Flow-diverting stent for ruptured intracranial dissecting aneurysm of vertebral artery.

BACKGROUND: The treatment of ruptured dissecting aneurysms of the intracranial vertebral artery (VA) with parent vessel preservation is a challenge for neurosurgeons and interventional neuroradiologists. OBJECTIVE: To propose an indication for flow-diverting treatment for reconstruction of a dissecting VA with acute subarachnoid hemorrhage. METHODS: Two male patients transferred after acute subarachnoid hemorrhage and dissecting aneurysm on the V4 segment of the dominant VA. An occlusion test was not performed because of their poor clinical state. A flow-diverting stent represented by the Pi

Coil embolization of intracranial dissecting vertebral artery aneurysms with subclavian steal.

Subclavian steal phenomenon occurs when cerebral blood flow is diverted to supply the arm. We report 3 patients with asymptomatic subclavian steal phenomenon with retrograde blood flow in the vertebral artery who presented with subarachnoid hemorrhage due to dissecting aneurysms of the involved vertebral artery. The association of subclavian steal phenomenon complicated by subarachnoid hemorrhage due to dissecting vertebral aneurysms has not been previously described.

Application of covered stent grafts for intracranial vertebral artery dissecting aneurysms.

OBJECT: Utilization of covered stent grafts in treating neurovascular disorders has been reported, but their efficacy and safety in vertebral artery (VA) dissecting aneurysms needs further investigation. METHODS: Six cases are presented involving VA dissecting aneurysms that were treated by positioning a covered stent graft. Two aneurysms were located distal to the posterior inferior cerebellar artery, and 4 were located proximal to the posterior inferior cerebellar artery. Aspirin as well as ticlopidine or clopidogrel were administered after the procedure to prevent stent-related thrombosis.

[Case of bilateral vertebral artery dissecting aneurysm presenting with subarachnoid hemorrhage].

Vertebral artery dissecting aneurysm (VADA) is a relatively rare cause of subarachnoid hemorrhage (SAH). Bilateral VADAs are even rarer, and there is no established treatment for this type of VADA. We report a case of bilateral VADAs with SAH. A 45-year-old man suddenly developed headache and consciousness disturbance and was referred to our hospital. CT scans demonstrated SAH mainly in the left cerebello-pontine cistern. Three dimensional computed tomographic angiography (3D-CTA) revealed fusiform dilatation of the bilateral vertebral arteries (VAs), suggesting dissecting aneurysms. The aneur

[Vertebral arteries dissection as a rare cause of central nervous system ischemia].

However rare, dissection of extracranial cerebral arteries is one of the most often causes of ischemic stroke in young people. Early diagnosis and treatment promote artery recanalization that results in total symptom regression. The proper diagnosis is in many cases is difficult. The reason is the variety of causative factors, clinical symptoms and difficulty of direct dissected vertebral arteries visualisation in common Duplex-Doppler examination. In our article we present 3 causes of vertebral arteries dissection. We would like to call your attention to consider this causative factor in diff

[Intracranial extension of extracranial vertebral artery dissections. A review of 16 cases].

INTRODUCTION: Vertebral artery (VA) dissections can involve both the extracranial and intracranial portions of the VA. Intradural extension explains the occurrence of subarachnoid hemorrage (SAH). We have studied the rate of this extension, the risk of associated SAH and the therapeutic repercussions at the acute stage. METHODS: From 1985 to 2001, 42 patients with a recent extracranial VA dissection were admitted to our department of neurology. When the diagnosis of extracranial VA dissection (involving the first, second or third segment of the VA) was established, we looked for an ipsilatera

Sudden onset pharyngeal pain associated with dissecting vertebral artery aneurysm.

BACKGROUND AND PURPOSE: We report a case of sudden onset pharyngeal pain due to dissection of the vertebral artery. CASE DESCRIPTION: A 65-year-old patient presented with sudden onset of unilateral pharyngeal pain, which was first, diagnosed as idiopathic glossopharyngeal neuralgia. Magnetic resonance image and cerebral angiography revealed a dissecting vertebral artery aneurysm. The anatomical site of the aneurysm may explain this complication. CONCLUSIONS: Sudden onset of pharyngeal pain, although rare, should be considered as a complication of dissection of the vertebral artery even thoug

Rebleeding of intracranial dissecting aneurysm in the vertebral artery following proximal clipping.

We describe a case of rebleeding from an intracranial vertebral dissecting aneurysm following proximal clipping. This case suggests that proximal clipping alone may not be an adequate surgical procedure to prevent rebleeding. Surgical treatment of intracranial dissecting aneurysms in the vertebral artery presenting as subarachnoid haemorrhage. (SAH) is discussed.

Intracranial vertebral artery dissections: clinical, radiological features, and surgical considerations.

We reviewed 24 patients with intracranial vertebral artery dissections treated during the last 12 years. Sixteen patients were admitted with subarachnoid hemorrhage (SAH) and 8 did not have SAH. The mean age at the time of onset was 50.0 years. Male preponderance was noted. Among 21 patients with acute onset, 6 (29%) experienced prodromal neck pain and 3 (60%) of 5 SAH patients showed nuchal stiffness when examined within 6 hours of onset. The preoperative angiographical findings were uniform in SAH cases in contrast to the varied angiographical findings seen in non-SAH cases. So-called pearl …

[Dissecting aneurysm of the vertebral artery--report of two cases and review of the literature].

Two cases of dissecting aneurysm of the vertebral artery are reported and the 70 cases reported previously are reviewed. Case 1 showed Wallenberg's syndrome following sudden headache on the right side without SAH. Angiograms demonstrated the "pearl and string sign" on the right vertebral artery, characteristic of a dissecting aneurysm. Through a right suboccipital craniectomy, the right vertebral artery was found to be discolored purplish-red and swollen, so the proximal vertebral artery was clipped. The postoperative course was uneventful. Case 2 showed Wallenberg's syndrome with antecedent h

Dissecting aneurysms of the vertebral arteries following cervical manipulation: a case report.

Neck manipulation may uncommonly be associated with serious and even fatal vascular complications. Although well recognised, the nature of the vascular injury has only rarely been directly established by pathological examination. The case is reported of a 43-year-old man who died following neck manipulation, and in whom multiple dissecting aneurysms within both vertebral arteries were demonstrated radiologically and found at necropsy. Bilateral dissecting aneurysms were found both at the level of atlanto-axial articulation and close to the origins of the vertebral arteries. No predisposition w

Headache characteristics of uncomplicated intracranial vertebral artery dissection and validation of ICHD-3 beta diagnostic criteria for headache attributed to intracranial artery dissection.

BACKGROUND AND PURPOSE: Headache may be a warning sign of subsequent stroke in patients with vertebral artery dissection (VAD). Even though the headache characteristics of VAD have been described predominantly in patients with extracranial VAD and neurological complications, headache semiology is not well known in patients with uncomplicated intracranial vertebral artery dissection (ICVAD). In the present study, we attempt to identify the headache semiology that characterizes ICVAD and validate the revised version of the International Classification of Headache Disorders (ICHD-3 beta) criteria…

Perfusion- and diffusion-weighted MR imaging-guided therapy of vertebral artery dissection: intraarterial thrombolysis through an occipital vertebral anastomosis.

Management of arterial dissections can be particularly challenging. We report a case of vertebral artery dissection in which perfusion- and diffusion-weighted MR imaging findings suggested the presence of salvageable tissue, despite that the patient had symptoms for more than 40 hours. Direct access to the distal vascular territory was unattainable, and the presence of collateral circulation through an occipital vertebral anastomosis provided the only pathway to administer intraarterial thrombolysis. This case demonstrates that perfusion- and diffusion-weighted MR imaging can be instrumental i…

De Novo Vertebral Artery Dissection after Endovascular Trapping for Ruptured Dissecting Internal Carotid Artery Aneurysm: Case Report.

The authors present an extremely rare case of a 54-year-old female patient with subarachnoid hemorrhage due to a rupture of a dissecting internal carotid artery (ICA) aneurysm, who developed de novo vertebral artery dissection in the spasm period after endovascular trapping of the ICA. Interestingly, postoperative cardiopulmonary monitoring showed high global end-diastolic volume index and mean arterial pressure, which could contribute to this de novo dissection via hemodynamic stress in the cerebral circulation. Spontaneous intracranial artery dissection of more than two arteries is rare, and…

Treatment of intracranial vertebral artery dissecting aneurysms involving the posterior inferior cerebellar artery origin.

BACKGROUND: Vertebral artery dissecting aneurysm (VADA) involving the origin of the posterior inferior cerebellar artery (PICA) is a complex disease entity in which the dual goals of preventing future rebleeding and maintaining perfusion of the lateral medulla must be considered. We present an illustrative case and review the literature surrounding treatment strategies. CASE DESCRIPTION: We report a patient presenting with extensive subarachnoid hemorrhage due to rupture of an intracranial VADA involving the PICA origin. After consideration of the patient's cerebral vasculature and robustness
