Potent inhibition of cicatricial contraction in proliferative vitreoretinal diseases by statins.

OBJECTIVE: Despite tremendous progress in vitreoretinal surgery, certain postsurgical complications limit the success in the treatment of proliferative vitreoretinal diseases (PVDs), such as proliferative diabetic retinopathy (PDR) and proliferative vitreoretinopathy (PVR). One of the most significant complications is the cicatricial contraction of proliferative membranes, resulting in tractional retinal detachment and severe vision loss. Novel pharmaceutical approaches are thus urgently needed for the management of these vision-threatening diseases. In the current study, we investigated the i…

Expansion of long-acting gas bubbles for intraocular use. Principles and practice.

Expansion of long-acting gas bubbles used in vitreoretinal surgery may lead to increased intraocular pressure and central retinal artery occlusion. To further understand the principles controlling the expansion of long-acting gases, we compared the expansion of perfluoropropane gas bubbles with that of sulfur hexafluoride gas bubbles in an in vitro model, as well as in rabbits. We found that perfluoropropane bubbles expand more rapidly and to a greater extent than do sulfur hexafluoride bubbles in the first 24 hours. The early rate of expansion of a long-acting gas is largely dependent on conv…

Results of surgery for ocular Toxocara canis.

It has been shown that the diagnosis of ocular Toxocara canis is highly accurate when the typical clinical findings are associated with a positive ELISA titer in the serum of 1:8 or greater. The inflammatory reaction in this disorder can be devastating and not infrequently leads to a tractional detachment of the posterior pole or a rhegmatogenous detachment and/or a progressive cyclitic membrane leading to phthisis bulbi. We have recently performed vitreoretinal surgery on 17 patients with complications secondary to ocular T canis. Fifteen patients had stability or improvement in visual acuity…

Lens capsular flap in the management of posterior retinal hole associated retinal detachment in high myopic eyes with previous internal limiting membrane peeling: 3 case reports.

RATIONALE: Managing retinal detachment due to posterior retinal holes is problematic since standard laser retinopexy or scleral buckling may be difficult to apply and may have brought serious complication. Another surgical method in treating posterior hole related retinal detachment is desired. PATIENT CONCERNS: Three high myopia patients with previous vitrectomy and membrane peeling history suffered from blurred vision and retinal detachment due to posterior pole retinal holes was diagnosed. DIAGNOSES: Patient diagnosed retinal detachment due to posterior retinal holes either as paracentral

[Childhood retinal detachment: ROP and myopia].

This article reviews the cause and treatment options for retinopathy of prematurity (ROP)-related tractional detachment and detachments in ROP residuals during childhood and early adulthood. Retinal vascularisation is incomplete after premature birth. Phase I of ROP consists of a delayed retinal vascular growth and vessel loss after premature birth resulting in hypoxia, phase II results in hypoxia-induced vascular proliferation and as a consequence to vitreoretinal traction. The anatomic and functional outcome of tractional detachment in ROP is determined by the previous treatment (e.g., laser

Case Report: Long-term visual rehabilitation in extreme early-onset axial myopia complicated by retinal detachment.

Extreme early-onset axial myopia causes profound refractive disability and lifelong vitreoretinal vulnerability. We report approximately 20 years of follow-up in a woman with bilateral myopia of -8.00 diopters (D) at 1 year of age, axial lengths exceeding 30 mm at 6 years and 35 mm before adulthood. At 18 years, severe spectacle-related minification, peripheral distortion, and inconvenience persisted. After discussion of contact lenses, phakic intraocular lenses, and refractive lens exchange, she chose sequential bilateral refractive lens exchange with toric intraocular lenses and a residual m

A paradigm shift in retinal detachment repair: The concept of integrity.

The management of rhegmatogenous retinal detachment has rapidly evolved over recent decades. A range of surgical techniques exist, all of which can achieve retinal reattachment in most cases. In recent years there have also been vast technical advances in retinal imaging that have introduced novel ways of visualizing and studying the retinal macro and microstructural anatomy following retinal detachment repair. Recent clinical trial data demonstrates that functional and patient-reported outcomes of retinal reattachment differ with surgical technique, accompanied by differences in anatomic biom

[Capillary hemangioma of the retina in cases of von Hippel-Lindau syndrome. New therapeutic directions].

Thermal photocoagulation of small peripheral angiomas is the treatment of choice for capillary hemangiomas in patients with von Hippel-Linda … onsidered as experimental. The treatment of juxtapapillary angiomas is still a therapeutic dilemma. Vitreoretinal surgery should be confined to advanced stages with tractional detachment or when no other treatment option is available to salvage the eye.

[Diagnostic and therapeutic options in diabetic retinopathy].

Diabetes mellitus is the systemic disease that most often leads to blindness. Since the diminishment of visual acuity is a late symptom of the disease, screening examinations are of particular importance, as only in this way can the optimal time point for treatment be determined. Stage-oriented laser therapy prevents blindness due to macular edema or proliferative diabetic retinopathy. For a number of years, vitreoretinal surgery has enabled the treatment of late ocular manifestations such as bleeding into the vitreous body and traction retinal detachment. With appropriate stage-oriented treat…

Reattachment of retina and retinoschisis in pit-macular syndrome by surgically-induced vitreous detachment and gas tamponade.

The origin of the subretinal fluid in pit-macular syndrome is unknown. Using optical coherence tomography, the authors observed that an optic disk pit is not a true pit but a cystic cavity covered with a superficial layer of the optic disk in a patient with retinal detachment and retinoschisis. The cyst was connected to the intraretinal space of the retina, which was split in several layers, or the retinoschisis in the papillomacular area. Vitrectomy and gas tamponade were performed to treat the patient. During surgically-induced vitreous detachment, strong vitreoretinal attachment at the disk
