ovarian stimulation do not predict follicular output in women with normo-ovarian reserve.

BACKGROUND: Progestin-primed ovarian stimulation (PPOS) has emerged as an effective alternative to conventional pituitary suppression strategies in assisted reproductive technology cycles. While suppression of luteinizing hormone (LH) during PPOS is well established, the clinical relevance of dynamic LH changes during stimulation remains unclear. This study aimed to investigate whether serial LH dynamics are associated with follicular output in women with non-low ovarian reserve undergoing PPOS. METHODS: This retrospective cohort study included 616 ovarian stimulation cycles performed using a

Quantitative assessment of anatomical change using a virtual proton depth radiograph for adaptive head and neck proton therapy.

The aim of this work is to demonstrate the feasibility of using water-equivalent thickness (WET) and virtual proton depth radiographs (PDRs) of intensity corrected cone-beam computed tomography (CBCT) to detect anatomical change and patient setup error to trigger adaptive head and neck proton therapy. The planning CT (pCT) and linear accelerator (linac) equipped CBCTs acquired weekly during treatment of a head and neck patient were used in this study. Deformable image registration (DIR) was used to register each CBCT with the pCT and map Hounsfield units (HUs) from the planning CT (pCT) onto t
