Several cytokines play significant roles in the development and pathogenesis of pleural effusion. Little is known, however, about possible interactions between individual cytokines in terms of regulation of their relative abundance in the effusion. We studied 93 patients presenting with pleural effusion to the National Sanyo Hospital (68 men and 25 women; mean age, 64 years). Twenty-two patients had tuberculous pleurisy, 40 had malignant pleuritis, and 31 had effusions due to an etiology other than tuberculosis or cancer (miscellaneous). Pleural fluid concentrations of IL-2, IL-4, IL-5, IL-10,

BACKGROUND: In China, tuberculous pleural effusion is the most common cause for pleural effusion. Elevated ADH and positive tuberculin test usually are characteristic of tuberculous pleural effusion. We reported a 71-year-old male patient with elevated ADH and positive tuberculin test firstly misdiagnosed as tuberculous pleural effusion finally proven as pleural mesothelial sarcoma by thoracoscopic pathology.
METHODS: Appropriate laboratory tests and thoracentesis were carried out. Thoracoscopy and pathological biopsy were performed to differentiate tuberculous pleural effusion.
RESULTS: Che

132 tuberculous pleurisies were diagnosed between 578 thoracoscopies from 1970 to June 1985. A histological confirmation of diagnosis was achieved in 51.5% of the specific pleurisies, whereas in 15.2% typical endoscopic findings were present. 33.3% of the diseases were diagnosed based on the clinical and roentgenological appearance or ex juvantibus after antituberculous therapy. An increase of histologically negative biopsies was noted after a pretreatment of more than 4 weeks before thoracoscopy. This fact is interpreted as caused by an increasing exudation of fibrin, which permits to perform

CASE PRESENTATION: A 63-year-old man presented with fever, thoracalgia, weight loss, diffuse lymphadenopathy, and a massive pleural effusion. Extensive laboratory and radiologic investigations for possible autoimmune, infectious, hematologic, and neoplastic conditions all resulted negative. A lymph node biopsy showed a granulomatous necrotizing lymphadenitis, suspicious for tuberculosis. Although mycobacterium tuberculosis (MT) was never isolated and tuberculin skin test resulted negative, diagnosis of extrapulmonary tuberculosis was made and anti-tubercular therapy was started. Despite the st

BACKGROUND: Extrapulmonary tuberculosis (EPTB) constitutes 15-20% of tuberculosis cases in India. Earlier studies have evaluated treatment outcomes of EPTB with little information on outcomes of individual site of EPTB.
AIMS: The objective was to study the outcome of Directly Observed Treatment Short course (DOTS) treatment of EPTB in different organ systems under Revised National Tuberculosis Control Programme.
METHODS: Multi-centric retrospectives record review was carried out in three states in India. Data were collected from TB registers and analysed.
RESULTS: Of the total 2219 patients

Retrospective studies of pleural biopsy, cytology and ADA in pleural effusion were performed in 116 patients with pleural effusion between 1980 and 1988. Pleural malignant disease was diagnosed in 25 patients (75.8%) by cytology, in 19 patients (57.6%) by pleural biopsy. Thus, cytology should be performed first in patients with pleurisy. Both of cytologic study and CEA in pleural effusion were negative in 3 cases of squamous cell carcinoma. Tuberculous pleuritis was diagnosed in 24 patients (50.0%) by pleural biopsy, in 5 patients (10.4%) by isolation of Mycobacterium tuberculosis. Both pleura

OBJECTIVE: This study aimed to assess Interleukin-39 (IL-39) levels in various types of pleural effusion (PE), explore IL-39's diagnostic value in tuberculous pleurisy, analyze its correlation with other PE and tuberculosis indicators, and confirm the involvement of IL-39 in tuberculosis infection and the resulting inflammatory response.
METHODS: This study enrolled 113 patients with PE caused by different etiologies: 20 with transudative effusion, 39 with malignant pleural effusion (MPE), 15 with uncomplicated parapneumonic effusion (UPPE), and 39 with tuberculous pleural effusion (TPE). Enz

UNLABELLED: Aiming to detect reliable markers indicating protection from or susceptibility to tuberculosis infection, we investigated both Th1/Th2 cytokines and total IgE plasma levels in health care workers occupationally exposed to M. tuberculosis, in patients with pulmonary tuberculosis and in healthy persons.
MATERIAL AND METHOD: The study groups have included 15 health care workers in close contact with TB patients, patients with active pulmonary tuberculosis at diagnosis and after treatment (12 advanced and 10 moderate TB, of which 6 had also pleurisy) and 20 healthy volunteers. Periphe

Pleural manometry (PM) is a novel tool that allows direct measurement of the pressure in the pleural space in the presence of either a pleural effusion or a pneumothorax. Originally it was used to guide therapy for tuberculosis (TB) before the development of anti-TB medications. It was relegated to highly specialized centers for thoracoscopies until Light used it to investigate pleural effusions in the 1980s. However, there remains lack of robust data to support the routine use of PM. Recently additional published studies have generated renewed interest supporting the use of PM in specialized

BACKGROUND: The prevalence of pleural effusion in Mexico is over 400 per 100 000 inhabitants. The etiology is infectious in 45.7% and neoplastic in 32.6%. Closed pleural biopsy sensibility is 48-70% in cancer and 50-59% in tuberculosis using Cope or Abrams needle. In 1989, Tru-cut needle biopsy was described in a small study for massive pleural effusions with a sensibility of 86%. Our Institute has a wide experience with this infrequently procedure with reliable results than using Cope needle. Diagnostic yield should be evaluated. We aimed to evaluate the diagnostic yield of Tru-cut vs. standa

Pleural fluid studies showing a glucose value of less than 10 mg/dl, an LDH level greater than 1,000 IU/L, eosinophilia, a high protein value, and low pH are characteristic of paragonimiasis. In a patient with a pleural effusion and compatible clinical, radiologic, and serologic findings, pleural fluid analysis will allow diagnosis with reasonable certainty even in the absence of positive ova studies, and is useful in distinguishing paragonimiasis from tuberculosis.

Macrophage activation syndrome (MAS) is a rare and potentially fatal condition. Also known as macrophage activation syndrome, its diagnosis is based on the presence of at least five clinical and biological criteria. Bone marrow examination helps confirm the diagnosis by demonstrating the presence of numerous activated macrophages with multiple hemophagocytosis images. Through the case of an immunocompromised patient followed for chronic lymphocytic leukemia (CLL), we emphasize the difficulty of diagnosing MAS and the need to screen for underlying tuberculosis, which may be masked by manifestat

STUDY DESIGN: Case report with a review of scientific literature.
OBJECTIVE: To describe the course of tuberculous spinal disease (Pott's disease) complicated by pyogenic and tuberculous empyema, and chylothorax as there has been an increase in the numbers of notified cases of tuberculosis in the UK(1). To the best of our knowledge, a similar case has not been reported previously in the UK, although there has been a report of bilateral chylothorax associated with Pott's disease.
SETTING: A national spinal injuries unit in a Scottish university teaching hospital.
METHODS: Review of literatur

Six hundred and eighty-five patients requiring pleural drainage were referred to and managed by the Cardiothoracic Unit of the University College Hospital, Ibadan between June 1985 and September 1992. Two hundred patients, in which Aldon's urobag (e.g. Simpla S4 type) was used for pleural drainage were studied. The indications for pleural drainage were pyothorax or pyopneumothorax (106 patients), malignant pleural effusion (53 patients), tuberculous effusions (23 patients) and chest trauma (18 patients). In seven of these patients, the Aldon's urobag was used for out-patient care pleural drain

Gallbladder carcinoma (GBC) is the most common malignant tumor of the biliary system and presents with frequent locoregional lymphadenopathy and distant metastasis. Gallbladder tuberculosis (GT) is rare abdominal tuberculosis (TB). GBC and GT mimic each other. The clinical examinations and radiographic investigations sometimes fail to exhibit the difference between these two which are confirmed only after postoperative histopathological assessment. Herein, we report a patient of GBC with coexistent GT with pleural metastasis and pleural effusion. We emphasize the importance of differential dia
