Routine use of mebendazole in pregnancy.

INTRODUCTION: The Ministry of Health has recommended the routine administration of a broad spectrum anthelmintic to all pregnant women after completion of the first trimester of pregnancy. OBJECTIVE: To estimate prevalence and intensity of geohelminth infections in pregnant women attending an antenatal clinic in Ragama and determine the use of anthelmintics by them. METHODS: Women on their first visit to antenatal clinics of the University Obstetrics Unit, General Hospital Colombo North, Ragama, during July-August 1995, were recruited for the study. Demographic details, duration of pregnancy

Health education to promote community involvement in the control of tropical diseases.

Health Education is any combination of learning activities that promote voluntary adaptations in health-related behaviour. In pursuing this definition, health workers generally work in two main directions, a community development approach and a behavioristic, communications approach. While Health Education in tropical, developing countries often takes the developmental approach with full community involvement, including the setting of programme priorities, Health Education for disease control has more often been characterized as the behavioristic mode utilizing simplistic, professionally deter…

[Epidemiology of sexually transmitted diseases, with the exception of AIDS].

Epidemiologic trends in sexually transmitted diseases strongly vary in the world. The situation is worrying in the developing countries. Both the incidences and prevalences are very high antibiotic resistance is expanding and interactions between sexually transmitted diseases and human immunodeficiency virus contribute to AIDS epidemic. In western Europe, the decline of some sexually transmitted diseases is likely to be linked with successful control programmes and gonorrhea and syphilis now are rare diseases. The USA situation is contrasted. Some inner-city minority population still have high…

Helicobacter pylori in duodenal and gastric ulcer disease.

The decision to treat a patient should in general always be based on potential risk and advantage. Widespread and uncontrolled use of all kinds of anti-H. pylori regimens may promote development of antimicrobial resistant strains. In particular, antimicrobial monotherapy is associated with failure to eradicate H. pylori and induction of resistant strains. Polychemotherapy is much more effective and has a lower risk for development of antimicrobial resistant H. pylori strains but carries the risk of significant drug-related side effects. If the prescribed anti-H. pylori regimen is not effective

Prevention of cardiovascular disease: from biomedical research to health policy.

Cardiovascular diseases (CVD) are the leading cause of premature death and disability in the developed world. Broad consensus exists on CVD preventability through reduction of their risk factors at both the individual and population level. The latter kind of intervention implies involvement of policy-making institutions, owing to the manifold implications (agriculture, industry, environment) of such programmes. They have to be developed through three phases in succession: observational studies; intervention trials; public health action programmes. The implementation of the latter can only resu

Quantitative interpretation of coprocultures in a population infected with Oesophagostomum bifurcum.

Coproculture is used in northern Togo and Ghana in the research on Oesophagostomum bifurcum, a common parasite of man in these regions. Prior to a follow-up study to investigate patterns of reinfection in Oesophagostomum and hookworm, it was attempted to evaluate the relevance of counting larvae for the assessment of the intensity of infection at the population level. Of 102 samples, one egg count (Kato-smear) and three coprocultures were carried out. Frequency distributions of counts of larvae of Oesophagostomum and of hookworm isolated in three coprocultures, showed log-normality. There was

The distribution of kwashiorkor in the southern region of Malawi.

Using existing nutritional survey data collected over an 11-month period during the recent drought, we sought to determine the distribution of kwashiorkor in the ten districts of the Southern Region of Malawi. Sampling and survey methods were identical and considerable efforts were made to ensure comparability. In 23 surveys, 25,824 children were weighed and measured and oedema was recorded. The district-adjusted prevalence of kwashiorkor was 18/1000. The northernmost districts of the Southern Region had rates five to ten times higher than the southernmost districts. Prevalence peaked at 18-23

Tubercular lymphadenitis: clinical manifestations.

One hundred and thirteen children suffering from tubercular lymphadenitis proven histopathologically, were studied for clinical and laboratory features. Age distribution was equal in all age groups except during infancy when it was rare. Sex ratio indicates a male preponderance with male to female in the ratio of 1.5:1 (67:46). Family history of contact with known tuberculous patient was positive in 19 (17%) children, 44 (40%) had received BCG, and 98 (88%) were either normal nourished or had mild malnutrition. Cervical, axillary and inguinal nodes were found in 90 (80%), 14 (12%), and 8 (7%)

[Etiology of secondary glaucoma at university clinics in Munich and Nairobi].

Glaucoma is one of the most common blinding diseases worldwide. For early detection and treatment of this insidious ailment investigations of risk factors are necessary, most of which require extensive technically demanding and time consuming procedures. Basic information on risk factors for the development of glaucomatous damage is essential for diagnosis of glaucoma, for the identification of groups at risk, and for the planning of prevention and intervention programs. Epidemiological data on secondary glaucomas is especially scarce, while at the same time the strict distinction between prim

[Transmission of poliomyelitis by drinking water and the problem of prevention].

The role of drinking water as a source of infection with wild poliovirus in modern cities was denied nearly 50 years ago on the basis of arguments taken from bacteriology. Recent findings concerning the persistence of viruses in water and their resistance to antibacterial agents applied to fresh water and waste water require a revision of those older concepts. There is now convincing evidence that in New York City, where poliomyelitis struck with extreme severity, drinking water was a main source of infection. The phenomenon of water-borne poliomyelitis may become important in the near future

Eye disease in newly diagnosed leprosy patients in eastern Nepal.

To determine the magnitude of eye lesions in newly diagnosed leprosy patients we examined their eyes. The Eastern Leprosy Control Project was supported by The Netherlands Leprosy Relief Association; we used the regional clinic in Biratnagar and 5 mobile clinics in surrounding districts as our survey area. All patients who presented at the clinics over 10 weeks, diagnosed as having untreated leprosy were included. Of the 260 examined patients 97 (37.3%, 95% confidence interval 28.3-40.3%) had an eye lesion; 12/260 patients (4.6%, 95% confidence interval 2.0-7.2%) had sight-threatening lesions (

Hypovitaminosis A: international programmatic issues.

The virtual elimination of vitamin A deficiency and all its consequences is high on the political agenda as a solvable public health problem by the end of the decade. Five to six times more children in the developing world are likely to be subclinically than clinically deficient. Subclinical deficiency can be detected by newer methodological approaches. Subclinically deficient children are at increased risk of severe and fatal infections. The problem at a population level is avoidable by the appropriate selection and application of a mix of available interventions. Countries are challenged to
