Missed proximal radioulnar joint translocation with ulnar nerve entrapment: an unusual injury in a child.

Traumatic elbow dislocation in a child is rare, and it is usually associated with fractures. Simultaneous proximal radioulnar joint (PRUJ) translocation with ulnar nerve palsy is even rarer. We report an unusual case of a missed PRUJ translocation with ulnar nerve entrapment in a 10-year-old child. The key to diagnosing the translocation is the position of the proximal radius, which lies medial to the ulna. This was treated by open reduction and release of the entrapped nerve. The ulnar nerve palsy recovered fully at the 1-year follow-up. Although PRUJ translocation with ulnar nerve entrapment

How to treat Guyon's canal syndrome? Results from the European HANDGUIDE study: a multidisciplinary treatment guideline.

BACKGROUND: Although Guyon's canal syndrome is not highly prevalent, a considerable knowledge of anatomy is needed to localise and treat the pathology. Data on the effectiveness of interventions for this disorder are lacking. OBJECTIVE: To achieve consensus on a multidisciplinary treatment guideline for this disorder based on experts' opinions. METHODS: A European Delphi consensus strategy was initiated. In total, 35 experts (hand surgeons/hand therapists selected by the national member associations of their European federations and Physical Medicine and Rehabilitation physicians) participat

Lipofibromatous hamartoma: review of early diagnosis and treatment.

BACKGROUND: Lipofibromatous harmartoma (LFH) is a rare tumour that requires early diagnosis and treatment. To alert physicians to the possibility of this tumour, we review the epidemiologic and pathological characteristics, the presentation, diagnosis and treatment of LFH in the upper extremity. METHODS: We obtained data from a MEDLINE search of the English literature from 1966 to June 2003, using the search terms "lipofibromatous," "hamartomas of the nerve," "macrodactyly" and "intraneural lipoma." STUDY SELECTION: Articles that demonstrated institutional experience (epidemiologic, diagnost

A new operative technique: cubital tunnel decompression with endoscopic assistance.

The final outcome of a surgical decompression of the ulnar nerve depends on the precise release of all potential entrapment points. Careful attention must be given to all anatomic structures around the ulnar nerve and elbow area to ensure adequate dissection and surgical accuracy. In the authors' new endoscopic technique, glass tubes have been specially designed to house an endoscope and guide a meniscus knife, allowing for the release of all involved structures up to 10 cm both proximally and distally through a 3-cm skin incision. The authors present their experience in a cadaver trial and wi

Nerve injuries about the elbow in the athlete.

The athlete's elbow is a remarkable example of motion, strength, and durability. The stress placed on the elbow during sport, including the throwing motion, may lead to soft-tissue ligamentous and nerve injury. The thrower's elbow illustrates one example of possible nerve injury about the elbow in sport, related to chronic repetitive tensile and compressive stresses to the ulnar nerve associated with elbow flexion and valgus position. Besides the throwing athlete, nerve injury from high-energy direct-impact forces may also damage nerves around the elbow in contact sports. Detailed history and

Ulnar nerve injury with open-heart surgery.

The effects of different arm positions during open-heart surgery on the incidence of ulnar nerve injury were studied prospectively in 35 patients. It was concluded that ulnar nerve damage was common and was due to nerve compression at the elbow. A significant reduction in the incidence of nerve injury can be effected by placing the forearms above the head rather than at the patient's side.

Deficits in the function of small and large afferent fibers in confirmed cases of carpal tunnel syndrome.

Nerve conduction studies and measurements of thermal thresholds (hot and cold) were performed in 25 healthy control subjects and 24 patients with carpal tunnel syndrome: first, to compare the extent of abnormality demonstrable in the function of large and small afferent axons; second, to correlate these abnormalities with clinical severity; and third, to validate a modified form of palmar stimulation as an additional neurophysiological test in suspected carpal tunnel syndrome. The routine nerve conduction studies compared the amplitudes and conduction velocities from digit II and digit V to wr

Neuropathy in a patient with McArdle's syndrome and diabetes mellitus.

A juvenile-type diabetic patient of five years standing presented with a mononeuritis and gave a history of painful muscle swelling induced by exertion. Failure of the blood lactate to rise during ischaemic exercise and a normal blood glucose rise following intravenous glucagon confirmed the clinical diagnosis of muscle glycogenosis. The association of diabetes and McArdle's Syndrome has not previously been documented. An ulnar nerve palsy, which persisted for many months, followed the ischaemic exercise test possibly due to compression by muscular swelling, but may have been exacerbated by th

Incidence of Carpal Tunnel Syndrome After the Diagnosis of Ulnar Neuropathy.

PURPOSE: The purpose of this study is to determine whether patients diagnosed with ulnar nerve lesions (UNLs) have an increased risk of developing carpal tunnel syndrome (CTS). METHODS: A retrospective cohort study was conducted using the TriNetX database, which contains data of >100 million patients. We identified 360,021 patients with UNLs and matched them 1:1 with controls. The incidence of CTS was assessed at 1, 2, 3, 4, and 5 years following the diagnosis of UNL. Statistical analyses were used to evaluate the development of CTS in the UNL cohort compared with controls. RESULTS: Patients

Functional Outcomes of Cubital Tunnel Release in Patients with Negative Electrodiagnostic Studies.

BACKGROUND: Cubital tunnel syndrome (CuTS) is the second most common compression neuropathy of the upper extremity. Electrodiagnostic studies (EDSs) are often used to confirm diagnosis. However, negative EDSs can present a difficult clinical challenge. The purpose of this study was to determine the functional outcomes and symptom improvement for patients with a clinical diagnosis of CuTS, but with negative EDSs, who are treated surgically. METHODS: Patients who had EDSs before ulnar nerve surgery were identified by means of database search. Chart review was performed on 867 cases to identify

Incidence of ulnar neuropathy at the elbow in the province of Siena (Italy).

BACKGROUND: Although ulnar neuropathy at the elbow (UNE) is the second most frequent focal peripheral neuropathy of the upper limb after carpal tunnel syndrome (CTS), no figures on its prevalence or incidence in the general population have been reported. OBJECTIVE: To determine the incidence of UNE in a general population. METHODS: Retrospective study to identify newly diagnosed cases on the basis of clinical symptoms or signs and slowing of motor conduction velocity of the ulnar nerve across the elbow or surgical evidence of nerve compression in the elbow region. In the 5-year period from 1

Yoga induced acute ulnar nerve compression by a ganglion cyst in Guyon's canal.

Acute ulnar neuropathy at the wrist can be difficult to diagnose, as it is an uncommon neuropathy with variable clinical presentations and numerous etiologies. We present a case of acute ulnar neuropathy of the deep motor branch caused by a ganglion cyst in Guyon's canal. Interestingly, this case of acute loss of motor function occurred after the patient participated in yoga (specifically the downward dog position), and resolved spontaneously over time after stopping yoga, without surgical excision of the ganglion, suggesting exacerbation or protrusion of an occult ganglion cyst due to increas

Pseudoaneurysm formation following a traumatic wrist laceration.

Pseudoaneurysms occur secondary to partial disruption of the arterial wall. They are a commonly described complication of arterial injury, with penetrating injury and iatrogenic arterial catheterization being the most common etiologies in children. Many present weeks to months after the injury, and the initial vascular injury is often missed. The complications of pseudoaneurysm, which include thromboembolism, neurapraxia, and compartment syndrome, underscore the importance of early recognition and management. Definitive therapy consists of ultrasound-guided compression or resection and possibl

An anomalous accessory flexor digiti minimi profundus muscle: a case study.

A well-developed anomalous muscle within the forearm flexor compartment was discovered during a routine cadaveric dissection. The identified muscle originated from the intercompartmental septum on the medial side of the forearm just proximal to the wrist joint and inserted on the proximal phalanx of the fifth digit. When stressed, the tendon of the muscle produced flexion of the metacarpophalangeal joint of the fifth digit. Contiguous muscles, including the flexor digitorum profundus, displayed normal morphology. The muscle appeared to be an accessory belly of flexor digitorum profundus. The c

[Electrophysiological diagnosis in ulnar nerve entrapment at elbow].

Using "inching technique" we recorded antidromic sensory nerve action potentials from the little finger and compound muscle action potentials from the abductor digiti minimi, first dorsal interosseous and flexor carpi ulnaris muscles in 30 entrapped ulnar nerves. In cubital tunnel syndrome, localized conduction delay occurred most commonly at a point 2 to 4 cm distal to the medial epicondyle. In other ulnar neuropathies, with the exception of cubitus valgus deformity, conduction block or delay was noted at a site just distal to the medial epicondyle. These conduction abnormalities were most co
