OBJECTIVE: To describe and assess the feasibility of a novel surgical technique (pneumovaginoscopy) for performing vaginal surgery.
DESIGN: Video of a single surgical procedure.
SETTING: A university hospital.

BACKGROUND: Opportunistic salpingectomy at the time of hysterectomy is currently recommended to reduce the lifetime risk of ovarian cancer. The vaginal route is least invasive, but surgeons sometimes may choose to perform this procedure laparoscopically or robotically when anticipating adnexal surgery as a result of limited visibility.
TECHNIQUE: We describe a surgical technique to make salpingectomy by the vaginal route easier and more feasible. Posterior rotation of the uterus provides excellent access to the tube, which can easily be removed before separation of the adnexa.

INTRODUCTION: Bartholin gland carcinoma is an extremely rare condition. Because of its, phase III trials have not been carried out, there exists no unanimous consensus on treatment and guidelines are missing.

Leiomyomas are benign, mesenchymal, monoclonal tumors that typically originate from myometrium smooth-muscle cells, although atypical sites such as the vagina, lungs, vascular structures, and retroperitoneal area have been reported. We present the case of a leiomyoma that originated from the vaginal cuff in a 70-year-old woman, 25 years after total abdominal hysterectomy and bilateral salphingo-oophorectomy.

Perivascular epithelioid cell tumor (PEComa) is a rare mesenchymal tumor with an unpredictable natural history. We present the first reported case of PEComa of the vagina diagnosed in an 8-year-old girl. This was initially diagnosed on biopsy as an embryonal rhabdomyosarcoma, and the patient underwent 3 cycles of chemotherapy according to the Mesenchymal Malignant Tumour 1989 protocol. A repeat scan, however, revealed an increase in tumor size. Total resection was achieved via a transvaginal (vertical sleeve) partial vaginectomy.
