Management of venous thromboembolism: a clinical practice guideline from the American College of Physicians and the American Academy of Family Physicians.

Venous thromboembolism is a common condition affecting 7.1 persons per 10,000 person-years among community residents. Incidence rates for venous thromboembolism are higher in men, African-Americans, and increase substantially with age. It is critical to treat deep venous thrombosis at an early stage to avoid development of further complications, such as pulmonary embolism or recurrent deep venous thrombosis. The target audience for this guideline is all clinicians caring for patients who have been given a diagnosis of deep venous thrombosis or pulmonary embolism. The target patient population

Synthesis of tumor necrosis factor-alpha mRNA in bronchoalveolar lavage cells from human immunodeficiency virus-infected persons with Pneumocystis carinii pneumonia.

Bronchoalveolar lavage fluid cells from a cohort of 34 human immunodeficiency virus-infected persons with established Pneumocystis carinii pneumonia were examined for expression of tumor necrosis factors (TNF-alpha) mRNA by fluorescence in situ hybridization with an antisense riboprobe. Video image analysis was used to develop a quantitative assay that evaluates relative single-cell levels of mRNA. The resulting data were analyzed as an antisense-to-sense ratio and examined for correlation between TNF-alpha mRNA expression and other measures of disease severity. Higher levels of TNF-alpha mRNA

Prosopagnosia without topographagnosia and object agnosia associated with a lesion confined to the right occipitotemporal region.

A patient is presented who developed prosopagnosia with a recent occipitotemporal infarct in the distribution of the right posterior cerebral artery. He did not have topographical agnosia or object agnosia. He regained the ability to recognize faces of familiar persons, whereas he remained unable to identify faces of persons whom he met after the disease onset. This case demonstrates that prosopagnosia may occur as a deficit of matching a perceived face to a memory store of the face, and that the failure to recognize unfamiliar faces may be due to the inability to form memory stores of new fac

Medical complications of spinal cord injury.

Recent medical advances have greatly improved the prognosis for people who sustain spinal cord injury (about 10 000 people initially survive spinal cord injury each year in the US). Physicians may become involved with the acute care of spinal cord injured persons or care for complications which develop later. Certain conditions such as heterotopic bone formation and autonomic dysreflexia are seen almost exclusively in this group of patients; other complications such as urinary tract and soft tissue infections occur quite often. Common medical complications of spinal cord injured persons are re

Diagnostic interactions. Alcoholism and antisocial personality.

The association of alcoholism with other psychiatric disorders is important from both a research and a therapeutic point of view. In a medically hospitalized inpatient sample, we found a strong relationship between alcoholism and antisocial personality. Controlling for the overlap of diagnostic symptomatology, antisocial subjects still had a significantly higher prevalence of alcoholism than nonantisocial. The antisocial individual was more likely to be exposed to problem drinking, and once exposed, he tended to be more susceptible to developing the full alcoholism syndrome. Gender and a famil

Personalized risk prediction for breast cancer pre-screening using artificial intelligence and thermal radiomics.

MOTIVATION: Breast cancer is the leading cause of cancer deaths among women today. Survival rates in developing countries are around 50%-60% due to late detection. A personalized, accurate risk scoring method can help in targeting the right population for follow-up tests and enables early detection of breast abnormalities. Most of the available risk assessment tools use generic and weakly correlated features like age, weight, height etc. While a personalized risk scoring from screening modalities such as mammography and ultrasound could be helpful, these tests are limited to very few metropoli

The landscape of knowledge graph and large language model-augmented knowledge graph applications in dementia caregiving support: a scoping review.

BACKGROUND AND OBJECTIVES: Dementia's rising prevalence places an immense burden on caregivers. Knowledge graphs (KGs) and large language model (LLM)-augmented KGs are emerging Artificial Intelligence (AI) approaches that organize complex dementia care knowledge and enable personalized, context-aware support, yet this field remains nascent. We aimed to map and synthesize research on KGs and LLM-augmented KGs in dementia caregiving, identifying system types, applications, outcomes, challenges, and ethical considerations. RESEARCH DESIGN AND METHODS: Following the Joanna Briggs Institute (JBI)

Coping together: a qualitative exploration of challenges identified by couples living with endometriosis.

BACKGROUND: Persons diagnosed with endometriosis report deleterious effects to their physical, psychological, and relational well-being. While some couples state that these challenges extend beyond the person in pain to also affect their partners, other couples have described positive changes to their lives associated with their ability to adapt to endometriosis. These differential experiences indicate that relational dynamics are involved when coping with a condition like endometriosis, yet there have been few investigations with concurrent inclusion of both members of the couple. OBJECTIVE:
