Autoantibodies in primary systemic vasculitis.

Strong evidence for the presence of autoantibodies in the primary systemic vasculitides now exists, and there is a particularly marked association between ANCA and Wegener's granulomatosis and microscopic polyarteritis. The presence of ANCA serves to place these disorders in a firm autoimmune setting. Although ANCAs are of undoubted diagnostic significance, their role in etiology and pathogenesis has still to be fully explored, so further developments in this area are awaited with interest.

HIV associated systemic necrotizing vasculitis.

The spectrum of HIV associated rheumatic syndromes continues to expand as does our experience with the disease. The following is a case of HIV associated systemic necrotizing vasculitis documented by rectal biopsy. The patient was treated successfully with corticosteroids alone, and after 9 months of followup has no evidence of progression to frank AIDS.

[Scrotal vasculitis simulating recurrent epididymitis].

We present a case of scrotal vasculitis, of difficult diagnosis despite the patient's history, where it was necessary to resort to surgery in order to establish definitely its diagnostic differentiation with a specific epididymitis. Taking advantage of this uncommon form of presentation of scrotal vasculitis, in this case an evolved Schonlein-Henoch syndrome in a 35-year-old patient, we analyse the different forms of gonad, epididymis and scrotal affectation recognised in cases of vasculitis, with special reference to the forms of acute presentation, simulating testicle torsion.

Fournier's gangrene in hypersensitivity vasculitis.

Fournier's gangrene in IgE positive hypersensitivity vasculitis demands concurrent high dose immunosuppression, radical surgery and intensive treatment of septic sequelae. Even extended autologous skin grafting is possible with immunosuppression. A case of fulminant necrotizing infection of the genitalia, perineum and abdominal wall during the acute generalization phase of IgE positive hypersensitivity vasculitis is described. Apart from the radical surgical approach, medical treatment demands a different strategy than in classical Fournier's gangrene. The favorable outcome in a 21-year-old ma…

Antithrombotic treatment in livedo vasculitis.

Two patients with livedo vasculitis were treated successfully with antiplatelet drugs including ticlopidine hydrochloride, dipyridamole, and low-dose aspirin. Increased platelet functions were restored 1 week after the beginning of the treatment, followed by dramatic improvement of painful leg ulcers within 1 month. Livedo status was unchanged. We claim that antiplatelet therapy should be the first choice of treatment in this disease.

Cerebral vasculitis in relapsing polychondritis.

Recurrent inflammation of cartilage in multiple sites is a hallmark of relapsing polychondritis (RP). Neurologic complications of this disease have begun to attract increasing attention, but the neuropathologic basis of these complications has not been described. We report a patient with RP whose autopsy showed extensive cerebral and systemic vasculitis.

Acute multifocal hemorrhagic retinal vasculitis.

The authors present a series of seven patients with acute visual loss associated with mild anterior uveitis, multifocal retinal vasculitis, retinal capillary nonperfusion, retinal hemorrhage, disc swelling, and vitreitis. Oral prednisone was of some benefit in these patients and oral acyclovir was generally ineffective. Neovascular complications including retinal, disc, choroidal, and iris new vessels were common, requiring photocoagulation in five patients. Horseshoe tears developed in two patients in zones of uninvolved retina but retinal detachment did not occur. The etiology remains unknow…

Vasculitis in hairy-cell leukemia.

Vasculitis associated with hairy-cell leukemia (HCL) has been reported occasionally. We determined the clinical and biological significance of this association by the retrospective study of a series of 50 patients with HCL, including nine patients with vasculitis. The development of vasculitis was not related to a variant of HCL on the basis of hematologic findings and survival. Vasculitis could occur at any time during the course of HCL, and was the circumstance for the diagnosis of HCL in three cases. Clinical and immunohistologic findings were those of hypersensitivity vasculitis in the nin…

Nasopharyngeal tumour in hypersensitivity vasculitis.

Hypersensitivity vasculitis refers to a large and heterogeneous group of clinical syndromes that have in common the predominant involvement of small vessels, especially in the skin. But any organ can be involved with its associated specific clinical and pathologic manifestations. As involvement of the upper airways has not previously been reported, a case of vasculitis associated with a nasopharyngeal tumour is presented. The possible etiology and relation to other diseases in the upper respiratory tract, especially Wegener's granulomatosis, is discussed.

[Retinal vasculitis in lupic disease].

Three cases of retinal vasculitis in SLE-type diseases are reported. The first was central retinal vein occlusion occurring during clinical remission of SLE in a 55 year old black female. Prednisone maintenance therapy was unchanged and visual loss rapidly regressed with heparin therapy. The second case was a 33 year old black female in whom SLE was discovered following relapsing bilateral optic neuritis. A progressive visual improvement was obtained with high dose of prednisone (1 mg/kg/day). The third cas was a 17 year old white girl with retinal vasculitis. She had an unclassified connectiv…

Atypical syphilitic chorioretinitis and vasculitis.

Although syphilis is frequently overlooked as a cause of ocular disease, it remains an endemic disease and its incidence is increasing. This article reports on four recent cases of ocular syphilis, including pseudoretinitis pigmentosa, chorioretinitis, and a rare presentation of an isolated retinal vasculitis involving both the arteries and the veins. Fundus photography, fluorescein angiography, and electrophysiologic testing are included. These cases show the diverse manifestations of ocular syphilis, which may involve any structure in the eye, and they demonstrate that atypical presentations…
