A five to ten year follow-up study of parietal cell vagotomy.

Records of 51 consecutive patients who underwent parietal cell vagotomy at the Lahey Clinic Medical Center and who had follow-up studies of five to ten years were reviewed. Operation was performed for intractability in 25 patients, intractability and obstruction in 19 patients and bleeding in seven patients. Patients with pyloroduodenal stenosis underwent digital dilation. No operative deaths occurred. Ulcer recurred in two of 25 patients (8 per cent) treated for intractability, in three of 19 patients with obstruction and in two of seven patients operated upon for bleeding. Ulcers recurring i…

Molecular forms of gastrin in antrum and duodenum after parietal cell vagotomy.

We examined whether parietal cell vagotomy (PCV) changed the distribution between the different molecular forms of gastrin. Serum, antrum, and duodenum from PCV patients, unoperated duodenal ulcer (DU) patients and control subjects were studied. PCV was followed by a twofold increase in the serum gastrin concentration, while the distribution between small and large gastrins were unaffected. However, sulfation of antral gastrins was reduced from 45 percent in unoperated DU patients to 34 percent in PCV patients (p less than 0.01). We conclude that gastrin sulfation is diminished after PCV. Dimi…

Parietal cell vagotomy for duodenal and pyloric ulcer: clinical and secretory results.

UNLABELLED: In the Department of Surgery, University of Turku, 310 patients underwent parietal cell vagotomy for duodenal (268 patients) or pyloric-prepyloric ulcer (42 patients) in the years 1973-82. The male/female ratio was 4/1 and mean age 43 years. There was no mortality. Splenic injury led to splenectomy in 2.6%. A relaparotomy for intraabdominal bleeding was done in 1%. No case of minor curve necrosis occurred. During the follow-up of 3-9 (mean 5) years 9 patients had died of unrelated causes and 29 could not be traced leaving 272 patients for study. Late symptoms occurred as follows: D…

Gastro-oesophageal reflux and oesophagitis before and after vagotomy for duodenal ulcer.

Fifty patients undergoing elective vagotomy for the treatment of chronic duodenal ulceration have been investigated pre-operatively and again 3 months postoperatively to determine the extent and severity of associated gastro-oesophageal reflux. Pre-operatively all patients had a normal lower oesophageal sphincter pressure but 50 per cent had symptoms of gastro-oesophageal reflux, 42 per cent had excessive reflux on 24 h pH monitoring and 30 per cent had oesophagitis on endoscopy and/or oesophageal biopsy. Postoperatively, reflux symptoms were present in only 12 per cent of patients but pH stud…

Bilateral trunkal vagotomy (BTV) regarded as a pathogenic process. An experimental study.

Experimentally, bilateral trunkal vagotomy (BTV) generated some early acute modifications represented during the first 14 days by blood stasis, partial thrombosis, inflammatory infiltrates and necrobiosis of the gastric mucosa. The vascular lesions are the first to occur, the tissular ones being secondary. Later on, two and three months after surgery, chronic inflammation and metaplasia of the intestinal type have been detected as well.

An easier method for identification of the antral nerves for a simplified proximal gastric vagotomy.

A practical method of easily identifying the antral nerves for marking the boundary between the antrum and the parietal cell bearing proximal stomach is suggested. This has reduced the operative time, simplified PGV and proved to be clinically effective in 79 patients who underwent operation.

Effects of proximal gastric vagotomy and antrectomy on parietal cell function in humans.

Both proximal gastric vagotomy and antrectomy reduce maximal gastric acid secretion in vivo by about 60%. The combination of vagotomy and antrectomy reduces the maximal acid secretion by about 80%. This additive effect indicates that these surgical procedures differ in their mode of action. The function of isolated human oxyntic glands was studied before and after vagotomy and antrectomy, respectively, using radioactively labeled aminopyrine as a marker of parietal cell response. The basal accumulation increased after vagotomy, suggesting a vagally controlled inhibitory component. The carbacho…

Long-term results of vagotomy and pyloroplasty in the treatment of gastric ulcer disease.

Two hundred thirteen patients were evaluated to establish the efficacy of vagotomy and pyloroplasty in the treatment of benign gastric ulcer disease. These patients underwent operation between the years 1960 to 1973. Ten patients (5%) were found to have occult gastric malignancies and four of these patients (40%) were cured after resectional therapy. Nineteen patients underwent pyloroplasty without vagotomy, and these patients were ultimately excluded from the study in view of the frequency (26%) of ulcer recurrence. The operative mortality rate was 8.4% overall (1.5% when related to elective …

The role of altered extrahepatic biliary function in the pathogenesis of gallstones after vagotomy.

Several clinical studies have documented an increased incidence of gallstones after truncal vagotomy. Previous laboratory studies, however, have failed to establish a clear mechanism for this phenomenon. Whereas lithogenic bile is a prerequisite for cholesterol cholelithiasis, gallbladder stasis has also been shown to be a critical factor in stone formation. We tested the hypothesis that truncal vagotomy alters extrahepatic biliary function by performing either sham laparotomy or truncal vagotomy and pyloroplasty in the prairie dog. After operation, animals were fed a trace-cholesterol (nonlit…

Numerical variation of parietal cells after vagotomy in cases of duodenal ulcer.

The present study was conducted histologically to elucidate the influence of vagotomy on the gastric mucosa. Bioptical tissues were obtained from 10 control cases and from 26 cases of duodenal ulcer using an endoscope. Of the 26 cases of duodenal ulcer, 20 were subjected to truncal vagotomy with pyloroplasty and 6 to truncal vagotomy with hemigastrectomy. In the cases of duodenal ulcer, tissues were obtained at the preoperative stage, and at 1, 3, 6 and 12 months on the time-course basis after the operation. The parietal cells were counted in a unit area (0.0176 mm2) between the middle and low…

Inhibition of the isoprenaline-induced renin release by bilateral vagotomy is mediated by vasopressin.

The influence of cervical vagotomy on the isoprenaline-induced renin and vasopressin release was investigated in rats. Plasma vasopressin levels were measured using a specific radioimmunoassay. Bilateral vagotomy enhanced the isoprenaline-provoked vasopressin release; however, the simultaneous increase in plasma renin concentration was diminished. This effect of bilateral vagotomy on renin release could be mimicked by i.v. infusion of vasopressin into rats whose vagi remained intact. On the other hand, in Brattleboro rats with vasopressin deficiency, bilateral vagotomy did not modify renin rel…

The effect of vagotomy on the distribution of sulphomucin-containing cells in the rat pyloric antrum.

The present histological investigation showed that bilateral truncal vagotomy (with accompanying pyloroplasty) reduced the sulphomucin-containing cell population of the pyloric antral mucosa of the rat stomach. Although this did not reach statistical significance (95% confidence limits), study of the absolute distribution of the sulphomucin-containing cells showed that significantly fewer of these cells were situated in the basal third of the mucosa as a result of the operation. A study of the percentage distribution revealed that a much higher percentage of sulphomucin-containing cells was pr…

Changes in gastric mucosal cell proliferation after antrectomy or vagotomy in man.

The rate of gastric epithelial cell proliferation was studied in peptic ulcer patients treated by partial gastrectomy or proximal gastric vagotomy (PGV). Endoscopic biopsies were incubated in vitro with 3H-thymidine and autoradiographs were prepared. The percentage of DNA-synthesizing cells (labeling index) in the progenitor cell region was estimated. Five patients were studied before and 3 months after antrectomy, whereas six other subjects were studied before and 2 weeks and 2 months after PGV. The results indicate that epithelial cell proliferation in fundic mucosa is accelerated after antr…

Reperitonealization and invagination of the lesser curvature of the stomach following proximal gastric vagotomy.

Lesser curve necrosis usually presents as free perforation. A case of large gastric ulcer occurring very shortly after proximal gastric vagotomy (PGV) for a duodenal ulcer that was almost certainly due to ischemic necrosis of the lesser curve is presented here. Reperitonealization and invagination of the lesser curve is recommended following PGV so that, if necrosis occurs, it will take place within the stomach and not into the free peritoneal cavity. This maneuver may also avoid possible vagal reinnervation and the formation of dense adhesions between the stomach and liver.
