A scintigraphic classification of Legg-Calvé-Perthes disease.

The onset of Legg-Calvé-Perthes (LCP) disease, avascular necrosis of the proximal femoral epiphysis, frequently is clinically undetectable for months before presentation. This traditional X-ray methods of diagnosis are insensitive for the diagnosis of early LCP as well as nonspecific for the diagnosis of advanced LCP. The X-ray may remain normal for weeks to months after the onset of LCP, and other disorders (steroid osteopathy, epiphyseal dysplasia) simulate the X-ray appearance of LCP during its later stages. Localization of bone-avid radiopharmaceuticals depends principally on bone perfusio

[Spasm of the adductor muscles, pre-dislocations and dislocations of the hip joints in children and adolescents with cerebral palsy. Clinical observations on aetiology, pathogenesis, therapy and rehabilitation. Part II. The importance of the iliopsoas tendon, its tenotomy, of the coxa valga antetorta, and correction through osteotomy turning the hip into varus (author's transl)].

The following factors besides spasm and contraction of the adductor muscles contribute to the occurrence of dislocations of the hip in spastic paralysis: Spasm and contraction of the iliopsoas muscle and enhanced valgus position and antetorsion. The author holds the opinion that in case of malformation of the proximal end of the femur, it is not only the indirect action of the spastic musculature via the proximal femur-epiphyseal cartilage which is responsible for this phenomen in accordance with the law on functional adaption through longitudinal growth (Pauwels), but also the direct traction

What Is the Prevalence of Cam Deformity After Prophylactic Pinning of the Contralateral Asymptomatic Hip in Unilateral Slipped Capital Femoral Epiphysis? A 10-year Minimum Followup Study.

BACKGROUND: Prophylactic pinning of the asymptomatic and normal-appearing contralateral hip in patients with unilateral slipped capital femoral epiphysis (SCFE) remains controversial. Understanding the minimal 10-year clinical, functional, and radiographic outcomes of the contralateral asymptomatic hip in unilateral SCFE may be helpful in the decision regarding whether the benefits associated with potentially preventing a SCFE are outweighed by the risk of additional surgery. QUESTIONS/PURPOSES: Among patients with SCFE treated with prophylactic pinning of the asymptomatic and contralateral h

Comparison of Growth Plate Violations for Transtibial and Anteromedial Surgical Techniques in Simulated Adolescent Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Increases in the rates and intensity of youth sports participation have led to an elevated incidence of anterior cruciate ligament (ACL) injuries and reconstruction in adolescents. Traditional reconstruction techniques in the skeletally immature patient may violate the growth plates and potentially lead to deformities. HYPOTHESIS/PURPOSE: The purpose of this study was to compare the volume and location of femoral growth plate violations resulting from anteromedial (AM) and transtibial (TT) techniques in ACL reconstruction. The hypothesis was that the more oblique angle used in fem

[APPLICATION OF BUTTERFLY SHAPED LOCKING COMPRESSION PLATE IN COMPLEX DISTAL RADIUS FRACTURES].

OBJECTIVE: To investigate the effectiveness of butterfly shaped locking compression plate for the treatment of complex distal radius fractures. METHODS: Between June 2011 and January 2013, 20 cases of complex distal radius fractures were treated with butterfly shaped locking compression plate fixation. There were 11 males and 9 females with an average age of 54 years (range, 25-75 years). Injury was caused by falling in 10 cases, by traffic accident in 7 cases, and by falling from height in 3 cases. All of fractures were closed. According to AO classification system, there were 8 cases of typ

Postoperative improvement of femoroacetabular impingement after intertrochanteric flexion osteotomy for SCFE.

BACKGROUND: Patients with slipped capital femoral epiphysis (SCFE) may develop cam-type femoroacetabular impingement (FAI). Early management of FAI has been advocated for patients with symptomatic FAI. The various treatment options, including reorientation surgeries, realignment procedures, and osteoplasty, remain controversial. QUESTIONS/PURPOSES: We asked whether an intertrochanteric flexion osteotomy improved the clinical symptoms of FAI in patients with SCFE and confirmed whether the radiographic signs were compatible with the clinical signs of FAI. METHODS: We retrospectively reviewed 3

Epiphysis preserving resection of malignant proximal tibial tumours.

PURPOSE: The purpose of the study was to evaluate the treatment of five patients with proximal tibial epiphysis preserving resection for malignant tumours of the tibia. METHOD: Three patients suffered from Ewing sarcoma, two had osteosarcoma. The proximal level of the resection was in the line of the growth plate in four patients, and 2 cm below of the growth plate in one patient. The distal resection level was in the diaphysis, depending on the tumour border. The resected part of the tibia was substituted by both the ipsilateral and contralateral fibulas. The two fibulas were proximally buil

Silent slipped capital femoral epiphysis in overweight and obese children and adolescents.

Abnormal loading of the hip in obese children may lead to anatomic alterations and an increased prevalence of slipped capital femoral epiphysis (SCFE). The aims of this study were to examine the hip motion in obese children and adolescents and to estimate the prevalence of SCFE in a subgroup of patients characterized by pathological clinical examination and/or pain in the knee or hip joint. A total of 411 individuals (196 males), mean age 14.5 ± 2.5 years (7.8-20.4), mean BMI of 32.9 ± 5.6 kg/m(2) (20.3-51.5, z score +2.65) who were consecutively admitted for an inpatient weight loss program w

Growth plate disturbance after transphyseal reconstruction of the anterior cruciate ligament in skeletally immature adolescent patients: an MR imaging study.

BACKGROUND: There are concerns of potential growth disturbance after transphyseal reconstruction of the anterior cruciate ligament in skeletally immature patients. The authors used magnetic resonance (MR) imaging to evaluate growth disturbance and associated physeal abnormalities after index surgery. METHODS: We retrospectively reviewed the follow-up MR imaging studies of 43 patients who underwent transphyseal reconstruction of the anterior cruciate ligament using a soft-tissue graft at the mean age of 14.8 years (range, 12.4 to 16.5 y). Mean time from surgery to follow-up MR imaging was 16 m

Wolcott-Rallison syndrome.

Wolcott-Rallison syndrome (WRS) is a rare autosomal recessive disease, characterized by neonatal/early-onset non-autoimmune insulin-requiring diabetes associated with skeletal dysplasia and growth retardation. Fewer than 60 cases have been described in the literature, although WRS is now recognised as the most frequent cause of neonatal/early-onset diabetes in patients with consanguineous parents. Typically, diabetes occurs before six months of age, and skeletal dysplasia is diagnosed within the first year or two of life. Other manifestations vary between patients in their nature and severity

[Comparison of the treatment of slipped capital femoral epiphysis with K-wires and cannulated titanium screws].

AIM: The treatment of slipped capital femoral epiphysis (SCFE) is usually treated operatively, but there is still no consensus about the method to be used. Up to a 30 degrees degree of slipping, the epiphysis is normally fixed in situ. The aim of our study was to compare the intermediate results after fixation in situ by K-wires versus cannulated titanium screws (Königsee-Implantate, Königsee-Aschau, Germany). METHODS: In this study 46 patients with SCFE grade I and II and mostly chronic slipping of the epiphysis were included. After fixation in situ and, if necessary, careful, closed reposit

Evolving concepts in the management of the bone gap in the upper limb. Long and small defects.

Vascularised bone graft is a well accepted technique when dealing with long defects. Its role in refractory nonunion, in small defects and in the growing patient is rarely discussed. In this paper the authors review the different alternatives to deal with bone defects in the upper extremity. The indications of vascularised corticoperiosteal graft for solving small defects harbouring refractory nonunion, and the use of vascularised bone phalanx and metatarsal for complex - but small - defects in the fingers is presented. The ability of the bone to grow and remodel when a living epiphysis is inc

Radiological evidence of femoroacetabular impingement in mild slipped capital femoral epiphysis: a mean follow-up of 14.4 years after pinning in situ.

Conventional treatment of mild slipped capital femoral epiphysis consists of fixation in situ with wires or screws. Recent contributions to the literature suggest that even a mild slip may lead to early damage of the acetabular labrum and adjacent cartilage by abutment of a prominent femoral metaphysis. It has been suggested that the appropriate treatment in mild slipped capital femoral epiphysis should not only prevent further slipping of the epiphysis, but also address potential femoroacetabular impingement by restoring the anatomy of the proximal femur. Between October 1984 and December 199

Results of transphyseal anterior cruciate ligament reconstruction using patellar tendon autograft in tanner stage 3 or 4 adolescents with clearly open growth plates.

BACKGROUND: Drilling across the physes for intra-articular anterior cruciate ligament reconstruction is considered risky for skeletally immature patients. HYPOTHESIS: Skeletally immature patients with clearly open growth plates can safely undergo intra-articular anterior cruciate ligament reconstruction with patellar tendon autograft without suffering growth plate disturbance. STUDY DESIGN: Retrospective review of prospectively collected data. METHODS: Surgery involved drilling tunnels through the tibial and femoral physes, the bone plugs were placed proximal to the physes, and button fixat

The assessment of contact stress in the hip joint after operative treatment for severe slipped capital femoral epiphysis.

We determined contact stress on the articular surface of the hip joint in a group of patients who underwent operative treatment for severe slipped capital femoral epiphysis. Two different procedures were considered: the modified osteotomy of Dunn-Fish and the osteotomy of Imhäuser. In order to determine the stress distribution, a three-dimensional mathematical model was used taking into account the geometrical parameters of the pelvis and hip, which were measured from standard antero-posterior radiographs. We found that the Dunn-Fish procedure produced lower peak stress than the Imhäuser proce

[Pain and gait problems in 3 (almost) adolescents with a dislocated hip].

Three young people, a boy aged 15 years and two girls aged 9 years and 13 years, had already suffered groin pain and knee pain for many months. They exhibited an antalgic walk, diminished function of the hip and radiographic signs of a slipped capital femoral epiphysis (SCFE). The complaints disappeared following in situ fixation with one or more screws. Epiphysiolysis of the caput femoris through the growth plate is a disorder of the growing hip. It is the most prevalent hip disease in adolescents. Obese boys are the most commonly affected. The aetiology is not known. The later SCFE is diagno

Patient self-assessment of tibial plateau fractures in 40 older adults.

This retrospective study examined the outcome of 40 patients 50 or more years of age treated between September 1988 and September 1993 for tibial plateau fractures. Our analysis was based on a review of the medical records of all patients as well a patient self-assessment questionnaire. The 22 women and 18 men studied had a mean age of 60 (range, 50 to 76 years) at the time of injury. A satisfactory overall outcome (based on patients' self-assessment) was observed in only 14 (35%) of the 40 cases. Unsatisfactory outcomes were observed in 23 (72%) of 32 cases treated operatively and 3 (38%) of

Slipped capital femoral epiphysis associated with endocrine disorders.

We reviewed 85 patients with endocrine disorders and slipped capital femoral epiphysis (SCFE). The disorders were hypothyroidism (40%), growth hormone deficiency (25%), and others (35%). The average age at diagnosis of the disorder was 13.2 +/- 6.2 years and 15.3 +/- 5.3 years at diagnosis of the first SCFE. In 53 hips, the bone and chronologic age were both known: 11.6 +/- 3.0 years bone age, 16.5 +/- 6.5 years chronologic age, p < 0.01. The age at presentation of the first SCFE ranged from 7-35 years; only those with hypothyroidism or growth hormone deficiency were < 10 years of age; all pat

"Popcorn" calcifications: a prognostic sign in osteogenesis imperfecta.

"Popcorn" calcifications appear roentgenographically as a collection of scalloped radiolucencies, each with a sclerotic margin and in some instances, with central densities. The radiographs of 46 children with osteogenesis imperfecta were reviewed retrospectively; 52% had "popcorn" calcifications at one or more sites. A study of these children indicated that these lesions are observed only during active skeletal growth, are not present at birth, occur predominantly around the knees and ankles, and are associated with irregularity or absence of the normal horizontal growth plate. Based on multi
