[Diagnosis and management of peritonsillar abscess].

A peritonsillar abscess is a common infection in young adults. Clinical signs often include unilateral pharyngodynia, a "hot potato" voice, unilateral swelling with uvular deviation, asymmetric palatal contraction, and trismus. Complications include infections of the deep neck spaces and mediastinitis, which require prompt recognition due to their severity. Rapid diagnosis is essential. While not consistently used, CT scanning provides excellent specificity. The management of a peritonsillar abscess is based on an individualized approach that combines antibiotic therapy with interventional pro…

Ludwig's angina, anterior neck abscess and cavernous venous thrombosis following third molar extraction.

Ludwig's angina (LA) is a rapidly progressing cellulitis that develops after oral infections or molar extractions, typically causing fever and chills and rarely progressing to trismus and cyanosis. It is associated with risk factors including diabetes mellitus, heavy alcohol use, oral malignancy, poor dentition or an immunocompromised state. This case report reviews a previously healthy patient with no appreciable risk factors presenting with LA following a third molar extraction, complicated by extensive anterior deep neck space abscesses, cavernous venous thrombosis and Lemierre's Syndrome. …

Death from a possible anaphylactic reaction to ecstasy.

Ecstasy (3,4 methylenedioxymethamphetamine, or MDMA) is a recreational drug widely used among young people in discos or rave parties (1,2). MDMA is taken because it gives a feeling of euphoria, enhances energy and sociability, and heightens sensations and sexual arousal. However, several side effects have been described: headache, nausea, anorexia, xerostomia, insomnia, myalgia, trismus, and bruxism (2,3). More serious complications have also been reported, sometimes even leading to death: hyperthermia, disseminated intravascular coagulopathy, rhabdomyolysis, acute renal failure, liver failure…

Secondary disorders of glycosylation in inborn errors of fructose metabolism.

Adamowicz and colleagues raised the alert in 2007 about patients with atypical hereditary fructose intolerance (HFI) primarily misdiagnosed as CDG Ix. We describe a girl with neonatal hypertonia, facial trismus, absent swallowing and coughing reflexes, gastro-oesophageal reflux and sporadically elevated Krebs cycle metabolites and lactate. At 14 months microcephaly and hepatomegaly were noted, with hypertransaminasaemia but normal blood coagulation, glucose, phosphate, and absent urinary reducing substances. Neurological impairment persisted. Because of hepatic and neurological abnormalities w…

Targeted Sacral Biopsy With Portable Gamma Camera and Intraoperative Gamma Probe.

A 58-year-old man with dysphagia and trismus symptoms presented a lobulated tumor in the right tonsil and ipsilateral laterocervical stony adenopathies in the physical examination. He had prostate carcinoma as clinical precedent. The tonsilar lesion showed focal FDG uptake (SUV max , 10.89) on PET/CT and increased glycemic metabolism in the left sacral wing (SUV max , 10.90) without clear morphological lesion in CT. There was also uptake of the tracer in sacrum in the bone scan. Metastatic involvement was suspected, requiring radioguided biopsy to determine the etiology. Due to radioguided bio…

Bilateral peritonsillar abscess: A rare emergency.

Peritonsillar abscess, or quinsy, is a rare complication of acute tonsillitis. It usually presents with odynophagia, trismus, and muffled voice, reflecting the space-occupying lesion in the oral cavity. Examination reveals a unilateral swelling on either side of the soft palate, which drains thick pus after an incision is made. It is regarded as an emergency as an upper airway obstruction can develop. Bilateral peritonsillar abscess is a rare presentation and results in catastrophic sequelae. We present a case of bilateral peritonsillar abscess that was initially referred by a primary care cen…

Case report: Death in the emergency department: an unrecognized awake malignant hyperthermia-like reaction in a six-year-old.

A healthy 6-year-old boy developed lower extremity rigidity, trismus, and fever after playing in a splash pool. On arrival in the emergency department, he appeared to be seizing. An endotracheal tube was emergently placed using succinylcholine. Cardiac arrest followed. He could not be resuscitated. Postmortem genetic analysis found a novel RYR1 variant. Family testing revealed the same variant in his father who also had muscle contracture testing diagnostic for susceptibility to malignant hyperthermia and central core disease diagnosed histologically. Because there was no exposure to volatile …

Pseudogout mimicking an infratemporal fossa tumor.

BACKGROUND: The infratemporal fossa is a well-concealed, complex anatomical space. Neoplasms arising in this area are heterogenous in nature and have an insidious onset and usually present late. METHODS AND RESULTS: We present a case of a 71-year-old woman who presented with worsening facial pain, trismus, and a large mass in her infratemporal fossa with minimal associated temporomandibular joint destruction. She underwent a surgical excision of the mass, which revealed a pseudogout deposit. CONCLUSION: The clinical and radiological features of patients with tophaceous pseudogout frequently …

Bilateral peritonsillar abscesses: not your usual sore throat.

We describe a rare case of a previously healthy 30-year-old man who presented to the Emergency Department (ED) with bilateral peritonsillar abscesses. The clinical presentation of fever, trismus and odynophagia was consistent with pertonsillar abscess (PTA), but the presence of symmetric tonsillar swelling and midline uvula confounded the diagnosis. The true incidence of bilateral peritonsillar abscesses is unknown, but the incidence of unsuspected contralateral peritonsillar abscess identified at tonsillectomy has been reported to be between 1.9% and 24%. The diagnosis of bilateral peritonsil…

Bilateral peritonsillar abscess: case report and presentation of its clinical appearance.

Although there is no consensus on its incidence, bilateral peritonsillar abscess is an unusual variant of an otherwise relatively common otolaryngologic disease. A bilateral peritonsillar abscess can be differentiated from other oropharyngeal pathology with a detailed physical examination and complementary imaging. Its diagnosis should always be considered in patients who have signs and symptoms that are suggestive of peritonsillar abscess but whose intraoral examination yields atypical findings, as well as in patients with marked distress or trismus. This article describes the case of a young…

Clinical predictors of mandibular fractures.

To evaluate the diagnostic accuracy of clinical signs and symptoms of mandibular fracture, a prospective study of emergency department patients presenting with mandibular trauma was undertaken. Patients with airway compromise, who were edentulous, or could not cooperate with the physical examination were excluded. Over a 1-year period, 119 patients were studied. The presence of malocclusion, trismus, facial asymmetry, or a positive result on the tongue blade test (inability to grasp and hold a tongue blade between the teeth) was significantly associated with a mandibular fracture. Malocclusion…

Chronic Oral Complications of Oral Cancer Therapy.

Head and neck cancer (HNC) patients are uniquely susceptible to adverse oral health outcomes following head and neck radiation therapy (RT). Various risk factors for these oral health outcomes post-RT were identified in the OraRad study, which followed HNC patients from pre-RT to 2 y post-RT. Chronic oral complications include hyposalivation, trismus, dental caries, periodontal disease, tooth loss, exposed bone, and osteoradionecrosis of the jaw. Results from OraRad provide a framework for oral health care providers both in preparing patients for RT and in managing sequelae of RT for best outc…

Anesthetic Management of a Patient With Myhre Syndrome.

Myhre syndrome (MS) is a rare genetic connective tissue disorder characterized by intellectual disability, growth deficiency, muscular pseudohypertrophy, hearing loss, restricted joint mobility, laryngotracheal stenosis, choanal stenosis, and facial deformities. We encountered a case of difficult endotracheal intubation in a patient with MS. A 17-year-old girl with trismus, macroglossia, and difficulty with neck flexion was scheduled to undergo general anesthesia for dental treatment. Because of these characteristic clinical manifestations, endotracheal intubation was performed with a flexible…

Botulinum toxin therapy improves masseter spasticity in Amyotrophic Lateral Sclerosis.

Fifteen ALS patients, with troublesome symptoms linked to masseter spasticity, benefited from BoNT-A injections in each masseter. Based on the medical records of patients, the effect of the first injection was assessed one month later. We retrospectively collected information for 12 patients. Eight of them reported a beneficial effect after the injection for the following symptoms: trismus, tongue, lip and cheek biting, and jaw clonus. Five patients indicated that dental care was easier after injection. Our study showed that injections of BoNT-A unequivocally reduced masseter spasticity in ALS…
