Recovery of vancomycin-resistant gram-positive cocci from pediatric liver transplant recipients.

Between November 1988 and October 1989, 49 first-time pediatric liver transplant recipients at the Children's Hospital of Pittsburgh were prospectively monitored for the presence of stool colonization and the development of disease caused by vancomycin-resistant gram-positive cocci (VRGPC). Quantitative stool culturing was done on a weekly basis, and cultures were planted onto a selective medium for VRGPC. Isolates for which the MIC was greater than or equal to 8 were considered resistant to vancomycin. Patients were monitored clinically for the development of infection, and their charts were

VraSR has an important role in immune evasion of Staphylococcus aureus with low level vancomycin resistance.

Vancomycin-intermediate Staphylococcus aureus (VISA) and heterogeneous VISA (hVISA) are increasingly being reported as associated with treatment failure. Previous studies indicated that VISA/hVISA resists clearance by the host immune system, thereby allowing persistence within the host. VraSR is a vancomycin-resistance-associated sensor/regulator that is highly expressed in VISA/hVISA strains. Whether VraSR plays an important role in immune escape by VISA/hVISA strains is unclear. Here, we constructed a vraSR deletion mutant strain (ΔvraSR) and complementary strain (CΔvraSR) in Mu3 to investig

Vancomycin-resistance gene cluster, vanC, in the gut microbiome of acute leukemia patients undergoing intensive chemotherapy.

Two recent reports suggested that the less common, less virulent enterococcal species, Enterococcus gallinarum and E. casseliflavus, with low-level vancomycin resistance due to chromosomally encoded vanC1 and vanC2/3, may influence host immunity. We reported that peri-transplant gut colonization with E. gallinarum and E. casseliflavus is associated with lower mortality after allogeneic hematopoietic cell transplantation (HCT). Because most acute leukemia patients undergoing HCT have received intensive chemotherapy (usually requiring prolonged hospitalization) for their underlying disease befor…

Loss of Microbiota-Mediated Colonization Resistance to Clostridium difficile Infection With Oral Vancomycin Compared With Metronidazole.

Antibiotic administration disrupts the intestinal microbiota, increasing susceptibility to pathogens such as Clostridium difficile. Metronidazole or oral vancomycin can cure C. difficile infection, and administration of these agents to prevent C. difficile infection in high-risk patients, although not sanctioned by Infectious Disease Society of America guidelines, has been considered. The relative impacts of metronidazole and vancomycin on the intestinal microbiota and colonization resistance are unknown. We investigated the effect of brief treatment with metronidazole and/or oral vancomycin o

BACKGROUND: In veterinary medicine, particularly concerning dogs and cats, there is limited data regarding vancomycin-resistant Enterococcus (VRE). However, multidrug-resistant Enterococcus is frequently identified, raising concerns about the potential for spreading these resistant microorganisms to humans due to their zoonotic nature. This study aimed to identify VRE colonizing animals admitted to a veterinary hospital and to determine the presence of the major resistance genes responsible for vancomycin resistance. RESULTS: Enterococcus was found to carry the vanA gene in 52.54% of cases, t…

Active surveillance of vancomycin-resistant Enterococcus faecium after its outbreak at patient admission.

Vancomycin-resistant Enterococcus faecium (VRE) causes high mortality and has been increasingly detected worldwide in recent years; however, its screening effectiveness at admission remains controversial. We aimed to evaluate the effectiveness of active surveillance culture (ASC) for VRE at admission following a nosocomial outbreak. A VRE outbreak occurred at Shizuoka General Hospital in 2022, and ASC for VRE was performed upon admission for high-risk cases after the outbreak between January 2023 and December 2023. ASC was conducted in 2941 patients at admission, with 13 of them testing positi

Rapid Transmission and Divergence of Vancomycin-Resistant Enterococcus faecium Sequence Type 80, China.

We investigated genomic evolution of vancomycin-resistant Enterococcus faecium (VREF) during an outbreak in Shenzhen, China. Whole-genome sequencing revealed 2 sequence type 80 VREF subpopulations diverging through insertion sequence-mediated recombination. One subpopulation acquired more antimicrobial resistance and carbohydrate metabolism genes. Persistent VREF transmission underscores the need for genomic surveillance to curb spread.

Repurposing bunamidine hydrochloride as a potent antimicrobial agent targeting vancomycin-resistant Enterococcus membranes.

Vancomycin-resistant enterococci (VRE) are a major cause of hospital-acquired infections, with limited treatment options due to rising antibiotic resistance. Targeting bacterial membranes offers a promising alternative to conventional therapies. In this study, we investigated the antimicrobial activity and mechanism of action of bunamidine hydrochloride (BUN) against VRE. BUN exhibited potent bactericidal effects against both VRE and vancomycin-susceptible enterococci (VSE), with minimum inhibitory concentrations (MICs) ranging from 2 to 4 µg/mL. BUN significantly inhibited biofilm formation a
