BACKGROUND: Decompensated hypothyroidism, formerly known as myxedema coma, is an endocrine emergency that commonly presents with altered mental status, as well as hypothermia and depressed vital signs. The condition is often caused by an inciting event, which may lead to significant delays in the diagnosis and management of this disease. Although the incidence is low, this disease is associated with significant morbidity and mortality. Therefore, it is important for emergency clinicians to be aware of this condition.
OBJECTIVE: This narrative review evaluates the emergency medicine diagnosis …

BACKGROUND: Monitoring disease progression in childhood interstitial lung diseases (chILD) is essential. No information for the minimal important difference (MID), which is defined as the smallest change in a parameter that is perceived as important prompting a clinician to change the treatment, is available. We calculated MIDs for vital signs (respiratory rate, peripheral oxygen saturation in room air, Fan severity score) and health-related quality of life (HrQoL) scores.
METHODS: This study used data from the Kids Lung Register, which is a web-based management platform that collects data of…

OBJECTIVE: To develop and validate heart and respiratory rate percentile curves for hospitalized children and compare their vital sign distr … bservations in our sample would have resulted in increased EWSs.
CONCLUSIONS: A high proportion of vital signs among hospitalized children would be considered out of range according to existing reference ranges and pediatric EWSs. The percentiles we derived may serve as useful references for clinicians and could be used to inform the development of evidence- …

INTRODUCTION: Orthopedic surgeries are commonly associated with significant postoperative pain and physiological stress, which can be objectively identified through alterations in vital signs, such as blood pressure and heart rate. During postoperative bedside care, procedures that involve patient handling and mobilization may intensify pain-related physiological responses, highlighting the need for interventions capable of modulating both pain perception and vital signs in this clinical context.
OBJECTIVE: To evaluate the effect of music as an intervention on pain and vital signs during beds…

INTRODUCTION: Coronavirus disease 2019 (COVID-19) can be a life-threatening lung disease or a trivial upper respiratory infection depending on whether the alveoli are involved. Emergency department (ED) evaluation of symptomatic patients with normal vital signs is frequently limited to chest auscultation and oro-nasopharyngeal swabs. We tested the null hypothesis that patients being screened for COVID-19 in the ED with normal vital signs and without hypoxia would have a point-of-care lung ultrasound (LUS) consistent with COVID-19 less than 2% of the time.
METHODS: We performed a retrospective…

Today's state of the art in exercise physiology, professional athletics and sports practice in general clearly shows that the best results depend on the personalisation and continuous update of the recommendations provided to an athlete training, a sports lover or a person whose medical condition demands regular physical exercise. The vital signs information gathered in telemonitoring systems can be better evaluated and exploited if processed along with data from the subject's electronic health records, training history and performance statistics. In this context, the current paper intends to …

Research has observed increased mortality among older people attending the emergency department (ED) who had systolic pressure > 7 mmHg lower than baseline primary care values. This study aimed to (1) assess feasibility of identifying this 'relative hypotension' using readily available ED data, (2) externally validate the 7 mmHg threshold, and (3) refine a threshold for clinically important relative hypotension. A single-centre retrospective cohort study linked year 2019 data for ED attendances by people aged over 64 to hospital discharge vital signs within the previous 18 months. Frailty and …

BACKGROUND: Elderly patients have a blunted host response, which may influence vital signs and clinical outcomes of sepsis. This study was aimed to investigate whether the associations between the vital signs and mortality are different in elderly and non-elderly patients with sepsis.
METHODS: This was a retrospective observational study. A Japanese multicenter sepsis cohort (FORECAST, n = 1148) was used for the discovery analyses. Significant discovery results were tested for replication using two validation cohorts of sepsis (JAAMSR, Japan, n = 624; SPH, Canada, n = 1004). Patients were cat…

BACKGROUND: The annual incidence of acute pancreatitis is approximately 30 per 100,000, with 20% progressing to severe acute pancreatitis and a mortality rate of 20%-40%. Traditional scoring models suffer from data lag or insufficient accuracy, while existing machine learning models mostly overlook the dynamic characteristics of vital signs. METHODS: Vital signs , laboratory and imaging indices within 24 hours of admission were collected. First, a bidirectional long short-term memory network model was constructed using time-series data . Then, key indices from laboratory and imaging data were s…

PURPOSE: To use machine learning to predict new-onset shock for at-risk intensive care unit (ICU) patients based on discrete vital sign data from the electronic health record.
METHODS AND RESULTS: We included 11,305 adult cardiac, medical, neurological, and surgical ICU patients who did not have shock within 4 h of ICU admission. We used routine vital sign data collected from the first 4 h of the ICU stay to predict new-onset shock within the subsequent 4 h. We compared logistic regression with machine learning models including elastic net, random forest, boosted trees and extreme gradient bo

Serotonergic mechanisms have an important function in the central control of circulation. Here, the acute effects of three selective serotonin (5-HT) reuptake inhibitors (SSRIs) on autonomic and cardiorespiratory variables were measured in rats. Although SSRIs require 2-3 weeks to achieve their full antidepressant effects, it has been shown that they cause an immediate inhibition of 5-HT reuptake. Seventy male Wistar rats were anesthetized with urethane and instrumented to record blood pressure, heart rate, renal sympathetic nerve activity (RSNA), and respiratory frequency. At lower doses, the

BACKGROUND: Many studies have evaluated the use of wearable monitoring systems to improve patient safety in hospital. Although some have demonstrated effects on intensive care admissions, there remains little evidence of impact on patient outcomes such as mortality, hospital length of stay, and time to antibiotic administration. Very few studies have focused on how wearable monitoring systems are used in clinical practice, including how the rate of manual vital sign measurements (MVSMs) is affected.
OBJECTIVE: Our primary aim was to describe the physiological pattern of vital signs in hospita…

This paper is concerned with predicting the occurrence of periventricular leukomalacia (PVL) using vital and blood gas data which are collected over a period of 12 h after the neonatal cardiac surgery. A data mining approach has been employed to generate a set of rules for classification of subjects as healthy or PVL affected. In view of the fact that blood gas and vital data have different sampling rates, in this study we have divided the data into two categories: 1) high resolution (vital), and 2) low resolution (blood gas), and designed a separate classifier based on each data category. The

BACKGROUND: Helium-oxygen mixture (heliox) ventilation has been known as an alternative treatment in patients with airway obstruction. Because of the physical properties of heliox, mechanical ventilation with this gas mixture may offer advantages in the management of respiratory failure associated with meconium aspiration syndrome (MAS).
OBJECTIVES: The purpose of this pilot study was to assess the effect of short-term mechanical ventilation with heliox in newborns with MAS on vital signs, oxygenation, acid-base balance and respiratory function parameters.
METHODS: The study was carried out …

Meningitis is a disease feared by parents and health professionals alike. The rapid onset and progression of the disease and potentially let … The emphasis should therefore be on regular, close monitoring of an ill child and assessment of the vital signs. Awareness of the recognised "red flag" symptoms of septicaemia--cold hands and feet, limb pain and pale or mottled skin--could also aid earlier diagnosis and hence potentially improve prognosis.
