Incorporating Systems-Level Stakeholder Perspectives into the Design of Mobile Integrated Health Programs.

OBJECTIVES: Despite early evidence of effectiveness, cost-savings, and resource optimization, mobile integrated health (MIH) programs have not been widely implemented in the United States. System, community, and organizational-level barriers often hinder evidence-based public health interventions, such as MIH programs, from being broadly adopted into real-world clinical practice. The objective of this study is to identify solutions to the barriers impeding the implementation of MIH through interviews with multilevel stakeholders. METHODS: Using the CENTERing multi-level partner voices in Impl

Predicting Unplanned ICU Admission in Trauma: Early Warning Signs on the Floor.

BackgroundUnplanned ICU admissions (UA-ICU) is a TQIP metric associated with increased morbidity, mortality, and resource utilization. Despite identification of certain predictive factors in prior publications, gaps remain in identifying patients at-risk, particularly once in-hospital and outside the emergency room setting. Our study investigates novel predictors and emphasizes outcomes in UA-ICU for trauma.MethodsRetrospective cohort study. Single ACS-verified Level 1 Trauma Center (2019-2023). N = 140 UA-ICU after initial non-ICU hospital ward admission and 140 controls never admitted to ICU

Machine learning and network analysis for diagnosis and prediction in disorders of consciousness.

BACKGROUND: Prolonged Disorders of Consciousness (PDOC) resulting from severe acquired brain injury can lead to complex disabilities that make diagnosis challenging. The role of machine learning (ML) in diagnosing PDOC states and identifying intervention strategies is relatively under-explored, having focused on predicting mortality and poor outcome. This study aims to: (a) apply ML techniques to predict PDOC diagnostic states from variables obtained from two non-invasive neurobehavior assessment tools; and (b) apply network analysis for guiding possible intervention strategies. METHODS: The

Protocol for a randomised clinical trial of multimodal postconcussion symptom treatment and recovery: the Concussion Essentials study.

INTRODUCTION: While most children recover from a concussion shortly after injury, approximately 30% experience persistent postconcussive symptoms (pPCS) beyond 1-month postinjury. Existing research into the treatment of pPCS have evaluated unimodal approaches, despite evidence suggesting that pPCS likely represent an interaction across various symptom clusters. The primary aim of this study is to evaluate the effectiveness of a multimodal, symptom-tailored intervention to accelerate symptom recovery and increase the proportion of children with resolved symptoms at 3 months postconcussion. Improving discharge care: the potential of a new organisational intervention to improve discharge after hospitalisation for acute stroke, a controlled before-after pilot study.

OBJECTIVE: Provision of a discharge care plan and prevention therapies is often suboptimal. Our objective was to design and pilot test an interdisciplinary, organisational intervention to improve discharge care using stroke as the case study using a mixed-methods, controlled before-after observational study design. SETTING: Acute care public hospitals in Queensland, Australia (n=15). The 15 hospitals were ranked against a benchmark based on a composite outcome of three discharge care processes. Clinicians from a 'top-ranked' hospital participated in a focus group to elicit their success facto

Management of COPD, equal treatment across age, gender, and social situation? A register study.

Chronic obstructive pulmonary disease (COPD) is a progressive chronic disease where treatment decisions should be based on disease severity and also should be equally distributed across age, gender, and social situation. The aim of this study was to determine to what extent patients with COPD are offered evidence-based interventions and how the interventions are distributed across demographic and clinical factors in the sample. Baseline registrations of demographic, disease-related, and management-related variables of 7,810 patients in the Swedish National Airway Register are presented. One-th

Benign radiographic coronal synostosis after sagittal synostosis repair.

Whether cranial vault remodeling surgery for nonsyndromic, isolated sagittal suture synostosis affects the patency of initially normal, unaffected sutures is unknown. The influence of coronal and lambdoidal suture patency after cranial vault remodeling on the trajectory of subsequent cranial growth is also unknown. Disruption of normal sutural anatomy during cranial vault reconstruction could influence the incidence of secondary craniosynostosis and need for reoperation in a small proportion of these patients. We performed a retrospective review of patients younger than 1 year with nonsyndromic

Position paper on symbiotic intelligence in healthcare: Can AI help us better understand suicidal behavior and prevent suicide?

Suicide is the second leading cause of death among young people aged 10-24 worldwide, yet identifying individuals at risk remains a major challenge. In Norway, suicide rates reached their highest level in 25 years in 2024, underscoring persistent knowledge gaps in national prevention efforts and the need for innovative, interdisciplinary, and supplementary approaches. There are no simple solutions to mental ill health or suicide, and no single factor can adequately explain such complex phenomena; however, it remains crucial to examine how interacting risk factors may increase vulnerability and

Vagus nerve stimulation for conservative therapy-refractive epilepsy and depression.

Numerous studies confirm that the vagus nerve stimulation (VNS) is an efficient, indirect neuromodulatory therapy with electrically induced current for epilepsy that cannot be treated by epilepsy surgery and is therapy-refractory and for drug therapy-refractory depression. VNS is an established, evidence-based and in the long-term cost-effective therapy in an interdisciplinary overall concept.Long-term data on the safety and tolerance of the method are available despite the heterogeneity of the patient populations. Stimulation-related side effects like hoarseness, paresthesia, cough or dyspnea

In-hospital outcome after trochanteric femur fractures is related to preoperative delay but not to the time of day of the procedure: a nationwide retrospective cohort study of 7184 patients.

INTRODUCTION: Hip fractures are a growing public health concern, and efficient management is critical. Surgical timing is a key modifiable factor influencing outcomes, but fracture-specific data - particularly for trochanteric fractures - remains limited. Moreover, concerns regarding the safety of nighttime surgery may contribute to procedural delays. AIM: This study investigates how time to surgery and timing of surgery (day vs. night) affect in-hospital complication and mortality rates in patients undergoing trochanteric fracture fixation. MATERIALS AND METHODS: This nationwide registry st
