Vestibular Preservation in Pediatric Cochlear Implantation.

OBJECTIVES: Evaluate the rate of preserved vestibular function in pediatric cochlear implant surgery. STUDY DESIGN: Retrospective case review. METHODS: Pre- and post-operative vestibular tests were compared in children who underwent cochlear implantation at a tertiary level pediatric hospital over a 4-year period. RESULTS: Data from 59 implanted ears in 44 children was included. Median age was 2.8 years at initial testing (range 7 months - 21 years) with 1:1 male/female ratio. Implant surgeries were 26 unilateral, 13 bilateral simultaneous, and 5 bilateral sequential. The majority were impl

[Acoustic neuroma--vestibular schwannoma--personal experience of up-to-date management].

Acoustic neuroma, properly called vestibular schwannoma, arises from the Schwann cells of the vestibular transitional zone of the vestibulocochlear nerve as the most frequent tumour of the posterior fossa. Its incidence is estimated at 1.2 vestibular schwannoma per a population of 100,000/year. As to size, vestibular schwannoma is classified into grades I to IV. Its benign variety threatens the patient with hearing and balance impairment; uncontrolled growth can lead to death. About one third of small vestibular schwannoma show hardly any growth, the larger ones grow aggressively. The author's

Discordant vestibulo-ocular reflex function according to the frequency and mode of stimulation.

This study aimed to determine the incidence, pattern, and etiology of dissociated vestibulo-ocular reflex (VOR) function according to the stimulus frequency in dizzy patients. We retrospectively evaluated the results of bithermal caloric tests and video-head impulse tests (vHITs) in 1022 patients with dizziness or vertigo between July 2016 and April 2021. Patients were classified into concordant group (normal or abnormal results on both tests) and discordant group (dissociated results between two tests). Of 1022 patients, discordant group had 159 (16%), comprising abnormal horizontal vHITs wit

Clinical applications of correlational vestibular autorotation test.

CONCLUSION: The correlational vestibular autorotation test (VAT) system has the advantages of good test-retest reliability and calibrations of absolute degrees of eye movement are unnecessary when acquiring a cross correlation coefficient (CCC). The approach is able to efficiently detect peripheral vestibulopathies. OBJECTIVE: A VAT has some drawbacks including poor test-retest reliability and slippage of sensor. This study aimed to develop a correlational VAT system and to evaluate the reliability and applicability of this system. METHODS: Twenty healthy participants and 10 vertiginous pati

Early vestibular physical therapy rehabilitation for Meniere's disease.

Meniere disease includes symptoms of fluctuating hearing loss, tinnitus, and subjective ear fullness accompanied by episodic vertigo. Along with these symptoms, patients with chronic Meniere often develop symptoms of disequilibrium and unsteadiness that extend beyond the episodic attacks and contribute to the total disability and reduced quality of life attributed to the disease. Vestibular rehabilitation physical therapy has been used only after vestibular ablation has stabilized the vestibular loss, and for patients stably managed on medical therapy who exhibit no fluctuation in symptoms. Dizziness of suspected cervical origin distinguished by posturographic assessment of human postural dynamics.

Useful clinical tests are lacking for the controversial entity "cervical vertigo". In earlier studies patients assumed to suffer from cervical vertigo or dizziness manifested disturbed postural control as compared to healthy subjects, but were hard to distinguish from patients with other balance disorders. Using posturography in which stance was perturbed by a vibratory stimulus applied towards the calf muscles, we studied 16 consecutive patients with recent onset of neck pain and concomitant complaints of vertigo or dizziness, but normal findings at otoneurological examination and electronyst

Adaptive mechanisms of VOR compensation after unilateral peripheral vestibular lesions in humans.

To further elucidate possible central plastic adaptive processes during the recovery from a unilateral peripheral vestibular lesion, we investigated vestibular functions in humans over a period of 2 months after an acute unilateral labyrinthine lesion. A unilateral peripheral vestibular lesion creates both a tonic imbalance that causes spontaneous nystagmus and a decrease and directional asymmetry of dynamic vestibular responses. We establish that the tonic imbalance expressed by the spontaneous nystagmus rapidly decreased (similar to other species), whether the lesion remained complete or not

Peripheral and central vestibular disorders in alcoholics. A three-year follow-up study.

Vestibular disorders were found in all 33 Japanese men who had been drinking alcohol for over 20 years and who had abstained from alcohol for over 3 months before the tests were done. Six, 12, and 15 patients showed peripheral, central, and combined types of vestibular disorders, respectively. The peripheral disorder occurred in younger alcoholics (under 50 years old) with a drinking history of 20-30 years, and the combined disorder in older patients with a longer drinking history. The vestibular disorders, however, were not observed in any of 16 control non-alcoholics. A follow-up study on se

Vestibular ocular reflexes in Ménière's disease patients evaluated by passive high frequency head rotation (yaw) and sidewards acceleration.

Despite the fact that vertigo is the most apparent complaint at the onset of Ménière's disease, standard vestibular examination seldom shows vestibular function loss in the early phase. In this study, three relatively advanced diagnostic tests (passive high frequency head shaking, passive head impulse test and sidewards linear acceleration) were applied to trace vestibular function loss in the early phase in 25 patients with possible Ménière's disease (American Academy of Otolaryngology-Head and Neck Surgery 1995 criteria); one test (passive high frequency head shaking) was also conducted in 2

[Ocular counterrolling as an indicator of vestibular otolith function].

Space Motion Sickness (SMS) occurred in the first few days of space flight correlated with the vestibular function. As the main influence of weightlessness in space focused on the otolith receptors. The test of otolith function is of great importance. Although there are a lot of methods to test otolith function, at present ocular counterrolling is the only relative pure indicator of otolith function. It provide a valid method for predicting SMS susceptibility, it has great application prospect in the research of the SMS mechanism and the readaptability of vestibular function after space flight

[Vertigo and oculomotor disorders: diagnosis, anatomical classification and etiology].

Dizziness and vertigo are frequent leading symptoms also in the ophthalmological practice and hospitals. They require a structured clinically oriented diagnostic approach. This article provides a practical approach based on a focused history taking using four key questions as well as a systematic examination of ocular motility (four static and four dynamic tests) and four tests assessing the vestibular system. The focus is on differentiating peripheral from central causes through structured history taking and clinical examination. In addition, the functional anatomy of the brainstem and cerebe

[Quantifying the stimulus parameters in the neck torsion test].

Cervically induced eye movements can be observed when the trunk is rotated and the head fixed. In most test situations, rotating chair motion and head fixation are carried out manually. As a result, specific and non-specific sources of error are involved in these procedures that hamper the reproducibility of test conditions. A system is described that provides quantifiable and reproducible, electronically controlled movements of the rotating chair. Ramp and rectangular stimulus profiles are achieved by infrared optical gate devices on the chair axis. Various processor-controlled sequences of a

[Current computerized support for vestibular function tests.I. Expert systems and their clinical applications].

Expert Systems are a new method developed by the branch of Computer Science known as Artificial Intelligence in order to make available the knowledge and the expertise of the specialists in a certain domain of the science to other operators in the same field. Most of their applications belong to the medical domain. In this paper the main features of the expert systems are briefly described. As an example the structure and the characteristics of the shell EXPERT (Weiss and Kulikowski, 1979) are presented in detail. This shell has already been used to develop several expert systems. It is the to

[Statistical study of gaze displacement calibration].

The value of quantitative nystagmus analyses depends strongly on the accuracy of the calibration of gaze displacements. Therefore, 120 nystagmograms with one or two controls of the first calibrations (total: 300 calibrations) were evaluated to get a view over the characteristic properties, mainly the variations, of the calibration factors. Systematic and stochastic deviations were taken into account as potential sources of errors. These are mainly ascribed to variations of the corneo-retinal potential and inaccurate visual angles, e.g. as a result of head movements. Statistic evaluation shows
