Use of the surgical Apgar score to enhance Veterans Affairs Surgical Quality Improvement Program surgical risk assessment in veterans undergoing major intra-abdominal surgery.

BACKGROUND: We investigated whether the surgical Apgar score (SAS) may enhance the Veterans Affairs Surgical Quality Improvement Program (VASQIP) risk assessment for prediction of early postoperative outcomes. METHODS: We retrospectively evaluated demographics, medical history, procedure, SAS, VASQIP assessment, and postoperative data for patients undergoing major/extensive intra-abdominal surgery at the Manhattan Veterans Affairs between October 2006 and September 2011. End points were overall morbidity and 30-, 60- , and 90-day mortality. Pearson's chi-square, ANOVA, and multivariate regres…

Mental Health Impact of Homecoming Experience Among 1730 Formerly Deployed Veterans From the Vietnam War to Current Conflicts: Results From the Veterans' Health Study.

We examined the effects of homecoming support on current mental health among 1730 deployed veterans from Vietnam, Iraq/Afghanistan, Persian Gulf, and other conflicts. The prevalence of current posttraumatic stress disorder (PTSD) was 5.4%, current depression was 8.3%, and 5.4% had suicidal thoughts in the past month. Overall, 26% of veterans had low homecoming support, which was more prevalent among Vietnam veterans (44.3%, p < 0.001). In multivariable logistic regressions, controlling for demographics, combat exposure, number of deployments, trauma history, and operational theater, low postde…

Geographical diffusion of prazosin across Veterans Health Administration: Examination of regional variation in daily dosing and quality indicators among veterans with posttraumatic stress disorder.

Posttraumatic stress disorder (PTSD) is a high-priority treatment area for the Veterans Health Administration (VHA), and dissemination patterns of innovative, efficacious therapies can inform areas for potential improvement of diffusion efforts and quality prescribing. In this study, we replicated a prior examination of the period prevalence of prazosin use as a function of distance from Puget Sound, Washington, where prazosin was first tested as an effective treatment for PTSD and where prazosin use was previously shown to be much greater than in other parts of the United States. We tested th…

Perspectives of Older Veterans on Aging in Permanent Supportive Housing.

Aging in place-the ability to live comfortably and safely in one's own home and community as one ages-is a priority for many older adults and a growing focus of research and policy efforts. Little is known about perspectives on aging in place among the growing population of older Veterans with experiences of homelessness. Data from qualitative interviews with 21 formerly homeless Veterans ages 50 and older engaged in U.S. Department of Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) Program were analyzed using rapid qualitative analysis. Analysis revealed six inter…

Housing Status and Receipt of Unplanned Operation in US Veterans.

BACKGROUND: People experiencing homelessness experience a disproportionately high rate of unplanned operations, suggesting barriers to timely elective surgical care. Whether these disparities persist within the Veterans Health Administration, which prioritizes supportive services for Veterans experiencing homelessness, remains unclear. STUDY DESIGN: We conducted a retrospective cohort study of Veterans undergoing abdominal aortic aneurysm repair, incisional hernia repair, or colectomy for access-sensitive surgical conditions between 2012 and 2019. Housing status was determined using multiple …

The Development and Implementation of a Veteran Family Stress Screening Tool.

Background/Objectives: Military and Veteran families face unique challenges, including deployment-related difficulties and transitions, mental health issues, educational setbacks, and more. The needs of Veteran families, specifically, are often overlooked and research among this population is insufficient. In an effort to promote the well-being of children of Veterans and their families, we developed a screening tool for identifying family needs and a workflow to implement the screening tool within the Veterans Health Administration. The objective of this quality improvement initiative was to …

Embedding Lived Expertise into Military and Veteran Medical Research.

Military and Veterans health research has substantially advanced in recent decades, particularly in terms of analytic sophistication aimed at characterizing the biopsychosocial complexity of health through biomarkers, neuroimaging, and predictive modeling. However, formal models (e.g., governance structures) for including individuals with lived experiences to guide this work have not evolved at the same pace. Drawing on our collective perspectives as Veterans, Caregivers, and academic partners, this commentary examines the structural limitations of current engagement processes. Although commun…
