OBJECTIVE: To characterize a cohort of children with airflow limitation resistant to bronchodilator (BD) therapy.
METHODS: Pulmonary function tests performed in children 6-17 years of age at 15 centers in a clinical research consortium were screened for resistant airflow limitation, defined as a post-BD FEV1 and/or an FEV1/FVC less than the lower limits of normal. Demographic and clinical data were analyzed for associations with pulmonary function.

The effect of closed mitral valvotomy on the spirometric pulmonary functions was studied in 25 patients with mitral stenosis. We describe in six men, recurrent episodes recurring over months or years, of sudden, brief complete obstruction to respiration followed by dyspnoea with loud inspiratory stridor lasting two to five minutes. Attacks occurred during wakefulness and/or sleep. In one patient an episode was witnessed endoscopically: the initial obstruction was seen to be caused by complete laryngeal closure. To determine whether normal nonsmoking individuals aged 55 years or greater have heightened bronchial reactivity to inhaled SO2, ten male subjects, 55 to 73 years of age, were exposed for 20 min at rest and 10 min during moderate exercise on a treadmill to the following: NaCl droplet aerosol, or 1.0 ppm of SO2 and NaCl droplet aerosol. Acetylsalicylic acid (ASA) and other nonsteroidal anti-inflammatory drugs (NSAID) cause a variety of symptoms in patients sensitive to these drugs. These include wheezing, rhinorrhea, flushing, pruritus, urticaria, hypotension and loss of consciousness. Conversely, improvement of asthma with the use of these drugs in patients who do not have idiosyncratic reactions to ASA (ASA-nonsensitive) has also been observed both with respect to clinical symptoms and pulmonary function tests.

During a short period of increased air pollution in a polluted area in the Netherlands, a majority of the inhabitants showed a lower VC and FEV1 value than during a period of extremely low pollution.
